    Alkaptonuria: Case report [Alkaptonüri]

    Alkaptonuria is a rare autosomal recessive condition arising as a result of a genetic deficiency of the enzyme homogentisic acid oxidase which has a role in the metabolism of tyrosine and phenylalanine. Blue-black discoloration which developes from excessive accumulation of homogentisic acid in various tissues such as connective tissue and cartilage due to deficiency of homogentisic acid oxidase enzyme, called ochronosis. The most important causes of morbidity are ochronotic arthropathy and cardiovascular involvement seen in fourth and sixth decades. There is no definitive treatment and treatment options generally are intended to prevent complications. Herein we report a 66-year-old woman diagnosed with alkaptonuria who presented with blue discoloration in auricle and nails. Alkaptonuria is a progressive systemic disease and it should be considered in the presence of appropriate clinical findings. © 2015 by Türkiye Klinikleri.

    Diz kontraktürlü hastalarda farklı fizik tedavi yöntemlerinin etkinliğinin karşılaştırılması

    Amaç: Bu çalışmanın amacı travmaya bağlı diz kontraktürlü hastalarda farklı fizik tedavi ve rehabilitasyon programlarının EHA üzerine etkinliğini değerlendirmektir. Gereç ve Yöntem: Bu çalışmaya Selçuk Üniversitesi Meram Tıp Fakültesi Fiziksel Tıp ve Rehabilitasyon Anabilim Dalı' nda yatarak tedavi gören travmaya bağlı diz kontraktürü tanısı almış 36 hasta alındı. Hastalar 4 gruba ayrıldı. 1. gruba GE, 2. gruba GE ve WP tedavisi, 3. gruba GE ve US tedavisi, 4. gruba GE, WP ve US tedavisi uygulandı. Hastaların diz EHA gonyometre ile tedavi öncesinde, tedavi sonrasında, tedaviden sonra 1. ay ve 3. ayda değerlendirildi. Ayrıca bu dört kontrol döneminde Beck Depresyon Skalası (BDÖ) ve sağlık durum anketi (Short Form-36, SF-36) doldurularak hastaların psikolojik ve fonksiyonel durumu değerlendirildi. Bulgular: Tedavi grupları arasında yaş, cinsiyet ve vücut kitle indeksi açısından fark yoktu (p>0,05). Tedavi gruplarının hepsinde tedavi öncesine göre tüm kontrol dönemlerinde diz EHA' da anlamlı artış saptandı, ancak gruplar arasında istatistiksel olarak anlamlı bir üstünlük tespit edilmedi (p>0,05). Tedavi gruplarında BDÖ sonuçları değerlendirildiğinde, tedavi öncesine göre kontrol dönemlerinde depresyon sıklığında azalma tespit edilirken, gruplar arasında istatistiksel olarak anlamlı bir fark tespit edilmedi (p>0,05). Hastaların fonksiyonel durumu değerlendirildiğinde tüm tedavi gruplarında tedavi öncesine göre uzun süreli takiplerde SF-36' nın alt grupları olan fiziksel fonksiyon, fiziksel kapasite ve ağrı yönünden anlamlı düzelme saptandı (p<0,05). SF-36' nın diğer alt grupları olan emosyonel kapasite ve genel sağlık, yaşam fonksiyonları, sosyal fonksiyonlar ve mental sağlık yönünden ise düzelme saptanmadı. Tedavi grupları arasında birbirine göre fark gözlenmedi (p>0,05). Sonuç: Diz kontraktürlü hastalarda fizik tedavi programları EHA, depresyon düzeyi ve fonksiyonel durum üzerine etkilidir ancak farklı tedavi modaliteleri arasında fark saptanamamıştır.

    Enthesitis and its relationship with disease activity, functional status, and quality of life in psoriatic arthritis: a multi-center study

