In this study, the spectral features of S1 and S2 component of the mechanical heart valve sounds obtained after aortic valve replacement (AVR) and mitral valve replacement (MVR) are compared. For this aim, AR methods are applied on the S1 and S2 components of heart sounds taken from 68 recording of 8 patients with AVR and 87 recordings from 11 patients with MVR, thereby yielding power spectrum of the heart sound and six different features are obtained from power spectrum of the heart sounds. Using these features, S1 and S2 components are compared. ©2010 IEEE.Öğe Benign Mature Cystic Teratoma of the Anterior Mediastinum Leading to Heart Failure: Report of a Case(Springer-Verlag, 2003) Özergin, Ufuk; Görmüş, Niyazi; Arıbaş, Olgun Kadir; Durgut, Kadir; Yüksek, TahirA 12-year-old male child was referred to our clinic for the surgical treatment of an anterior mediastinal mass, suspected to be a thymic cyst, which was considered to potentially lead to cardiac failure. The mass was resected completely with a median sternotomy. The postoperative course was uneventful. A pathological examination revealed a mature cystic teratoma of anterior mediastinum which is a very rare cystic tumor at this location. The management and treatment principles of children with thoracic trauma were discussed with the data reported in the literature. Methods: Of the chest injury diagnosed in 1653 patients, 225 were children in the last 17-year period. There were 199 boys (88.44%) and 26 girls (11.55%). The most common causes were blunt injuries in 135 cases (60%), stab wounds in 67 cases (29.77%) and gunshot wounds in 22 cases (9.77%). Results: Out of 225, 217 patients were treated conservatively and eight patients were treated surgically. There was no mortality and morbidity. Conclusions: The prevalence of chest trauma in children due to blunt injuries is high in Turkey. Extremity injury is thought to be the most commonly associated extra-thoracic injury. However, thoracic trauma in children can be managed conservatively in most of the cases.Öğe Clinical Features and Surgical Treatment of Cardiac Myxoma: Report of 18 Cases(2002) Durgut, Kadir; Görmüş, Niyazi; Özülkü, Mehmet; Özergin, Ufuk; Özpınar, CevatMyxomas are the most common benign primary cardiac tumors. As many patients with cardiac myxoma suffer from cerebral or systemic embolism, which are serious complications, diagnosis of the tumor is vital. Between 1990 and 2000, 18 patients (6 males, 12 females), aged 24 to 73 years (mean, 55.3 years), were operated on for cardiac myxoma. The most common location of the myxoma was the left atrium (78%), and the transseptal surgical approach was preferred (78%) as it allows total resection of the left atrial myxoma along with its pedicle. Carney complex, a familial autosomal dominant form of atrial myxoma, was not found in any of the patients. There had been no operative or postoperative mortality and morbidity. The mean postoperative follow-up period was 5 years (mean, 1 to 10 years). No recurrence had been seen. We believe that the transseptal approach, in allowing total resection of the myxoma, prevents recurrence.Öğe Cold Abscess of the Chest Wall as an Unusual Complication of BCG Vaccination(Elsevier Science Bv, 2002) Arıbaş, Olgun Kadir; Kanat, Fikret; Görmüş, Niyazi; Türk, EmelBacillus-Calmette-Guerin (BCG) vaccination often results in local adverse effects; however, serious or long-term complications are rare. The involvement of sternum among skeletal BCG osteomyelitis is a rarely seen complication of BCG vaccination. Such a complication may confuse with a chest wall tumor and a surgical intervention may be needed for the definite diagnosis. A 9-month-old infant who had a parasternal cold abscess in the anterior chest wall and sternal osteomyelitis of tuberculosis in the late period of BCG vaccination of whom the etiological diagnosis was histopathologically confirmed after surgery is presented and the preoperative diagnostic problems are discussed.