Öğe A comparison of feature extraction techniques for diagnosis of lumbar intervertebral degenerative disc disease(2011) Ünal, Yavuz; Koçer, Hasan Erdinç; Akkurt, Halil EkremThe reduction of fluid that acts as shock absorber placed in lumbar intervertebral discs causes pains and this case is named as degenerative disc disease. Magentic Resonance Imaging is generally used for diagnosis of this disease by radiologists or doctors. However, due to personal errors such as fatigue, inexperience, oversight, wrong diagnosis is possible. In order to prevent these, computer-aided diagnostic (CAD) methods are mostly preferred. In this work, the performance of two different feature extraction methods is compared. The saggital MR images taken from 9 patients were feature extracted by using grey level co-occurrence matrix (GLCM) and average absolute deviation (AAD) methods. The obtained feature vectors were classified by using multi-layered perceptron (MLP) artificial neural networks. © 2011 IEEE.Öğe Evaluation of Sexual Dysfunction in Females With Ankylosing Spondylitis(TURKISH LEAGUE AGAINST RHEUMATISM, 2016) Akkurt, Halil Ekrem; Yilmaz, Halim; Yilmaz, Sema; Parlak, Lutfiye; Ordahan, Banu; Salli, AliObjectives: This study aims to evaluate sexual function in females with ankylosing spondylitis (AS), compare them with healthy controls, and demonstrate the effects of AS on female sexual functions. Patients and methods: Fifty-four AS patients (mean age 39.33 +/- 8.57 years; range 20 to 55 years) and 56 similar aged healthy controls were included in the study. Depression levels and sexual functions of all participants were evaluated by the Beck Depression Inventory and Female Sexual Function Index (FSFI), respectively. Other assessment methods included the visual analog scale for pain, fatigue, and importance of sexual life; Bath Ankylosing Spondylitis Disease Activity Index for disease activity, Bath Ankylosing Spondylitis Functional Index for functionality, Bath Ankylosing Spondylitis Metrology Index for spinal mobility, and Short Form-36 (SF-36) for quality of life. Results: Total FSFI and all FSFI subscale scores and number of weekly sexual intercourse were significantly lower while Beck Depression Inventory score was significantly higher in females with AS when compared to controls. In AS patients with depression, total FSFI score and FSFI subscales scores of desire and arousal were significantly lower than those without depression. In females with AS, there were negative correlations between total FSFI score and duration of complaint, Beck Depression Inventory score, Bath Ankylosing Spondylitis Metrology Index score, visual analog scale score, age, and duration of marriage while positive correlations existed between total FSFI score and visual analog scale importance of sexual life score and number of weekly sexual intercourse, SF-36 fatigue, SF-36 social function, SF-36 pain, and SF-36 mental component scores. Conclusion: Sexual dysfunction was more common in female AS patients without marked impairment in body image and hip involvement when compared to normal population. Sexual problems which are generally neglected should be handled regardless of disease activity when evaluating patients with AS and establishing a treatment plan.Öğe The evaluation of sexual function in women with stroke(WOLTERS KLUWER MEDKNOW PUBLICATIONS, 2017) Yılmaz, Halim; Gümüş, Haluk; Yılmaz, Sema D.; Akkurt, Halil Ekrem; Odabaş, Ömer FarukBackground: Although very common, sexual dysfunction is a neglected disorder in women with stroke. Aim: To investigate the physical, psychological and sexual changes in women with stroke, and to determine the factors related to these changes. Settings and Design: This descriptive study was conducted at a tertiary care university hospital. Material and Methods: A total of 112 women (51 stroke patients, 61 healthy controls) were included in the study. The independence level of stroke patients was evaluated with the