    Psoriatic arthritis (PsA) is an inflammatory arthritis with distinct phenotypic subtypes. Enthesitis is assigned as a hallmark of the disease, given its significant relations to disease activity and quality of life. Our objective is to evaluate the prevalence of enthesitis and its association with some clinical parameters, particularly quality of life, using data from a national registry. Patients with PsA meeting ClASsification criteria for Psoriatic Arthritis (CASPAR) were enrolled by means of a multi-centre Turkish League Against Rheumatism (TLAR) Network Project. The following information was recorded in web-based case report forms: demographic, clinical and radiographic data; physical examination findings, including tender and swollen joint counts (TJC and SJC); nail and skin involvement; Disease Activity Score-28 for Rheumatoid Arthritis with Erythrocyte Sedimentation Rate (DAS 28-ESR); Bath Ankylosing Spondylitis Disease Activity Index (BASDAI); Maastricht Ankylosing Spondylitis Enthesitis Score (MASES); Psoriasis Area Severity Index (PASI); Bath Ankylosing Spondylitis Radiology Index for the spine (BASRI-s); Health Assessment Questionnaire (HAQ); Bath Ankylosing Spondylitis Functional Index (BASFI); Health Assessment Questionnaire for the spondyloarthropathies (HAQ-s); Psoriatic arthritis quality of Life scale (PsAQoL); Short Form 36 (SF-36); Hospital Anxiety Depression Scale (HADS); Functional Assessment of Chronic Illness Therapy-Fatigue (FACIT-F); and Fibromyalgia Rapid Screening Tool (FiRST) scores. The patients were divided into two groups, namely with and without enthesitis, based on the triple Likert-type physician-reported statement of 'active enthesitis', 'history of enthesitis' or 'none' in the case report forms. Patients with active enthesitis were compared to others in terms of these clinical parameters. A total of 1130 patients were enrolled in this observational study. Of these patients, 251 (22.2%) had active enthesitis according to the clinical assessment. TJC, HAQ-s, BASDAI, FiRST and PsAQoL were significantly higher whereas the SF-36 scores were lower in patients with enthesitis (p < 0.05). Chronic back pain, dactylitis, and tenosynovitis were more frequent in the enthesopathy group (59.4%/39%, 13.1%/6.5% and 24.7%/3.4%, respectively). Significant positive correlations between the MASES score and the TJC, HAQ, DAS 28-ESR, BASDAI, FiRST and PsAQoL scores, and a negative correlation with the SF-36 score were found. When linear regression analysis was performed, the SF-36 MCS and PCS scores decreased by - 9.740 and - 11.795 units, and the FiRST scores increased by 1.223 units in patients with enthesitis. Enthesitis is an important involvement of PsA with significant relations to quality of life determined with PsAQoL and SF-36 scores. Our study found higher frequency of dactylitis and chronic back pain, and worse quality of life determined with SF-36 and PsAQoL scores in patients with enthesitis.

    Background PsA is a chronic musculoskeletal disease. The prevalence of axial involvement in PsA varies according to the duration of the disease. In early stage the incidence varies between 5% and 28%, but it increases up to 25-70% in later stages of the disease. In the literature, there is limited data on the differences in disease activity, functional status and quality of life of men and women with axial PsA.

    The GUSS test as a good indicator to evaluate dysphagia in healthy older people: a multicenter reliability and validity study

    Purpose Dysphagia is known to be a disorder of the swallowing function, and is a growing health problem in aging populations. Swallowing screening tests have mostly been studied in comorbidities such as stroke associated with old age. There is no simple, quick and easy screening test to best determine the risk of oropharyngeal dysphagia in geriatric guidelines. We aimed to evaluate whether the Gugging Swallowing Screen (GUSS) test is an effective method for evaluating swallowing difficulty in healthy older people. Methods This cross-sectional and multicenter study was conducted at 13 hospitals between September 2017 and February 2019. The study included 1163 participants aged >= 65 years and who had no secondary dysphagia. Reliability was evaluated for data quality, scaling assumptions, acceptability, reliability, and validity as well as cutoff points, specificity and sensitivity. Results The age distribution of 773 (66.5%) patients was between 65 and 74 years and 347 (29.8%) of them were male and 767 (66%) patients were female. The average total GUSS score was 18.57 +/- 1.41. The Cronbach's alpha was 0.968. There was a moderate statistically significant negative correlation between the total GUSS and 10-item Eating Assessment Tool scores as well as between the total GUSS score and quality of life. The cutoff point of the total GUSS score was 18.50, sensitivity was 95.5% and specificity was 94.4%. Conclusions The GUSS test is a valid and reliable test to identify possible oropharyngeal dysphagia risk in healthy older people who had no secondary dysphagia. It is suitable as a screen test for clinical practice. Key summary pointsAim We aimed to evaluate whether the Gugging Swallowing Screen (GUSS) test is an effective method for evaluating swallowing difficulty in healthy older people. Findings Total GUSS score sensitivity was 95.5% and its specificity was 94.4%.     Kronik Non-Spesifik Bel Ağrılı Bireylerde Kor Stabilite, Ağrı ve Disabilite Arasındaki İlişki

