Öğe Clinical and metabolic effects of medroxyprogesterone acetate and ethinyl estradiol plus drospirenone in women with polycystic ovary syndrome(WILEY, 2008) Ozdemir, Suna; Gorkemli, Huseyin; Gezginc, Kazim; Ozdemir, Mustafa; Kiyici, AyselObjectives: To investigate the effects of treatment with medroxyprogesterone acetate (MPA), 10 days per month for 6 months, on lipid and carbohydrate metabolism in women with polycystic ovary syndrome (PCOS). Methods: Sixty-three women with PCOS were randomized to receive MPA or ethinyl estradiol plus drospirenone. Results: There were no changes in lipid or carbohydrate metabolism in the MPA group, but serum levels of luteinizing hormone (P < 0.001) and total testosterone (P < 0.003) significantly decreased, as did the free androgen index (P < 0.02) and acne (P < 0.03) and seborrhea (P < 0.04) scores. In the ethinyl estradiol plus drospirenone group lipid and hormone values significantly increased whereas acne, seborrhea, hair Loss, and Ferriman-Gallwey scores decreased. There was no statistically significant change in the total cholesterol to high-density cholesterol ratio in either group. Conclusion: Treatment of PCOS patients with MPA provided good menstrual cycle control, beneficial changes in hormonal values associated with hyperandrogenism, and no significant changes in lipid or carbohydrate metabolism. Sait; Demir, Halil; Ceyhan, Ayhan AbdullahThe ammonium biborate tetrahydrate was synthesized by stoichiometric incorporation of boric acid and ammonium hydroxide in aqueous solution and the effects of supersaturation, temperature, pH of solution, cooling rate and presence of impurities upon the metastable zone width (MZW) were investigated. The MZW of ammonium biborate tetrahydrate aqueous solution was increased with increasing concentration of impurities. The saturation temperature was increased in the presence of Ca (II) ion and decreased in the presence of Pb(II) and Mg (II) ions.Öğe Evaluation of application of multiple needle pricks on the pathergy reaction(WILEY, 2008) Ozdemir, Mustafa; Bodur, Said; Engin, Burhan; Baysal, IbrahimBackground The pathergy reaction (PR) to a needle prick is a well-known feature of Behcet's disease (BD). Although PR is accepted as a major criterion in this disease, there is a lack of a standardized method to measure PR. The aim of this study was to evaluate the effect on the positive rate of PR of the application of numerous (one to six) needle pricks. Methods Six needle pricks were performed on 105 patients with BD. The results of the test were noted from the proximal to the distal region for each area of each patient. The positive rates of all combinations (one, two, three, four, five, and six needle pricks) were calculated as percentages by a computer-based program (SPSS version 13 for Windows), and the mean percentage rates were obtained. Results There was an increase in the percentage of positive PR rates in each combination relative to the former combination. When one needle prick was performed, the mean percentage positive PR rate was 13%. The positive PR rates were 19%, 24%, 28%, 30%, and 33% for two, three, four, five and six needle pricks, respectively. The greatest increase in the mean percentage of positive PR rates was detected for the application of the two needle prick combination. Conclusion There is no consensus about the number of needle pricks required to detect PR, and multiple needle pricks have been used in many studies. We suggest that two needle pricks are suitable for the PR skin test as a useful adjunct to the diagnosis of BD.Öğe Evaluation of patients with Beh double dagger et's disease by cervical cytology and colposcopic examination(SPRINGER HEIDELBERG, 2012) Ozdemir, Suna; Ozdemir, Mustafa; Celik, Cetin; Balevi, Ali; Toy, Hatice; Kamis, UmitWe evaluated the genital involvement in women with Beh double dagger et's disease (BD) by cervical cytology and colposcopy, and compared these findings with healthy controls. A total of 152 women were referred to the Gynecology Clinic of Meram School of Medicine between March 2008 and December 2009. The study group was composed of 78 women with BD, and the control group included 74 healthy women admitted for routine gynecologic examination. These patients were subjected to cervical cytology and colposcopic examination. Cervical biopsy was performed in cases with abnormal cytology. These findings and some gynecologic complaints were compared. The demographic features were similar between each group. Genital involvement (scar + ulcer) was detected in 58 (74.3%) patients. The most common sites of ulcers (55.1%) and scars (7.6%) were the labia majora. Abnormal cytology in study and control groups was detected in nine (11.5%) and three (4%) of the patients, respectively, and a slight statistical significance was obtained (p = 0.048). Acetowhite and iodine-negative epithelium were higher in the study group than in controls (p = 0.015 and p = 0.042). Dyspareunia was higher in BD patients (p = 0.001). Patients with BD are more prone to having an abnormal cervical cytology and acetowhite and iodine-negative epithelium on colposcopic examination. However, these findings are mostly benign conditions. We screened HPGD mutations in six patients from three unrelated Turkish families with PHO, in which we showed one previously reported, p.A140P, and one novel, p.M1L, homozygous mutations. Both mutations co-segregated with the phenotype in all three families