    Diurnal Variation of Anterior Chamber Flare

    Objectives: To investigate the ideal time and reproducibility of anterior chamber flare measurements. Materials and Met-hods: Anterior chamber flare measurements were performed with laser flaremetre device at 8 am to 45 volunteers and these measurements were repeated on the same day at 12 pm and 4 pm. Results: Twenty- five (55.5%) of the volunteers were women and 20 (44.5%) were men; mean age was 28.67 +/- 7.40 (18- 49) years. The mean anterior chamber flare measurements taken following the ophthalmologic examination were 5.94 +/- 1.41 foton/msn at 8 am, 5.65 +/- 1.45 foton/msn at 12 pm, and 5.79 +/- 1.20 foton/msn at 4 pm. No statistical difference was found between the measurements (p=0.08). Subgroup analysis according to eye color, revealed no significant difference between flare measurements in brown, hazel, and green eyes (p=0.21). Correlation analysis demonstrated association between age and all flare measurements within the day (r=0.24, p=0.03; r=0.41, p=0.01, r=0.27, p=0.01). Conclusion: No significant diurnal change was detected in the flare measurements of our study subjects but positive correlation with age was observed. Hence, all flare measurements within a day are reliable and have high repeatability in healthy subjects.

    Efficacy of Vitamin a Supplementation in Keratoconjunctivitis Associated With Adult-Type Measles

    Sixty-four adults with maculopapular rash due to adult-type measles were examined on admission and days 7 and 14. Among the 25 patients with keratoconjunctivids at admission, 12 patients were given vitamin A (group 1) and 13 patients were not (group 2). Ten patients in group 1 and 2 patients in group 2 showed improvement at day 7. At-day 14, no patient had keratoconjunctivitis, including 18 patients without initial keratoconjunctivitis who received vitamin A.

    In Vivo Confocal Microscopic Findings of 2 Patients with Bietti Crystalline Corneoretinal Dystrophy

    Purpose: To describe the clinical and in vivo confocal microscopic findings of the cornea in 2 patients with Bietti crystalline corneoretinal dystrophy using Heidelberg Retina Tomograph II Rostock Cornea Module. Methods: Two women 25 and 33 years of age underwent ophthalmologic assessment, including fundus photography, optical coherence tomography, fluorescein angiography, electroretinography, and in vivo confocal microscopy. Results: Slit-lamp examination revealed crystalline deposits at the superior limbus of the cornea in both of the subjects. Fundus examination disclosed numerous glistening yellowish white crystalline deposits scattered throughout the posterior pole and midperipheral retina, retina pigment epithelium and choriocapillaris atrophy; pigment clumping; and retinal scarring. Optical coherence tomography demonstrated hyperreflective red and white areas corresponding to the crystalline deposits in the retinal pigment epithelium-choriocapillaris complex. In vivo confocal microscopy of the superior paralimbal area showed randomly oriented needle-shaped or rod-shaped crystals up to 40 mu m in length and 4-8 mu m in width in the epithelium and the stroma. In other areas, the epithelium, stroma, and endothelium had normal appearance with no deposits. Conclusions: In vivo confocal microscopy is a noninvasive examination technique that shows clearly the corneal crystals located mainly in the superior paralimbal area in Bietti crystalline corneoretinal dystrophy, which can easily be missed even by an experienced ophthalmologist and therefore may aid further in the diagnosis.

    Ocular Changes in Primary Hypothyroidism

    Background: To determine the ocular changes related to hypothyrodism in newly diagnosed patients without orbitopathy. Findings: Thirty-three patients diagnosed to have primary overt hypothyroidism were enrolled in the study. All subjects were assigned to underwent central corneal thickness (CCT), anterior chamber volume, depth and angle measurements with the Scheimpflug camera (Pentacam, Oculus) and cup to disc ratio (C/D), mean retinal thickness and mean retinal nerve fiber layer (RNFL) thickness measurements with optical coherence tomography (OCT) in addition to ophthalmological examination preceeding the replacement therapy and at the 1st, 3rd and 6th months of treatment. The mean age of the patients included in the study were 40.58 ± 1.32 years. The thyroid hormone levels return to normal levels in all patients during the follow-up period, however the mean intraocular pressure (IOP) revealed no significant change. The mean CCT was 538.05 ± 3.85 ? initially and demonstrated no statistically significant change as the anterior chamber volume, depth and angle measurements did. The mean C/D ratio was 0.29 ± 0.03 and the mean retinal thickness was 255.83 ± 19.49 µ initially and the treatment did not give rise to any significant change. The mean RNFL thickness was also stable during the control visits, so no statistically significant change was encountered. Conclusions: Neither hypothyroidism, nor its replacement therapy gave rise to any change of IOP, CCT, anterior chamber parameters, RNFL, retinal thickness and C/D ratio.