    Amaç: Çalışmamızda, kronik non-spesifik bel ağrısı olan bireylerde lumbopelvik kor stabilite seviyesi, ağrı şiddeti ve fonksiyonel disabilite düzeyi arasındaki ilişkileri incelemeyi amaçladık. Yöntem: Bu prospektif kohort çalışması, kronik non-spesifik bel ağrılı bireyler ile gerçekleştirildi. Bireylerin ağrı şiddetini ölçmek için vizuel analog skala, fonksiyonel disabilite düzeyini belirlemek için Oswestry Özürlülük Skalası, lumbopelvik kor stabilite seviyesini değerlendirmek için Sahrmann Core Stabilite Testi uygulandı. İstatistiksel analizde, verilerin korelasyonları incelemek için Spearman’ın sıra korelasyon katsayısı kullanıldı. Anlamlılık değeri p<0,05 olarak belirlendi. Bulgular: Çalışmamız kapsamında yaşlarının ortanca değeri 35 yıl olan kronik non-spesifik bel ağrılı 36 birey değerlendirildi. Bireylerin kor stabilite seviyesi ile ağrı şiddeti arasında yüksek düzeyde negatif korelasyon olduğu belirlendi (p<0,001; r=-0,781). Fonksiyonel disabilite düzeyi ile kor stabilite seviyesi arasında orta düzeyde negatif korelasyon (p<0,001; r=-0,652); fonksiyonel disabilite düzeyi ile ağrı şiddeti arasında orta düzeyde pozitif korelasyon olduğu belirlendi (p<0,001; r=0,656). Sonuç: Çalışma sonuçlarımıza göre kronik non-spesifik bel ağrılı bireylerde lumbopelvik kor stabilitedeki azalma ağrı şiddetinin ve fonksiyonel disabilitenin artmasına yol açmaktadır. Buna göre non-spesifik bel ağrılı bireylerde lumbopelvik kor stabilitenin geliştirilmesi ile bireylerin ağrı şiddetinde ve fonksiyonel disabilite düzeyinde azalma sağlanabilir.

    Pain, depression levels, fatigue, sleep quality, and quality of life in elderly patients with rheumatoid arthritis

    Background/aim: The aim of this study was to evaluate and determine the relationships (if any) among pain, depression levels, fatigue, sleep quality, and quality of life in patients with rheumatoid arthritis (RA) aged 65 years and over, and to compare the results with those of RA patients under 65 years of age. Materials and methods: The study included 52 patients with RA aged 65 years and over (Group 1) and 84 patients with RA under 65 years of age (Group 2). Pain, depression levels, fatigue, sleep quality, quality of life, and disease activity of all of the participants were evaluated using a visual analog scale (VAS), the Beck Depression Inventory (BDI), the Checklist Individual Strength (CIS), the Pittsburgh Sleep Quality Index (PSQI), the Short Form-36 (SF-36), and the Disease Activity Score-28, respectively. Results: When the two groups were compared, higher scores for the VAS, BDI, total CIS, and PSQI were found in Group 1 compared to Group 2 (P = 0.003, P = 0.003, P = 0.007, and P = 0.001, respectively). The SF-36 subscales of the physical component summary and mental component summary were not statistically significantly different between the two groups (P > 0.05). Conclusion: This study evaluated the situation in elderly patients with RA and showed that pain, depression level, fatigue, and sleep quality worsen with age.

    Background: Psoriatic arthritis (PsA), a chronic rheumatic disease associated with reduced quality of life. Obesity is an important clinical problem which may interfere with loss of functioning and quality of life. Obesity is usually an overlooked entity in patients with PsA. Several studies were invastigated prevalence and the impact of obesity on disease activity in patients with PsA, however relationship between psychological status and quality of life have not been evaluated comparatively. Objectives: To assess the impact of obesity on quality of life, psychological status and clinical parameters in patients with PsA.

    Background/Purpose : Psoriatic arthritis (PsA) has been de? ned as an in? ammatory arthritis associated with pso-riasis. The disease activity can be evaluated using many scales in patients with PsA. There is a great temperature difference between geographic region in Turkey. For example,the average annual air temperature in Erzurum (one of the Eastern Anatolian cities) is 5.7° C whereas in Antalya (one of the Mediterranean region cities) average temperature is 18.7° C. Furthermore, altitude of Eastern Anatolian cities is higher than Mediterranean cities.
