Özet
Çekimli vakalarda maksiller kaninlerin retraksiyonu ortodontik tedavinin esas safhalarından birisidir. Retraksiyon sonrasında kaninlerin doğru pozisyonda olmaları fonksiyon, estetik ve stabilite için önemlidir. Edgewise tekniğinde kaninler iki yolla retrakte edilmektedir. Birinci yöntem sürtünmeli yöntem olarak da adlandırılır, devamlı ark teli üzerinde kaninlere kuvvet uygulanması ile ark üzerinde gerçekleştirilen yöntemdir. İkinci yöntemde ise bölümlü arklar ve springler yardımıyla sürtünmesiz bir şekilde retraksiyon sağlanmaktadır. Sürtünmesiz sistem ile boşluk kapama yöntemi; bukkal segmental looplar veya retraksiyon springlerinin kullanımı yardımıyla bilinen M/F oranları oluşturmaya dayanmaktadır. Ayrıca optimum büyüklükte, nisbeten sabit ve önceden belirlenebilen kuvvetlerin salınımı öngörülmektedir. Çalışmanın amacı prefabrike LasVegas spring ve Ladanyi springin, elde bükümü yapılmış olan reverse closing loop ile kıyaslanması, ankraj dişler ve kaninler üzerinde meydana gelen değişimleri değerlendirmektir. Çalışmaya yaşları 12,51 ila 17,23 yıl arasında, ortalama 15,62 ±1,33 yıl olan hastalar dahil edilmiştir. LasVegas spring ve reverse closing loopun kıyaslanacağı 10 kişilik bir grup ve Ladanyi spring reverse closing loopun kıyaslanacğı 12 kişilik bir grup oluşturulmuştur. Prefabrike springler olan Ladanyi ve LasVegas springler hastaların sağ taraflarına, reverse closing loop ise sol tarafına yerleştirilmiştir. Seanslar 4 haftada bir yapılmıştır. Her hastadan retraksiyon başı, 1. ay, 3. ay ve retraksiyonun bitiş safhalarında modeller, referans telleri yerleştirilerek panoramik ve sefalometrik radyograflar alınmıştır. 1.ay, 3.ay ve bitiş periyotlarının sağ ve sol fark karşılaştırılmasında 2-bağımsız örnek testi Mann-Whitney U analizi kullanılmıştır. Diğer taraftan periyotlar arası (3 dönem) karşılaştırmada Friedman testi kullanılmıştır. Çalışmamızda kanin retraksiyonu LasVegas grubunda ortalama 9,20±1.28 ay, Ladanyi grubunda ise 5,15±0,75 ay devam etmiştir. Kanin retraksiyonu sonrasında molar dişlerde ankraj kaybı, kaninlerde meziobukkal-molarlarda meziopalatinal rotasyon oluşumu, sellanasion düzlemine göre kaninlerde intruzyon-molarlarda ekstruzyon ve overjette spontan düzelme gözlenmiştir. Ayrıca LasVegas spring uygulanmış dişler hariç kaninlerde distale devrilme, molarlarda meziale devrilme gözlenmiştir.
Canine retraction is one of the main stages of the orthodontic treatment in extraction cases. The right position of canine after retraction is important for function, esthetic and stability. There are two common ways to retract canine in Edgewise technique. One of them is frictional method that canine is retracted by applying force on a continuous arch wire. In the second method, canine is retracted by the help of segmental arches or springs; this method is called frictionless technique and canines are moved with archwire. Space closure method using frictionless technique based upon generating pre-known M/F ratios by using buccal segmental loops or retraction springs and outcome of large amount of forces that are relatively stable and pre-determinable are anticipated.The purpose of the study is the comparison of the usage effects of prefabricated LasVegas and Ladanyi spring with handmade reverse closing loop, on canines and anchorage teeth. Patients included in the study were 12,51 to 17,23 years of age the average of which is 15,62 ±1,33. Two different groups were designed. LasVegas spring and reverse closing loop group consist of 10 patients. Ladanyi spring and reverse closing loop group consist of 12 patients. The prefabricated springs of LasVegas and Ladanyi springs were inserted on patients? right side teeth and reverse closing loops were inserted on patients? left side. Cast models and reference wires installed panoramic and cephalometric radiographs were provided during the initial, after the first month, after the third month and final retraction phases. In the comparison of the difference between left and right segments, in the first, the third and finishing periods, 2 independent sample test Mann-Wihitney U analysis has been used. On the other hand, the comparison of periods Friedman Test has been used. Our results showed that canine retraction duration was 9,20±1.28 months in the LasVegas group and 5,15±0,75 months in the Ladanyi group. After the canine retaction anchorage loss in the molar teeth, mesiobuccal rotation in canine teeth, mesiopalatinal rotation in the molar teeth, according to S-N plane canine intrusion and molar extrusion, spontaneous normalization of overjet were noted. Also distal tipping of the canines and mesial tipping of the first molars except the teeth that were retraced by LasVegas spring, were observed.
