Özet
Bu çalışmada, cavitas nasi ve oluşumlarının morfometrik gelişiminin araştırılması amaçlandı. Çalışma iki aşamada gerçekleştirildi. Birinci aşama S.U. Meram Tıp Fakültesi, Anatomi Anabilim Dalı fetus laboratuarında, 13-40 hafta aralığında, 63 ikinci trimester, 17 üçüncü trimester olmak üzere toplam 80 (40 dişi, 40 erkek) adet spontan abort insan fetusunda diseksiyon aşaması gerçekleştirildi. İkinci aşama S.U. Meram Tıp Fakültesi, Radyololi Anabilim Dalında, 12 ikinci trimester, 28 üçüncü trimester olmak üzere toplam 40 (21 dişi, 19 erkek) adet abort insan fetus BT'sinde gerçekleştirildi. Tüm ölçümler, aynı kişi tarafından yapıldı. Diseksiyon aşamasında cavitas nasi yüksekliği ve uzunluğu, konka uzunluğu, ostium pharyngeum tubae auditivae'nın spina nasalis ant.'e olan uzaklığı, ostium pharyngeum tubae auditivae'nın limen nasi'ye olan uzaklığı, ductus nasolacrimalis'in limen nasi'ye olan uzaklığı, limen nasi'nin ostium sinus maxillaris'e olan uzaklığı, limen nasi'nin ostium sinus frontalis'e olan uzaklığı,ostium sinus sphenoidalis'in limen nasi'ye olan uzaklığı, ostium sinus sphenoidalis'in horizontal düzlemle yaptığı açının ölçümü, ostium sinus sphenoidalis'in nares ile olan uzaklığı, palatum durum'un uzunluğu, palatum molle'nin uzunluğu, palatum durum ile palatum molle'nin uzunlukları oranı, choanae çapı, ostium sinus maxillaris'in ethmoid infundibulum üzerindeki açılma lokalizasyonu ve nares transvers çapı ölçüldü. Yapısal anomaliler belirlendi. Radyolojik çalışma aşamasında ise; sagittal planda konka uzunlukları, aksiyal planda septum nasi uzunluğu, septum nasi izlendiği orta hat seviyesinde anterior, orta ve posterior seviyede cavitas nasi hava boşluklarının mediolateral aralığı, choanae genişliği ölçüldü. Aksiyal ve koronal reformat görüntülerde nazal septum deviasyonu değerlendirildi, anomaliler belirlendi. Elde edilen veriler SPSS 13.0 paket programına girildi. Gruplar arası karşılaştırmada student t testi kullanıldı. Parametreler arası ilişki pearson korelasyn testi ile değerlendirildi. Cinsiyet ve lateralizasyona göre ölçülen özelliklerin birçoğunda anlamlı farklılık tespit edilmemesine rağmen, timesterler arasında anlamlı farklılık gözlendi (P>0.05). Ayrıca bu parametrelerin birbirleri ve gestasyonel yaş ile arasında korelasyon saptandı. Bu çalışmanın sonuçlarının hem medikal hem de cerrahi yaklaşımların yapılması açısından önem taşır.
In this study, it was aimed to research the morphometric development of the nasal cavity and the formation in this cavity. The study was carried out in two different phases using dissection and evaluating with radiological scanning methods. Dissection phase was performed on total 80 (40 female, 40 male) spontaneous aborted fetuses who had 13-40 week gestational age, 63 of whom were in the 2nd trimester and 17 were in the 3rd trimester, in the fetus collection of anatomy department of Meram Faculty of Medicine, Selcuk University. Radiological scanning phase was carried out on total 40 (21female, 19 male) aborted fetuses 12 of whom were in the 2nd trimester and 28 were in the 3rd trimester by obtaining axial, sagittal and coronal images with multidetector computed tomography at Radiology department of Meram Faculty of Medicine, Selcuk University. On the dissected fetuses; the length and height of nasal cavity, the length of concha, the distance of ostium pharyngeum tubae auditivae from anterior nasal spine, the distance of ostium pharyngeum tubae auditivae from nasal limen, the distance of nasolacrimal duct from nasal limen, the distance of nasal limen from maxillar sinus ostium, the distance of nasal limen from frontal sinus ostium, the distance of sphenoidal sinus ostium from nasal limen, the angle sphenoidal sinus ostium made on the horizontal plane, the distance of sphenoidal sinus ostium from nares, the length of palatum state, the length of palatum molle, the rate between the palatum state and the length of soft palate, the diameter of choanae, the dilatation localization of maxillar sinus ostium on ethmoid infundibulum , and transverse diameter of nares were measured. Structural anomalies were determined. In the radiologically scanned fetuses, the lengths of concha in sagittal plane, nasal septum length in axial plane, the mediolateral intervals of nasal cavity air spaces at anterior, middle and posterior levels where nasal septum is observed, and choanae width were measured. Nasal septum deviation was evaluated with axial and coronal reformat images, and anomalies were determined. The obtained data were loaded into SPSS 13.0 packet program. Student-t test was used to compare between the groups. The correlation between the parameters evaluated with Pearson Correlation test. Although there was no significant difference in most of the measured features according to sexes and laterization, there was considerable difference between the trimesters (P>0.05). Moreover, there was correlation between these parameters themselves and gestational age. We are of the opinion that the results of this study can be helpful to plan both medical and surgical approaches.
