Öğe Comparison of gingivectomy procedures for patient satisfaction: Conventional and diode laser surgery(Selçuk Üniversitesi, 2017) Öncü, Elif; Erbeyoğlu, Ahmet Afşin; Alan, RaifBackground: To compare the use of the 940 nm diode laser with conventional surgery in the management of soft tissue in gingivectomy procedures in terms of patient satisfaction. Methods: This controlled clinical trial was conducted on 20 healthy patients who needing esthetic-only gingivectomy in the anterior maxilla. The patients were randomly divided into two groups of 10 each: experimental (diode laser-assisted surgery) and control (traditional surgery using scalpels). The bleeding rate following the surgery was assessed using the bleeding criteria established by the World Health Organization. The postsurgical discomfort level was recorded using visual analog scales (VAS), surgery time was evaluated for each operation and postoperative analgesic requirements were evaluated and compared. Results: In control groups, anesthesia requirements were found statistically higher during surgery (p<0.001). The average bleeding rates were 1.32 and 0.24 in the conventional and laser groups, respectively (p<0.001). Experimental patients had no postsurgical pain but in the control group, VAS pain level was found higher. The difference between VAS values in each groups were significant (p<0.001). There was no significant difference for surgery time between the treatment type. Conclusion: This study shows that the diode laser has a great advantage over conventional surgery in the gingivectomy procedures.Öğe Comparison of removable dentures: Patient satisfaction and oral health-related quality of life(Selçuk Üniversitesi, 2019) Alan, Raif; Çelebi, HakkıBackground: Health-related quality of life is significantly influenced by oral health of individuals. This study aimed to compare patient satisfaction and oral health–related quality of life (OHRQoL) among patients using complete dentures (CDs) and partial dentures (PDs). Materials and Methods: The study was carried out patients with complete or partial edentulous who refered to the Necmettin Erbakan University, Faculty of Dentistry between March 2015 and February 2017. The quality of life of the patients was assessed using a questionnaire including the Oral Health Impact Profile (OHIP-14). The patients were divided into the following groups according to their prosthesis type and location: 1) Group I: CDmaxmand, 2) Group II: CDmax-PDmand, 3) Group III: PDmax-CDmand and 4) Group IV: PDmax-mand. Sociodemographic datas, prosthesis age, and effects of prosthetic location on the OHRQoL were also examined. Results: Statistical analyses showed that age, gender, educational status and duration of prosthesis usage did not have an impact on the OHIP-14 scores of patients (p>0,05). Physical pain scores of patients in Group I and Group III were significantly higher than those of patients in Group IV (p<0.05). In addition, patients in Group III were found to have higher scores for social handicap compared with patients in Group II and, patients in Group I and Group III have also higher scores than those of patients in Group IV (p<0.05). Conclusion: Prosthetic rehabilitation with CD and PD has an important impact on improving the OHRQoL of patients. The OHRQoL can be further improved with regular dental control and/or the need to replacement of prosthesis at regular intervals.Öğe Effect of Prosthodontic Rehabilitation on Perioral Units in Edentulous Patients(Selçuk Üniversitesi, 2021) Çelebi, Hakkı; Alan, Raif; Kartal, ElifBackground: Complete edentulous patients experience loss in fundamental mastication functions, the progressive absorption of the alveolar bone, and variation in facial appearance. Complete dentures (CD) fundamentally target edentulous patients and not only increase functionality but also restore the appearance of the face. This study aims to evaluate the effect of rehabilitation with removable CD on perioral wrinkles in complete edentulous patients, in line with growing aesthetic demand. Methods: Patients who had CD rehabilitation performed due to total edentulism were photographed before and after rehabilitation from the same angles to evaluate the perioral soft tissue changes. The photographs were scored using verified ranking scales and through evaluation by two observers. To evaluate the repeatability of the scales, measurements were repeated with the same method one month later. Results: When the effect of prosthetic rehabilitation administered successfully on perioral aesthetics and lip volume was analyzed, a significant improvement was observed in all other parameters evaluated, except for in the labiomental crease. The improvement in the labiomental crease was not statistically significant. In addition, re-measurement reliability was high (?0.80) for all four perioral units (nasolabial folds, corners of mouth, labiomental crease and upper radial lip). In addition, all correlations were statistically significant (p<0.05). Conclusion: Rehabilitation with CD contributed positively to the development of the perioral wrinkles and lip volume, which are also observed as a result of edentulism.