Modified Rankin Scale (mRS); the severity of stroke and the clinical status of patients after stroke, with the National Institute of Health Stroke Scale (NIHSS); depression levels, with the Beck Depression Inventory (BDI); and, patients' sexual function, with the Female Sexual Function Inventory (FSFI). Statistical Analysis: The Kolmogorov-Smirnov, the student's t, the Chi-square and the Mann- Whitney-U tests, and the Spearman's correlation analysis were performed in the appropriate conditions. Results: While the scores of total FSFI and FSFI subgroups in women with stroke were lower than in healthy controls, BDI scores were found to be higher than those of controls. A negative correlation was detected between the total FSFI scores, and BDI, MRS, NIHSS, age, duration of marriage and number of children in women with stroke, while a positive correlation was found between total FSFI scores and educational level. Conclusion: Sexual dysfunction is a commonly seen disorder in women with stroke, and is influenced by the severity of disease, level of dependence and accompanying depression. Therefore, evaluation of sexual dysfunction, depression and levels of physical function should be done in women with stroke during their follow up.Öğe Is pain the only symptom in patients with benign joint hypermobility syndrome?(SPRINGER LONDON LTD, 2015) Albayrak, İlknur; Yılmaz, Halim; Akkurt, Halil Ekrem; Sallı, Ali; Karaca, GültenThe aims of this study were to evaluate pain, depression level, fatigue, sleep, and quality of life (QoL) among patients with benign joint hypermobility syndrome (BJHS) and to compare their results with those of healthy controls. The study involved 115 patients and 114 healthy volunteers. Pain level was rated using visual analogue scale (VAS) for all patients. Depression level, fatigue, sleep quality, and QoL of all the participants were evaluated by the Beck Depression Inventory (BDI), the Checklist Individual Strength (CIS), the Pittsburgh Sleep Quality Index (PSQI), and the Short Form-36 (SF-36), respectively. VAS value was 6.29 +/- 0.94 in the patient group. Comparison of two groups showed that there were statistically significant differences between the patient group and the control group with respect to BDI, total CIS, PSQI scores, SF-36 subscales (physical function, role physical, bodily pain, general health, role emotional, and mental health), and mental component summary (p < 0.001). While pain is the predominant symptom among BJHS patients, depression, fatigue, impaired sleep, and QoL also commonly occur. Thus, all of these components should be taken into account when assessing patients with BJHS.Öğe Patellofemoral ağrı sendromlu hastalarda EMG aktiasyonlarının değerlendirilmesi ve konsantrik izokinetik egzersizin etkisi(Selçuk Üniversitesi Tıp Fakültesi, 2006) Akkurt, Halil Ekrem; Özerbil, M. ÖnderAmaç: PFAS'h bireylerde kas kontraksiyonlan esnasında EMG incelemesiyle VMO kasının VL kasma göre başlangıç aktivasyon zamanlarında gecikme olup olmadığının araştırılması ve bu hastalara yaptırılan izokinetik egzersizin başlangıç aktivasyon zamanlarına etkisinin değerlendirilmesi. Hastalar ve Metot: Çalışmaya ön diz ağrısı şikayeti ile başvuran ve PFAS tanısı alan 30 hastanın 60 dizi alındı. 13 hastanın her iki dizinde de şikayet mevcuttu. Tek taraflı şikayeti olan 17 hastanın sağlam dizleri kontrol grubunu olarak alındı. Tüm hastaların her iki dizi için egzersiz ve ölçümleri software destekli izokinetik test ve egzersiz sistemi(Biodex System 3 Pro, ABD) ve EMG ölçümleri EBNeuro marka 4 kanallı EMG cihazı ile yapıldı. EMG ölçümleri için; tam ekstansiyon 0° kabul edilmek üzere hastalardan izokinetik dinamometre ve bilgisayar destek sistemi kontrolünde 15o,30°,45o,60° ve 75° lerde, her bir açıda üçer defa maksimal diz ekstansiyonu yapmaları istendi ve bu esnada VMO ve VL'nin EMG aktiviteleri kaydedildi. EMG de kontraksiyon başlangıç zamanlan arasındaki fark bütün açılarda her 3 ölçüm için hesaplandı ve