The finding of intraosseous gas and its extension into the pubic joint and the left anterior abdominal wall is depicted. Intraosseous gas is a rare but worrying finding for osteomyelitis in the absence of a penetrative wound, recent surgery, biopsy or fracture. Copyright 2013 BMJ Publishing Group. This disorder characterized by developmental malformation of the lymphatic system surrounding the kidneys. A few cases with the retroperitoneal and mediastinal lymphangiectasia reported in the literature. We present the case of an 18-year-old male with retroperitoneal lymphangiectasia and mediastinal extension who presented with abdominal pain.