Öğe Comparison of Autologous Basilic Vein Transpositions Between Forearm and Upper Arm Regions(Springer-Verlag, 2003) Görmüş, Niyazi; Özergin, Ufuk; Durgut, Kadir; Yüksek, Tahir; Solak, HasanArteriovenous fistulas are crucial for hemodialysis performances in patients with end-stage renal disease. In this report, we compare the results of basilic vein transposition in the forearm and upper arm regions. Autologous basilic vein transpositions were used in 20 patients with end-stage renal failure between May 2000 and January 2002. In 10 cases the basilic vein transposition was in the upper arm region for construction of a brachiobasilic fistula (group 1); in the remaining 10 cases it was in the forearm region for creation of a radiobasilic fistula (group 2). The mean age of the patients in group 1 was 53.6 years, and in group 2 it was 47.3. At the end of the follow-up period (mean, 10 months) patency rates in group 1 were 80% and in group 2, 90%. In group 2, early-term thrombosis, which could be treated easily with second intervention and anticoagulation treatment (low-molecular-weight heparin), was the most common complication (10%). For patients who have unsuitable cephalic vein and require long-term hemodialysis, transposition of the basilic vein in the forearm region (under the elbow) can be a good secondary choice for access, as can a brachiobasilic fistula in the upper arm region.Öğe Comparison of Coronary Angiographic and Exercise Testing Parameters of Silent and Symptomatic Ischemic Patients Without Previous Myocardial Infarction(2001) Telli, Hasan Hüseyin; Temizhan, Ahmet; Cin, Gökhan V.; Görmüş, Niyazi; Bük, SalihAmaç: Noninvaziv kardiyak egzersiz testlerinde tespit edilen sessiz iskemi daha hafif bir koroner arter hastalığının göstergesi midir ya da egzersiz parametreleri Semptomatik İskemili hastalandan farklı mıdır sorusunun cevabı tam olarak bilinmemektedir. Bu nedenle sessiz İskemili ve semptomatik iskemili hastaların aterosklerotik risk faktörlerini, ejeksiyon fraksiyonlarını, anjiografik ve egzersiz parametrelerini karşılaştırmayı amaçladık. Yöntem: Egzersiz testi laboratuarımıza başvuran klinik olarak stabil olan ve egzersiz testine pozitif cevap veren 138 hasta çalışmaya alındı ve hepsine koroner anjiografi yapıldı. Hastaların koroner arter hastalık skorları, ejeksiyon fraksiyonları, egzersiz test parametreleri ve aterosklerotik risk faktörleri analiz edildi. Bulgular: Asemptomatik ST çökmesi olan 80 hastanın 50O'sinde (%57), semptomatik ST çökmesi olan 50 hastanın da 42'sinde (%84) anjiografik olarak koroner arter hastalığı tespit edildi (p0.01). Sessiz iskemisi olanlarda sigara ve aile öyküsü daha yüksek oranda tespit edildi (p0.05). Diğer risk faktörleri farklı değildi. Sessiz İskemili hastaların egzersiz testinde ulaştıkları total egzersiz süresi, maksimal kalp hızı ve double product değerleri daha yüksekti (sırasıyla p0.05, p0.01, p0.05). Sol ön inen koroner arter lezyonu sessiz İskemili hastalarda daha yüksek oranda saptanırken ejeksiyon fraksiyonu, koroner arter skoru, hastalıklı damar sayısı, sağ koroner, sol sirkumfleks ve sol ana koroner arter hastalık sıklığı iki gurup arasında anlamlı farklılık göstermiyordu. Sonuç: Tanısal amaçlı yapılan egzersiz testinde, göğüs ağrısı ile birlikte saptanan ST segment çökmesinin tekbaşına ST segment çökmesine göre koroner arter hastalığının daha güçlü bir göstergesi olduğunu düşünmekteyiz. Sessiz İskemili hastaların anjiografi ile tespit edilen koroner arter hastalık ciddiyeti semptomatik İskemili hastalarla oldukça benzerdir ve egzersiz kapasitelerinin daha yüksek olması koroner arter hastalığının daha hafif olduğunu anlamına gelmemektedir.Öğe Concomitant Heart Surgery with Pulmonary Hamartoma Resection: A Case Report(Selçuk Üniversitesi, 2022) Tanyeli, Ömer; Görmüş, Niyazi; Orhan, AtillaConcomitant lesions of the heart and lung are rare, but when present, they pose a therapeutic challenge to surgeons. A combined procedure clears the need for a second major procedure, improving outcomes and providing economic benefits. However, cardiopulmonary bypass may adversely affect the natural history of pulmonary masses when malignancy is suspected. To avoid these suspects, off-pump techniques may be preferred in suitable patients. This article presents a case of simultaneous off-pump coronary artery bypass grafting and pulmonary hamartoma resection in a 53-year-old man who detected a lung mass during preoperative preparation.Öğe Cost-Effective and Time-Saving Surgical Treatment of Pulmonary Hydatid Cysts With Multiple Localization(Springer-Verlag, 2002) Ceran, Sami; Sunam, Güven Sadi; Görmüş, Niyazi; Solak, Hasan; Şahin, MustafaPurpose. The most frequent anatomic locations of hydatid cysts are the liver and lungs. Because there is no effective medical therapy against this parasitic disease, surgery is the treatment of choice. The aim of this retrospective study was to compare the cost and effect of a one-stage operation with those of two- or three-stage operations in the treatment of lung hydatid cysts with multiple localizations. Methods. We evaluated 364 patients who underwent surgical treatment for hydatid cysts, all of whom had multiple localizations. To avoid two- or three-staged operations, we performed median sternotomy, simultaneous bilateral thoracotomy and unilateral thoracotomy with a transdiaphragmatic approach. Results. For the treatment of 460 hydatid cyst localizations in 364 patients, a collective 381 operations were performed. The number of operations and periods of hospitalization were reduced. Conclusion. A one-stage surgical procedure for bilateral lung and liver hydatid cysts is superior to the traditional two- and three-stage operations because it reduces morbidity, hospital stay, and cost.