It has been hypothesized that glutamine may become a conditionally essential amino acid in patients with catabolic disease. The objective of this study is to investigate the prognostic effect of glutamine. Methods: Fort this study, we selected 48 patients from the intensive care unit. Group I consisted of 33 patients whose treatment included glutamine. We placed the remaining 15 patients in group 11, and they did not receive glutamine in their treatment. We retrospectively investigated treatment time, leucocyte levels and outcome. We carried out the study between January 2002 and January 2003 in Konya Governmental Hospital, Turkey. Results: The average duration of hospital stay in the glutamine group was 8 +/- 1.2 days, 58% of them leaving hospital with surrogate. However, in the group whose treatment did not include glutamine, 42% of them left the hospital surrogate, their average hospital stay being 12 3 days. In the group receiving glutamine in the treatment, there was a prominent decrease in leukocyte levels compared to the other group, and hospitalization times were shorter but there was no statistically significant difference in mortality or survival rates. Conclusions: Glutamine may decrease the catabolism. it may also have a positive effect on treatment time and the consequences of therapy in critically ill patients.Öğe Finite element analysis and practical modeling of reinforced concrete multi-bin circular silos(AMER CONCRETE INST, 2006) Balkaya, C; Kalkan, E; Yuksel, SBStress resultants in overlapping wall regions (intersection walls) of multi-bin circular silos require a significant computational effort to determine forces due to structural continuity. This paper presents a practical equivalent beam model for computing design forces along the silo walls when subjected to various internal and interstice loadings. The equivalent beam model of intersection wall was developed based on the effective length concept, and verified in a comprehensive series of finite element (FE) analyses of a cluster of four silos for various silo-wall thicknesses. The influence of wall thickness on hoop forces and bending moments acting on interstice and external walls were also examined, and simple empirical expressions were presented for design applications. The proposed beam model yields an accurate estimation of bending moments and hoop forces with a maximum 7% deviation compared with those obtained from detailed FE models.Öğe Klippel-Feil syndrome associated with persistent trigeminal artery(LIPPINCOTT WILLIAMS & WILKINS, 2004) Paksoy, Y; Seker, M; Kalkan, EStudy Design. In the present report, radiologic findings of the patient with the Klippel-Feil syndrome associated with persistent trigeminal artery are reviewed. In same case, there was also spina bifida occulta, cervical spondylosis, atlanto-occipital assimilation, bilateral cervical ribs, and lack of flow signal in the C4 segment of contralateral vertebral artery. The persistent trigeminal artery and Klippel-Feil syndrome can be seen together and with the other anomalies. It should be considered that they might be related with the insufficiency or dysfunction of the embryologic developmental process. Objectives. To draw attention to this rare presentation of Klippel-Feil syndrome associated with persistent trigeminal artery. Summary of Background Data. Klippel-Feil syndrome is a congenital spinal malformation characterized by the failure in segmentation of 2 or more cervical vertebrae. Although the anomaly is defined by its skeletal component, Klippel-Feil Syndrome may also be associated with developmental defects in many other organ systems including the inner ear, spinal cord, heart, and genitourinary tract. The primitive trigeminal, otic, hypoglossal, and proatlantal intersegmental arteries are persistent fetal anastomoses between the carotid and vertebrobasilar circulations. The authors report the association of a persistent trigeminal artery with the Klippel-Feil syndrome. Methods. Radiologic findings of a case of Klippel-Feil syndrome associated with persistent trigeminal artery are described. Results. There was vertebral fusion, spina bifida occulta, bilateral cervical ribs, atlanto-occipital assimilation, and cervical spondylosis. Magnetic resonance angiography showed the persistent trigeminal artery between the left internal carotid and basilar artery, but there was no sign of flow in the C4 segment of right vertebral artery. Conclusions. It should be kept in mind that a persistent trigeminal artery and Klippel-Feil syndrome can be seen together and with the other anomalies. Because of this reason, this type of case should prompt a search for the other related anomalies, and magnetic resonance angiography can be used as a noninvasive diagnostic technique in the persistent trigeminal artery determination.Öğe A new neuroimplantable device: The tulgar implant. Initial results of animal testing(WILEY, 2005) Kalkan, E; Arican, M; Kalkan, SS; Erayman, I; Erol, A; Tulgar, AO; Tulgar, MA new neuroimplant system, namely the Tulgar implant, developed to solve the practical problems encountered with the presently available implants, was tested as a spinal cord stimulator in ten sheep. The response of living tissue and technical performance of the new system were evaluated. Electrodes implanted in the low thoracic (T9-10) vertebral levels by means of hemilaminectomy were subcutaneously connected to the passive coil receiver element located in the anterior-inferior chest wall for 28 days. Laboratory parameters including hematology, biochemistry, and microbiology were investigated over the study, and histopathologic examinations were done by the end of study. Animal tests showed that the new system could reliably be implantable in the living tissue. Intra-operative radicular stimulation of the right dorsal root in T13-L1 levels, by means of burst mode of signals, resulted in observable contractions of hip muscles in the right upper leg.