    Topical Anesthetic Abuse Keratopathy: A Commonly Overlooked Health Care Problem

    Purpose: To evaluate the clinical course, treatment modality, factors affecting the epithelization period, and visual outcome in patients with topical anesthetic abuse keratopathy. Methods: The medical records of 19 patients with a confirmed diagnosis of topical anesthetic abuse keratopathy were retrospectively examined; occupation, initiating event, biomicroscopic findings, treatment modality, epithelization period, and best-corrected visual acuity (BCVA) were noted. The Wilcoxon signed rank test was used to compare BCVA before and after treatment; P values < 0.05 were considered statistically significant. Results: In all, 26 eyes in 19 men aged 21-44 (mean age: 31 +/- 6) years were included. Initiating events included exposure to arc welding flash (8 patients), metallic foreign body injury (8 patients), and chemical injury (3 patients). On admission to hospital, 10 patients (52.6%) reported that they were using topical anesthetics. Upon admission to the hospital, 10 patients (52.6%) self reported that they were using topical anesthetics. The remaining 9 subjects were discovered to be using topical anaesthetic drops during hospitalization. Twelve patients (63.2%) were found to continue using these agents during their hospitalization. Oval corneal epithelial defect, stromal infiltrate, ring-shaped keratitis, and hypopyon were noted in 100%, 46.2%, 57.7%, and 42.3% of the eyes, respectively. Topical antibiotics (fluoroquinolones or combined fortified cephalosporins and aminoglycosides), preservative-free lubricants/autologous serum, and bandage contact lens/eye patches were used for treatment. Mean epithelization period was 19.96 +/- 11.16 days (range: 6-50 days). Mean pretreatment and posttreatment BCVA was 0.12 +/- 0.16 (range: 0.001-0.7) and 0.66 +/- 0.30 (range: 0.0-1.0), respectively (P < 0.001). Conclusions: Ophthalmologists should be suspicious of topical anesthetic abuse keratopathy in young male manual laborers specialized in welding business and foundry work presenting with persistent epithelial defects, ring-shaped keratitis, and accompanying severe ocular pain.

    Yırtıklı retina dekolmanlarında maküler dekolman süresinin tedavi sonrası erken dönem görme keskinliğine etkisi

    Amaç: Yırtıklı retina dekolmanlı gözlerde, maküler dekolman süresinin pnömotik retinopeksi sonrası düzeltilmiş görme keskinliği üzerine etkisinin değerlendirilmesi. Gereç ve Yöntem: Kliniğimizde Mayıs 2006-Mayıs 2009 tarihleri arasında pnömotik retinopeksi uygulanmış ve tek seansta başarı sağlanmış maküler dekolman süresi 2 hafta ve altında olan yırtıklı retina dekolmanı olgularının dosya bilgileri geriye dönük olarak tarandı. Maküler dekolman sürelerine göre 47 göz 1-5 gün (21 göz), 6-10 gün (14 göz) ve 11-14 gün (12 göz) içinde tedavi edilenler olarak üç gruba ve ayrıca birinci hafta (26 göz) ve ikinci hafta (21 göz) tedavi edilenler olarak iki ayrı gruba ayrılarak incelendi ve cerrahiden 3 ay sonra düzeltilmiş görme keskinliği gruplar arasında karşılaştırıldı. Bulgular: Maküler dekolman süresine göre üç zaman dilimli olarak incelememizde üç grup arasında tedavi sonrası görme keskinliği açısından anlamlı fark bulundu (p0.001, Kruskal Wallis testi). Yine ikili zaman dilimli incelemede ilk hafta tedavi edilenlerde görme keskinliği (0.560.19), ikinci hafta tedavi edilenlere göre (0.320.13) anlamlı olarak yüksek bulundu (p0.001, Mann-Whitney testi). Sonuç: İki haftalık süreç 3 ve 2 zaman dilimine göre incelendiğinde maküler dekolman süresi uzadıkça, görme keskinliği azalmaktadır. Maküler dekolman süresi tedavi sonrası görme keskinliğini değerlendirmede göz önünde bulundurulması gereken önemli bir prognoz belirleyicidir.