Öğe Effect of Cooling on the Responses of Human Saphenous Vein to Fentanyl, Remifentanil and Sufentanil(Blackwell Publishing, 2006) Şahin, Ayşe Saide; Duman, Ateş; Günaydın, İshak Gürsel; Şahin, Tahir Kemal; Görmüş, Niyazi; Duman, İpekWe studied the vasodilatory effects of fentanyl, remifentanil and sufentanil on the human saphenous vein strips at 37, 32 and 28 degrees C. Fentanyl produced concentration-dependent relaxation of human saphenous vein strips precontracted with 5-hydroxytryptamine (5-HT) at every temperature studied. Compared with vein strips at 37 degrees C, relaxant responses to each one concentration of fentanyl were significantly reduced at 32 and 28 degrees C. Remifentanil relaxed vein strips in a concentration-dependent way and the relaxation for all concentrations were significantly greater at 32 and 28 degrees C compared with 37 degrees C. Sufentanil produced concentration-dependent relaxation in saphenous vein strips precontracted with 5-HT. These relaxant responses were similar at 32 degrees C compared with 37 degrees C. When bath temperature was lowered from 37 to 28 degrees C, the relaxant responses to sufentanil were significantly reduced. In summary, the present study suggests that cooling reduces the relaxation caused by fentanyl and sufentanil on human saphenous veins but augments the relaxation with remifentanil. The augmented vasodilatory effect of remifentanil with cooling may be useful on systemic vascular resistance and organ preservation under hypothermic conditions like cardiopulmonary bypass surgery.Öğe Endovascular stent-graft placement for the emergency treatment of ruptured descending aortic aneurysm(2014) Koç, Osman; Batur, Abdussamet; Vural, Alaattin; Günbey, Leman Karapekmez; Görmüş, Niyazi; Çil, BarbarosRuptured aortic aneurysm is a condition that requires emergency treatment because of its high mortality rate. In this case, we presented the emergency endo- vascular treatment of a ruptured descending thoracic aortic aneurysm. A female patient, admitted to the emergency with sudden onset back pain, was treated by emergency endovascular stent-graft after CT angiography examination. According to endovascular treatment, surgical treatment has a high mortality and morbidity rate in the treatment of the ruptured descending thoracic aortic aneurysms. The endovascular stent-graft treatment is applied safely and successfully to the intervention of this pathology. Closed drainage treatment was unsuccessful. Chest computed tomography demonstrated pneumomediastinum and subcutaneous emphysema with multiple air bubbles within the spinal canal between the levels Th3 and Th11. Resection of bullae on the upper lobe and partial pleurectomy were performed. Postoperative period was uneventful. Epidural emphysema was resolved spontaneously without neurologic symptoms and signs. Intraspinal air, or pneumorachis, associated with spontaneous pneumothorax and pneumomediastinum is an extremely rare condition. In these cases the totally resected tumors were examined histologically and immunohistochemically. and based on these studies benign localized solitary fibrous tumors originating from the diaphragmatic pleura were diagnosed. We observed no recurrence in the long-term follow-up. which was necessary for these cases due to the tumors' unpredictable clinical behavior even when they were thought to he benign histologically. Olgu sunumu: 65 yaşındaki bir erkek hastada Kaposi sarkomunun deri lezyonları ve lenfödem ortaya çıktı. Hastada sağ bacak, sol el bileği ve alında kahverengi-siyah renkli ağrısız papül şeklinde birbiriyle birleşik lezyonlar mevcuttu. Sonuç: Kaposi sarkomu nadir bir karsinom türüdür. In computerized tomographic examination an aneurysm of right internal iliac artery and an abnormally enlarged descending colon were seen. During an emergency operation a small rupture in the subserosa of the rectosigmoid junction was found.