Öğe Peri-implant crevicular fluid levels of cathepsin-K, RANKL, and OPG around standard, short, and mini dental implants after prosthodontic loading(KOREAN ACAD PERIODONTOLOGY, 2015) Alan, Raif; Marakoglu, Ismail; Haliloglu, SeyfullahPurpose: Despite the high success rates of endosseous dental implants, their placement is restricted according to the height and volume of bone available. The use of short or mini dental implants could be one way to overcome this limitation. Thus, this study aimed to compare standard, short, and mini dental implants with regard to associated clinical parameters and pen-implant crevicular fluid (PICF) levels of cathepsin-K (CTSK), RANK ligand (RANKL), and osteoprotegerin (UPS), after prosthodontic loading. Methods: A total of 78 non-submerged implants (Euroteknika, Aesthetica(+2), Sallanches, France) were installed in 30 subjects (13 male, 17 female; range, 26-62 years) who visited the clinic of the Periodontology Department Faculty of Dentistry, Selcuk University. Sampling and measurements were performed on the loading date (baseline) and 2, 14, and 90 days after loading. Assessment of the pen-implant status for the implant sites was performed using the pocket probing depth (PPD), modified plaque index, modified gingival index, modified sulcular bleeding index, and radiographic signs of bone loss. PICF samples collected from each implant were evaluated for CTSK, RANKL, and UPS levels using the ELISA method. Keratinized tissue and marginal bone loss (MBL) were also noted. Results: Clinical parameters statistically significantly increased in each group but did not show statistical differences between groups without PPD. Although implant groups showed a higher MBL in the upper jaw, only the standard dental group demonstrated a statistically significant difference. At 90 days, the OPG:sRANKL ratio and total amounts of CTSK for each group did not differ from baseline. Conclusions: Within the limitations of this study, both short and mini dental implants were achieving the same outcomes as the standard dental implants in the early period after loading.Öğe Standart, kısa ve mini dental implantlarda protetik yükleme sonrası peri-implant oluğu sıvısında katepsin-k, rankl ve opg düzeyleri(Selçuk Üniversitesi Sağlık Bilimleri Enstitüsü, 2015) Alan, Raif; Marakoğlu, İsmailKemik içi dental implantların yüksek başarı oranlarına rağmen, mevcut kemiğin yüksekliği ve hacmi ile ilgili olarak yerleştirilmelerinde kısıtlamaların olduğu savunulmaktadır. Kısa ya da mini dental implantların kullanımı, bu kısıtlamanın üstesinden gelmek için bir seçenek olabilir. Bu nedenle mevcut çalışmada, standart, kısa ve mini dental implantlarda protetik yükleme sonrası peri-implant oluğu sıvısı (PİOS)'nda katepsin-K, RANKL ve OPG düzeylerinin değerlendirilmesi amaçlanmıştır. Selçuk Üniversitesi Dişhekimliği Fakültesi Periodontoloji Anabilim Dalı kliniği'ne müraacat eden toplam 30 bireyde (13 erkek, 17 kadın; yaş aralığı, 26-62), 78 implant kullanıldı. Bireyler, 6-15 mm uzunluk ve 2,7-4,8 mm arasında bir çapa sahip standart, kısa ve mini implantlar ile tedavi edildi. Örnekleme ve ölçümler, yükleme günü ve yükleme sonrası 2., 14. ve 90. günlerde yapıldı. İmplant bölgeleri için peri-implant durumun değerlendirilmesi sondlama cep derinliği (SCD), modifiye plak indeksi, modifiye gingival indeks, modifiye sulkuler kanama indeksi ve kemik kaybının radyografik bulguları kullanılarak gerçekleştirildi. Katepsin-K, RANKL ve OPG düzeyleri, her bir implanttan toplanan PİOS örnekleri ile ELİSA yöntemi kullanılarak değerlendirildi. Ayrıca kuron-implant (K/İ) oranları, keratinize doku miktarı (KDM) ve protez çeşidi, ark ve sigara içme durumu ile ilişkili marjinal kemik kaybı (KKM) da kaydedildi. Klinik parametrelerde her bir grupta istatistiksel olarak anlamlı artış izlenmiştir ancak, SCD haricindeki parametreler gruplararası istatistiksel olarak anlamlı fark göstermemiştir. İmplant grupları maksillada daha yüksek KKM göstermesine rağmen, sadece standart grupta istatistiksel olarak anlamlı sonuç gözlendi (p<0.005). Diğer yandan gruplardaki K/İ oranı, KKM ve KDM ile korelasyon göstermemektedir. Yükleme günü ile karşılaştırıldığında 90. günde, gruplardaki OPG/sRANKL oranı ve katepsin-K total miktarlarında farklılık bulunmamaktadır. Çalışmanın limitleri dahilinde yükleme sonrası hem kısa hem de mini dental implantlarda, standart dental implantlara benzer sonuçlar elde edilmiş olup, bu konuyla ilgili daha fazla uzun dönem takip çalışmalarına ihtiyaç olduğu düşünülmektedir.
