Özet
Yirmi yaşındaki kadın hasta üç gündür süren ateş, titreme ve kusma yakınmalarıyla kliniğe başvurdu. Hastanın 20 gün önce Pakistan'dan geldiği öğrenildi. Fizik muayenesinde vücut ısısı 39.5 C, konjunktivalar soluk ve skleralar ikterikti. Rutin laloratuvar incelemesinde anemi vardı. Tanı kalın damla ve periferik yaymada Plasmodium falciparum trofozoitlerinin görülmesi ile konuldu. Hastaya primakinklorakin başlandı. Üçüncü günün sonunda ateşin düşmemesi, paraziteminin devam etmesi nedeniyle hastada klorakin direnci düşünüldü. Tek doz meflokin 1000 mg verildi. Hastanın 48. saatte ateşi düştü. Komplikasyonsuz düzeldi. Bu olgu klorakine dirençli, Pakistan kaynaklı P. falciparum sıtması, Türkiye1 de ender görülmesi nedeniyle sunuldu.
Twentytwo-years old woman was admitted to the hospital with complaints of fever, shiverings and vomiting for three days. It was learned that she had returned from Pakistan twenty days ago. Her physical examination revealed a body temperature of 39.5° C, pale conjuctiva and icteric sclera. Her laboratory findings showed anemia. The diagnosis was reached by showing Plasmodium falciparum trophozoids in thick and thin blood films. The patient was given chloroquine and primaquine. As after three days her fever and parazitemia continued, chloroqine resistance was thought and mefloquine (15 mg/kg) was given. Her body temperature returned to normal at the 48th hour. She improved without complication. This case is reported as P. falciparum malaria is imported from Pakistan and resistance to chloroquine is rare in Turkey.
