Bilateral subinguinal varicocelectomy with operative microscope was done. Serum testosterone level, total motile sperm count, 24-hour sperm motility, and libido markedly improved. The couple spontaneously conceived four months post-operatively. Varicocelectomy improved both spermatogenesis and testosterone synthesis in a man with acquired hypogonadotropic hypogonadism.