ortalaması kaydedildi. Kas kuvveti ölçümü için; hastalara tedavi öncesi ve tedavi soması, 15°,30°,45o,60° ve 75°'lerde maksimal izometrik kuadrispes kuvvetleri N-M olarak ölçüldü ve kaydedildi. Ağrı ve Fonksiyonel kapasitenin değerlendirilmesi için; SF-36 değerlendirme formu ve 0-10 vizüel analog skala(VAS) ölçümleri kullanıldı. Ekersiz programı olarak; Hastalara, 6 hafta boyunca haftada 3 gün, tam diz ekstansiyonu 0° kabul edilmek üzere 0°-30° eklem hareket açıklığında izokinetik konsantrik-konsantrik egzersiz yaptırıldı. Bulgular: Hastaların 8'i erkek, 22'si kadındı. Yaş ortalaması 34,73 ± 8,42 yıldı. 43 PFAS'lu diz ölçümleri 17 kontrol diz ile karşılaştırıldığında, 15° ve 30°'lerde daha belirgin olarak 15°,30o,45°'lerde VMO, EMG aktivite başlangıç zamanında VL'e göre istatistiksel olarak anlamlı düzeyde gecikme saptandı (p<0,001). 60° ve 75° ölçümlerinde VMO veVL arasında anlamlı fark yoktu (p>0,05). Egzersiz sonrası bu gecikmenin kaybolduğu ve kontrol grubu ile eşitlendiği görüldü. PFAS'lu dizlerdeki tüm açılardaki maksimal izometrik kuadriseps kas kuvvetleri egzersiz öncesine göre egzersiz sonrasında istatistiksel olarak anlamlı artış sağlandı (pO.001). Hastalardaki tüm VAS değerleri egzersiz sonrası ölçümlerde, egzersiz öncesi ölçümlere göre istatistiksel olarak anlamlı azalma saptandı (p< 0.001), fonksiyonel değerlendirmede kullanılan SF-36; egzersiz sonrası ölçümlerde, egzersiz öncesi) ölçümlere göre istatistiksel olarak anlamlı artış saptandı(p< 0.001). 52 Sonuç: PFAS'lı hastalara verdiğimiz egzersizler kuadriseps kas gücünde artma sağladığı gibi, VMO kasnıdaki kontraksiyona başlamadaki gecikmeyi düzeltmiştir. Bu bulgular bize PFAS'lı hastalara verilecek egzersiz programında patellofemoral stresin en az olduğu eklem hareket aralığında yapılan ve VMO'ya spesifik olarak güçlendiren egzersizlerin bulunması gerektiğini göstermiştir.Öğe Sexual Dysfunction in Women with Lumbosacral Disc Disease(2017) Akkurt, Halil Ekrem; Yılmaz, Halim; Yılmaz, Sema Dereli; Ordahan, Banu; Şen, Zafer; Gürbüz, Cemal; Göksu, HamitObjective: The aim of this study is to investigate the effects of lumbar disc disease(LDD) on sexual functions of women at reproductive period. Material and Methods: Forty-threewomen with LDD at reproductive period and 45 healthy controls at reproductive period were enrolledto the study. Patients and controls were evaluated with the Oswestry Disability Scale (ODS),Beck Depression Inventory (BDI), Visual Analogue Scale (VAS) and Female Sexual Function Inventory(FSFI). Results: Both groups had similar traits in terms of age, body mass index, durationof marriage, number of weekly intercourse before LDD, number of living children, monthly incomelevel and family structure. The total FSFI score, the priority of sexual life score, number ofweekly sexual intercourses were found to be lower than control group. Negative correlation wasfound between total FSFI score and BDI, ODS and VAS scores. Conclusion: In women with LDD,sexual function is negatively influenced and seems to be associated with increased disease activity,pain and accompanying depression level.Öğe Sexual Life of Women with Multiple Sclerosis: A Qualitative Study(ROUTLEDGE JOURNALS, TAYLOR & FRANCIS LTD, 2017) Yılmaz, Sema D.; Gümüş, Haluk; Odabaş, Ömer Faruk; Akkurt, Halil Ekrem; Yılmaz, HalimObjectives: To define the thoughts, emotions, experiences and perceptions of women with multiple sclerosis (MS) and to determine sexual, physical, and emotional effects of MS on patients. Methods: Twenty-one women were included into this qualitatively designed study. Results: Three baseline themes were defined as uncertainty arising from complaints due to numbness in extremities and loss of sense, balance or vision, alterations in sexual life, and inability as a result of deformities in marital status and gender role. Conclusion: Healthcare providers should evaluate MS patients meticulously as to sexual life and refer such patients to appropriate health centers.
