    The association of social support structure, health status, and disability in the elderly living home [Evde yaşayan yaşlilarda sosyal destek yapisi, sa?lik durumu ve yeti?yi?ti?mi?i?li?şki?si?]

    Background and Aim: Loss of social support is strongly associated with health. Changing in frequency and quality of social support structure occurs in the elderly much more than other age groups. This descriptive study examines the social support structure in relation to health and disability status in the elderly: Materials and methods: The study group is a probability sample of 245 community-dwelling elders aged 65 and over. Social Atom Scale (SAS), Rivermead Mobility Index (RMI) and Brief Disability Questionaire (BDQ) were used to assess social support structure and disability. Kruskal Wallis test, student t test and Mann Whitney U test were used for the associations between social support structure and health and disability. Results: Being women, and widoved were associated with social support structure, whilst age, education, living alone, having social security were not. In addition, self-rated health, having chronic conditions and mobility disability were statistically significant in predicting low social support structure score, whilst physical-social disability and being in hospital bed were not. Conclusion: In conclusion, it is suggested that nurses and other health professionals should pay more attention to the social support system on health impact among elderly do well to focus attention on health impact of social support structure in elderly.

    Insomnia in rural communities and Dysfunctional Beliefs and Attitudes about Sleep as a risk factor of Insomnia

    The aim of the study was to define the prevalence of insomnia symptoms in rural regions and to determine the relationship between insomnia and the dysfunctional beliefs and attitudes about insomnia, and other risk factors. We used an own survey that included the Insomnia Severity Index and the Dysfunctional Beliefs and Attitudes about Sleep Scale. The study population was composed of individuals aged from 18 to 65 years from two rural areas. In total, 523 individuals were included in our study. Insomnia symptoms prevalence was determined as: 37.3% subthreshold insomnia, 9.2% moderate severe insomnia and 0.4% severe insomnia. Significant risk factors for insomnia were having at least one of the problems of sleep walking, sleep talking and sleep bruxism (OR: 2.163 CI: 1.320-3.543) and dysfunctional beliefs and attitudes such as worrying about insomnia (OR: 1.068 CI: 1.037-1.100). According to the multiple regression analysis, the following are also determinants of insomnia score: the worry about insomnia (?=0.380) and the use of medication (?=0.098) dimensions of Dysfunctional Beliefs and Attitudes About Sleep Index, having at least one of the problems of sleep walking, sleep talking and sleep bruxism (?=0.131) and being obese or overweight (?=0.083).

    Male condom use errors according to the female partner reports [Kadın partnerin bildirimine göre erkek kondomu kullanım hataları]

    The condom is important for the prevention of sexually transmitted infections, but its success depends on its correct use. The aim of this study was to examine male condom use errors according to the reports of the female partner. This was a descriptive study, and the study group consisted of 160 married women. A questionnaire was used to collect data. Chi-square and logistic regression tests were used for statistical analyses. Of the participants 77.5% reported one or more errors related condom use. The most common problems were "not checking the expiration date" (56.9%), "not examining for breakage after sex" (52.5%), "starting intercourse without a condom or taking it off later" (34.4%). Not receiving instruction about correct condom use was a risky factor in terms of "condom use error". Condom use errors were common and generally associated with insufficient instruction. © Gülhane Askeri Ti{dotless}p Akademisi 2010.

    Prevalence of urinary incontinence and affecting risk factors in women [Kadinlarda üriner i?nkontinans sikli?i ve etkileyen risk faktörleri]

    Objective: Urinary incontinence (UI) is a common health condition that can cause medical, psychosocial and personal hygienic problems. This was a cross-sectional investigation to identify the prevalence of UI and risk factors related with parturition in elderly women. Material and Methods: The study consisted of 543 women older than 65 years without any sampling. Data were collected by a questionnaire. Chi-square and logistic regression analysis were used for statistical analysis. Results: Elderly women had an incontinence rate of 43.6% and 41.4% of them referred to a physician with this complaint. Overall 27.6% of the participants had a history of labor longer than 24 hours and 28.9% of them had genital prolapsus. The rate of UI was higher in women with a history of labor longer then 24 hours and who had genital prolapsus than those without PROLAPSUS. There was no significant correlation between UI and child number, first birth age, last birth age, ?4 kg birth weight and episiotomy during delivery. Conclusion: Almost half of the elderly women had urinary incontinence and some characteristics related with fertility influenced the development of UI. We suggest that assessment of UI symptoms and risk factors in elderly women and counseling on conservative and curative health services are important. Copyright © 2008 by Türkiye Klinikleri.

    The validity and reliability of Turkish version of Rivermead Mobility Index (RMI) in the elderly [Rivermead Mobi?li?te İndeksi?(RMİ) Türkçe formunun yaşlilarda geçerli?li?k ve güveni?li?rl?i?i]

    Introduction: The aim of this study is to adapt the Rivermead Mobility Index (RMI) into Turkish and to test its validity and reliability. Materials and Method: After the translation procedure, reliability was assessed with test-retest technique to determine the stability of measure. Reliability was also assessed using KR-20 reliability coefficient to examine internal consistency. Validity was assessed with known-groups technique using student t test and also with comparing known disability instruments using Pearson's correlation coefficient to examine construct validity. The study was undertaken in two different settings, one was an institution where healthy and frail elders live together and the other was the region of a primary health care center which is in the service of elders living at home. Ambulatory (n= 112) and bedbound (n= 37) elders living institution and community-dwelling elders (n= 245) selected randomly. Results: Test-retest reliability coefficient (r=0.98) (p<0.001), and KR-20 reliability coefficient (r = 0.91) (p<0.001) was high and known-groups validity was statisticaly significant (p<0.001) and RMI score was correlated with Brief Disability Questionnaire-BDQ (r = 0.66) (p<0.001) score. Conclusion. The Turkish version of RMI seems to be reliable and valid measure of mobility disability in elderly people.
