Öğe Conservative Approach in the Treatment of Renal Trauma in Children(KARGER, 2014) Pirincci, Necip; Kaba, Mehmet; Gecit, Ilhan; Gunes, Mustafa; Tanik, Serhat; Ceylan, KadirObjective: Our aim was to evaluate the role of a conservative approach in the treatment of renal trauma in the pediatric age group. Materials and Methods: A total of 31 pediatric patients with renal trauma (male = 25, female = 6) in whom we used a conservative approach in our clinic between the years 2002 and 2010 were included in the study. Patients were assessed according to the type of trauma, type of treatment, findings of radiological imaging, concomitant other organ injuries and complications occurring during follow-up. Results: Surgery was required in just 4 of 31 pediatric patients who were treated with the conservative approach. Of the patients who underwent surgery, only one child needed nephrectomy. No complications were detected in the clinical and radiological follow-up of the patients who received the conservative approach. Conclusions: The main objective in the treatment of renal trauma occurring in childhood is to protect the kidneys. If the hemodynannic findings are stable, a conservative approach should be the first preferred method of treatment in every grade of renal trauma that can occur in childhood. Copyright (C) 2013 S. Karger AG, BaselÖğe Prevalence of Ureaplasma and Mycoplasma in Infertile Men in Van Region and Effects to Semen Parameters(DERMAN MEDICAL PUBL, 2015) Taken, Kerem; Yildiz, Abdullah; Gunes, Mustafa; Eryilmaz, Recep; Gecit, Ilhan; Ceylan, KadirAim: The purpose of this study was to assess the prevalence of Ureaplasma urealyticum (UU) and Mycoplasma hominis (MH) in semen cultures of cases with primary infertility in the Van Province, and also to determine the effect of therapy on sperm parameters. Material and Method: The study included 106 individuals divided into three groups: The infertile group (41 cases), the group with lower urinary tract symptoms (33 cases), and the control group (32 cases). The patients in the infertile group had no history of varicocele, testicular torsion, hydrocele, undescended testis, and hormonal disorders. The control group included cases without infertility and lower urinary tract symptoms. The parameters of culture-positive cases in the infertile group were determined before and after therapy. The identification of Mycoplasma species was made using the Biomerieu (R) Mycoplasma 1ST 2 (RCS Lyon France) kit. The sperm count was carried out with the Makler counting chamber (Self Medical Industries, Haifa, Israel). Results: In the infertile group, UU was isolated from 17 and MH was isolated from 3 cases. In the group with lower urinary tract symptoms, UU was isolated from 15 (45.559) and MH was isolated from 6 (18.8%) cases. In the control group, UU was isolated from 6 (18.8%) cases, but MH was isolated from none of the cases. In the infertile group, the sperm counts in 3 culture-positive cases (15%) and in 10 culture-negative cases (50%) were <15 million each (p<0.025). Prior to therapy, the sperm motility in the culture-positive 14 cases (70%) was lower than 40%, and after therapy, 5 cases (2598) demonstrated a sperm motility of <4059, With therapy, a significant increase in sperm motility was obtained. After therapy of the 20 culture-positive cases (49%), the wives of 4 (203% patients became pregnant (p<0.025). Discussion: Antibiotherapy in culture positive cases with undetermined cause of infertility contributes to therapy by positively affecting sperm motility.Öğe Serum adenosine deaminase, catalase, and carbonic anhydrase activities in patients with renal cell carcinoma(TAYLOR & FRANCIS LTD, 2017) Pirincci, Necip; Kaya, Tacettin Yekta; Kaba, Mehmet; Ozan, Tunc; Gecit, Ilhan; Ozveren, Huseyin; Eren, HuseyinObjectives: To determine whether serum levels of adenosine deaminase (AD), catalase (CAT), and carbonic anhydrase (CA) enzymes may be useful biomarkers in the diagnosis of renal tumors and may lead to early diagnosis of renal tumors. Material and methods: The study included 33 patients with renal cell carcinoma (RCC) and 31 healthy controls. The activity of serum AD, CA, and CAT was determined and analyzed using the Giusti spectrophotometric method, H2O2 substrate, and C0(2) hydration, respectively. Results: Serum AD and CA activity were significantly higher in patients with RCC than in controls. However, serum CAT activity was significantly lower in patients with RCC than in controls. Conclusion: These markers might be potentially important as an additional biochemical tool for diagnosing RCC. We believe multidisciplinary studies are needed to plan patients' preoperative and postoperative treatment and to create follow-up protocols.Öğe Tissue damage in kidney, adrenal glands and diaphragm following extracorporeal shock wave lithotripsy(SAGE PUBLICATIONS INC, 2014) Gecit, Ilhan; Kavak, Servet; Oguz, Elif Kaval; Pirincci, Necip; Gunes, Mustafa; Kara, Mikail; Ceylan, KadirThis study was designed to investigate whether exposure to short-term extracorporeal shock wave lithotripsy (ESWL) produces histologic changes or induces apoptosis in the kidney, adrenal glands or diaphragm muscle in rats. The effect of shock waves on the kidney of male Wistar rats (n = 12) was investigated in an experimental setting using a special ESWL device. Animals were killed at 72 h after the last ESWL, and the tissues were stained with an in situ Cell Death Detection Kit, Fluorescein. Microscopic examination was performed by fluorescent microscopy. Apoptotic cell deaths in the renal tissue were not observed in the control group under fluorescent microscopy. In the ESWL group, local apoptotic changes were observed in the kidney in the area where the shock wave was focused. The apoptotic cell deaths observed in the adrenal gland of the control group were similar to those observed in the ESWL groups, and apoptosis was occasionally observed around the capsular structure. Apoptotic cell deaths in the diaphragm muscle were infrequently observed in the control group. Apoptosis in the ESWL group was limited to the mesothelial cells. This study demonstrated that serious kidney, adrenal gland and diaphragm muscles damage occurred following ESWL, which necessitated the removal of the organ in the rat model. It is recognized that the ESWL complications related to the kidney, adrenal gland and diaphragm muscles are rare and may be managed conservatively.