and were absent in 100 Turkish controls. The lesions had raised, firm erythematous borders included a trailing scale. On histologic examination, a perivascular inflammatory cell infiltrate composed of neutrophils and mononuclear inflammatory cells in the upper dermis was shown. This situation may lead to changes in serum levels of adipocyte-derived cytokines which have influence on the pathogenesis of psoriasis. The aim of this study was to evaluate the effect of cyclosporin treatment on some adipocyte-derived cytokines in psoriatic patients. This casecontrol study was performed between June 2009 and March 2010, at the Department of Dermatology of Meram School of Medicine. Serum leptin, adiponectin, resistin and ghrelin levels were assessed in 26 patients with psoriasis before and after cyclosporin treatment and body mass index-matched 26 healthy control subjects. The adipokines levels were compared between the groups. Serum leptin, ghrelin, resistin and adiponectin levels in patients with psoriasis before the treatment were higher than those of the control group but the differences were not statistically significant. A positive correlation between serum leptin and family history of psoriasis was detected (r = 0.398, P = 0.044). A strong negative correlation between the Psoriasis Area and Severity Index and serum ghrelin levels was seen (r = -0.52, P = 0.001) and there was a strong positive correlation between the Nail Psoriasis Severity Index and serum resistin levels (r = 0.62, P = 0.001). Following the treatment, a significant increase was seen in the serum level of adiponectin (P = 0.02) and resistin (P = 0.003). The correlations between the adipokines and the disease parameters before the treatment were lost after the treatment. Our results suggest that levels of some adipocyte-derived cytokines in psoriatic patients are affected by cyclosporin treatment.Öğe Serum Neuroinflammatory Markers' Levels and Evaluation of Peripheral Large Nerve Fibre Function in Patients with Psoriasis(GALENOS YAYINCILIK, 2016) Balevi, Ali; Ozdemir, Mustafa; Guney, Figen; Tol, Huseyin; Okudan, Nilsel; Belviranli, Muaz; Sahin, Kemal TahirObjective: Currently, tumor necrosis factor-alpha (TNF-alpha) inhibitors are widely used all over the world, especially in the treatment of severe psoriasis. However, multiple sclerosis (MS) and peripheral neuropathy occurrence associated with TNF-a inhibitors have been reported in the treatment of patients with psoriasis. It is not known whether TNF-a inhibitors reveal underlying neurological disorders in patients with psoriasis. The aim of this study was to investigate whether there is an association between psoriasis and MS or peripheral neuropathy. Methods: In this study, 50 psoriasis patients and 25 healthy controls were enrolled. Neurological examination and needle electromyography test were performed. Serum levels of matrix metalloproteinase-2 (MMP-2), matrix metalloproteinase-9 (MMP-9), S-100 calcium-binding protein (S100B) and neuron specific enolase (NSE), which are involved in neuroinflammatory process, were measured. Results: Serum MMP-9 levels were higher in psoriasis patients compared to healthy controls. Serum MMP-2 and NSE levels were decreased in psoriasis patients compared to healthy controls. There was no significant difference in S100B levels between two groups. All neurophysiological parameters in each individual patient and control were found to be within normal limits and there was no difference between groups. Conclusion: NSE and MMP-9 might play role in the occurrence of MS in psoriasis patients. Especially, we are in the opinion that it might be useful to measure the serum levels of these markers before the administration of TNF-a inhibitors in psoriasis patients with family history of MS.Öğe Torakolomber Burst Kırığına Bağli Anterior Dural Laserasyon(2003) Ozdemir, Mustafa; Ogün, Tunç; Kapicioglu, Mehmet İsmail SafaAmaç: Bu çal›flmada, torakolomber burst k›r›¤›nda anterior dural yaralanma insidans›, önceden tesbiti ve tedavi prensipleri araflt›r›lm›flt›r. Gereç ve yöntem: Yafl ortalamas› 42 olan 3 hastada anterior dekompresif cerrahi esnas›nda anterior dural yaralanma tesbit edildi. Hastalar›n ameliyat öncesi ve sonras› klinik ve radyolojik bulgular› retrospektif olarak analiz edildi. Ortalama takip süresi 20 ayd›r. Bulgular: 35 burst k›r›¤›ndan 3’ünde (%8.2) anterior dural yaralanma saptand›. ‹ki hastada y›rt›k primer olarak tamir edildi. Tüm hastalar son kontrollerinde nörolojik olarak normaldi ve herhangi bir beyinomurilik s›v›s› kaç›fl›na rastlanmad›. This neurological abnormality may lead to difficulties such as severe pain and burning sensations in treatment of hypertrichosis of faun tail with laser or laser-like devices. Objective: We evaluated outcomes of an intense pulsed light source in two patients with faun tail. Methods: The Lumina intense pulsed light system [650-nm handpiece (550-1100 nm)] was used for the treatment. Magnetic resonance imaging and neurological examination were done. Results: Tethered cord syndrome was detected as a neurological abnormality. The patients were treated with an energy fluence of 18-26 J/cm(2), pulse sequencing of 3 to 4, and a delay time of 20-35 ms. Local anesthesia was applied in one patient during treatment for severe pain sensation. A mean of 85% hair reduction was achieved. Conclusion: A good cosmetic result with intense pulsed light treatment was achieved in the patients with faun tail.
