Öğe Comparison of the effects of an adhesion barrier and chitin on experimental epidural fibrosis [Deneysel epidural fibroziste adezyon bariyeri ile chitin'in etkinli?inin karşlaştırılması](Turkish Neurosurgical Society, 2010) Keskin F.; Esen H.Aim: Epidural fibrosis is an important factor for postoperative failed back syndrome development and causes clinical complaints in 6-25% of cases. An effective treatment modality has not been found yet. The aim of this study is to investigate the anti-adhesive effects of a novel agent chitin and compare these effects with a popular adhesion barrier collagen matrix. Mat erial and Methods: 21 rabbits were allocated into three groups including 7 rabbits each. L5 total laminectomy was performed to all groups. No treatment was given to Group 1 (Control group). Collagen matrix was used in Group 2 and chitin was used in Group 3. Six weeks later all rabbits were sacrificed and the laminectomy areas were entirely resected and investigated histopathologically. Results: He and Revel grade III epidural fibrosis was detected in the control group. Statistically significant reduction of epidural fibrosis was achieved with both of the anti-adhesive agents, collagen matrix and chitin, when compared with the control group (p<0.05). The results were not different between treatment groups (p>0.05). Conclusion: The novel agent chitin was found effective for preventing epidural fibrosis and this effect was not significantly different from the collagen matrix. In light of our findings we suggest that chitin is an effective alternative for adhesion barriers.Öğe The evaluation of the biopsy results taken under colposcopy in patients with abnormal cervicovaginal smear results [Anormal servikovajinal smear saptanan hastalarda kolposkopi eşli?inde alinan biyopsi sonuçlarinin de? erlendirilmesi](2011) Yazici F.; Tazegül A.; Esen H.; Çelik Ç.Objective: To evaluate the results of biopsies taken during colposcopy in patients with abnormal cervicovaginal smear results. Material and Methods: Cervical biopsies were taken during colposcopy examination of 111 patients whose cervicovaginal smears showed abnormal epithelial cells. 111 patients were prospectively evaluated. Their socio-demographic data, age, gravida, parity, socioeconomic status, educational status, age at first intercourse, number of partners, and being or not being a smoker were investigated. Results: According to the classification of Bethesda, squamous cell abnormalities were detected in 96 (86.5%) patients and 15 (13.5%) patients had abnormal glandular cells. When the biopsy results of the patients were evaluated, out of 40 ASC-US cases CIN-I was detected in 5 (%12.5) cases, CIN-II in 1 (%2.5) case, CIN-III in 2 (5%) cases and invasive cancer was detected in 1 (2.5%) case. Out of the 3 patients with detected ASC-H, in 1 (33.3%) patient carcinoma in situ and in the other 2 (66.7%) patients invasive cancers were detected. In the 39 LSIL patients, 28 (71%) had normal pathology results and 1 (2.6%) had carcinoma in situ. Out of the 14 patients with detected HSIL, 4 (28.6%) patients were diagnosed with inflammatory changes and 5 (35.7%) with invasive cancer. 10 (66.7%) patients out of the 15 whose smear tests showed AGUS were diagnosed with normal pathological findings, and another 3 (20%) were diagnosed with invasive cervical cancer. Conclusion: Colposcopic examination must be performed in the presence of glandular cell and high-grade cytologic abnormalities. In the low-grade lesions, in order to not miss out a preinvasive lesion or in order to avoid unnecessary invasive applications, the medical examinations should be carried out with the aid of colposcopy or the patients should be chosen carefully for the follow-ups. Copyright © 2011 by Türkiye Klinikleri.Öğe Spontaneous retroperitoneal hemorrhage presenting as hemoperitoneum secondary to renal cyst rupture in a peritoneal dialysis patient with acquired cystic kidney disease(Medknow Publications, 2015) Biyik Z.; Solak Y.; Gaipov A.; Ozbek O.; Esen H.; Turk S.Spontaneous retroperitoneal hemorrhage (SRH) is a rare and potentially fatal condition. Acquired cystic kidney disease (ACKD) may cause SRH in hemodialysis patients. However, presentation of retroperitoneal hematoma as hemoperitoneum in peritoneal dialysis (PD) patients is exceedingly rare. We report a 44-year-old male PD patient who presented with hemoperitoneum secondary to retroperitoneal hematoma. The reason of SRH was rupture of the cysts of ACKD. The patient underwent unilateral nephrectomy with subsequent disappearance of hemoperitoneum. The importance of this case lies in the fact that the patients who have been receiving dialysis for a long time should be under surveillance in terms of ACKD development and potential associated complications such as cyst hemorrhage and malignancy.
