Öğe Dilemma in predicting the infarct-related artery in acute inferior myocardial infarction: A case report and review of the literature(VIA MEDICA, 2011) Gul, Enes E.; Nikus, Kjell C.; Sonmez, Osman; Kayrak, MehmetThe electrocardiogram (ECG) has being used for decades as a reliable and inexpensive tool to diagnose acute myocardial infarction (AMI). ECG diagnosis of an occluded coronary artery is of the utmost importance. We present the case of a 46 year-old man admitted to our hospital for inferior AMI. The ECG findings suggested right coronary artery occlusion. Coronary angiography showed left circumflex artery occlusion. We also briefly review the literature. (Cardiol J 2011; 18, 2: 204-206)Öğe Hyperkalemia-Induced Accelerated Idioventricular Rhythm in a Patient with Acute Renal Failure(Informa Healthcare, 2012) Gül, Enes Elvin; Erdoğan, Halil ibrahim; Yıldırım, Oğuzhan; Soylu, Ahmet; Nikus, Kjell C.Electrolyte disorders can alter cardiac ionic currents and depending on the changes can promote proarrhythmic effects. Potassium (K+) is the most common intracellular cation related to arrhythmic disorders. Hyperkalemia is mainly seen in the setting of impaired renal function. Severe hyperkalemia may lead to rhythm disorders. Herein, we report a patient with accelerated idioventricular rhythm (AIVR) due to hyperkalemia, which was successfully treated with glucose-insulin (GI) infusion.Öğe An unusual presentation of left anterior descending artery occlusion: significance of lead aVR and T-wave direction(CHURCHILL LIVINGSTONE INC MEDICAL PUBLISHERS, 2011) Gul, Enes E.; Nikus, Kjell C.Acute occlusion of the left anterior descending coronary artery (LAD) is frequently encountered in acute ST-elevation myocardial infarction. Early detection of the clinical entity by the presenting electrocardiogram (ECG) should result in immediate aggressive clinical management. Although the typical ECG pattern of LAD occlusion is ST elevation, also atypical presentations, like ST depression, may occur. We describe a case with an unusual ECG pattern that suggested acute anterior myocardial infarction due to LAD occlusion. (C) 2011 Elsevier Inc. The diagnosis of pulmonary embolism (PE) was made and thrombolytic therapy was started. Baseline ECG showed signs of right ventricular overload and after thrombolysis, short PR, wide QRS, and delta wave indicating Wolf-Parkinson-White (WPW) pattern were appeared. In this case report, we discuss ECG of patient with PE mimicknig WPW.
