    Ankle joint position sense in male Taekwondo athletes after wobble board training

    Background and Study Aim: There is evidence of an improvement in sportive performance in the athletes after completing a wobble board training program. The aim of this study was to determine whether a six-week wobble board training increased the awareness of joint position sense on ankle joint proprioception in male taekwondo athletes. Material/Methods: Eighteen male taekwondo athletes took part in this study. Participants were randomly divided into experimental and control groups. For experimental group (n=10) the average age, height, weight and experience time of the subjects were 19.50 +/- 2.07 years, 175.80 +/- 7.27 cm, 64.80 +/- 4.71 kg and 6.30 +/- 1.49 years, respectively. For control group (n=8) it was 19.88 +/- 2.30 years, 173.75 +/- 5.06 cm, 67.50 +/- 5.55 kg and 7.13 +/- 2.56 years, respectively. Experimental group took wobble board was training three times a week for the period of six weeks. To assess ankle joint position sense (JPS), passive angle reproduction test was performed by the Biodex System 3 Dynamometer (Biodex Medical Systems, Shirley, NY, USA). Passive angle reproduction test was conducted on dominant and non-dominant ankle at 5 degrees and 25 degrees of plantar flexion angles. Measurements were taken twice, before and after training. Results: There was no significant difference in dominant ankle at 5 degrees between JPS measurements before and after training in experimental group (t=1.920, p=0.087). JPS increased significantly in dominant ankle at 25 degrees (t=3.060, p=0.014), non-dominant ankle at 5 degrees (t=2.959, p=0.016) and 25 degrees (t=3.213, p=0.011) in experimental group. Conclusions: The WBT of taekwondo athletes had improved JPS, especially in non-dominant ankle. The proprioception training with wobble board may provide an advantage in using dominant leg during performing taekwondo sport moves and in decreasing the number of ankle injuries in male taekwondo athletes.

    Axis I and Axis II Psychiatric Disorders in Patients With Fibromyalgia

    Objective: To determine the current prevalence of Axis I and Axis II psychiatric disorders in patients with fibromyalgia. Method: The study sample includes 103 patients with fibromyalgia and 83 control subjects. Axis I and Axis 11 disorders were determined by structured clinical interviews. Results: The rate or any Axis I psychiatric disorder (47.6% vs. 15.7%), major depression (14.6% vs. 4.8%), specific phobia (13.6% vs. 4.8%), any Axis II disorder (31.1% vs. 13.3%), obsessive compulsive (23.3% vs. 3.6%) and avoidant (10.7% vs. 2.4%) personality disorders were significantly more common in the patient group compared to the control group. Conclusion: Our results suggest that a considerable proportion of patients with fibromyalgia also present with Axis I and Axis II psychopathologies.

    The Effects of Fibromyalgia Syndrome on Female Sexuality: A Controlled Study

    Introduction. Fibromyalgia may have negative effects on sexual function in women. Aims. To evaluate the sexuality of women with fibromyalgia and healthy control subjects, and to investigate the relation between sexuality and clinical parameters of fibromyalgia. Main Outcome Measures. Female Sexual Function Index (FSFI), Tender Points Count (TPC), Beck Depression Inventory (BDI), Fibromyalgia Impact Questionnaire (FIQ), and Visual Analog Scale (VAS). Methods. Clinical evaluation and surveys were done with 126 women with fibromyalgia and 132 healthy women. Results. Patients with fibromyalgia had higher BDI scores and lower frequency of sexual intercourse than control subjects. The mean FSFI scores (total and all domains) were significantly lower in patients with fibromyalgia than control subjects. Mean FSFI scores (total and most domains) were significantly lower in patients with fibromyalgia who had BDI score >= 17 than those who had BDI score < 17. In women with fibromyalgia, a significant negative correlation was noted between total FSFI score, and both FIQ and BDI scores. Conclusions.     Evaluation of knee proprioception and effects of proprioception exercise in patients with benign joint hypermobility syndrome

    The first aim is to show if there is a disorder in proprioception in cases with benign joint hypermobility syndrome (BJHS) when compared to healthy subjects. The second aim is to evaluate the effect of proprioception exercise in BJHS cases. To evaluate the proprioceptive sensibility of the knee joint with 40 BJHS and 30 healthy subjects enrolled in the study. Then, cases with BJHS were randomized into two groups; proprioceptive exercises were applied to 15 patients for 8 weeks in clinic and 25 patients were taken as controls. Outcome measures included proprioceptive sensation, AIMS2 and VAS. Proprioception is significantly impaired in cases with BJHS. In BJHS group, significant decreases in VAS levels were detected in cases who did exercise compared with cases who did not, and statistically significant improvements were detected in occupational activity. For this reason proprioception exercises cause decrease in pain and improvement of functional status in BJHS group.

    Evaluation of Sexual Dysfunction in Females With Ankylosing Spondylitis

    Objectives: This study aims to evaluate sexual function in females with ankylosing spondylitis (AS), compare them with healthy controls, and demonstrate the effects of AS on female sexual functions. Patients and methods: Fifty-four AS patients (mean age 39.33 +/- 8.57 years; range 20 to 55 years) and 56 similar aged healthy controls were included in the study. Depression levels and sexual functions of all participants were evaluated by the Beck Depression Inventory and Female Sexual Function Index (FSFI), respectively. Other assessment methods included the visual analog scale for pain, fatigue, and importance of sexual life; Bath Ankylosing Spondylitis Disease Activity Index for disease activity, Bath Ankylosing Spondylitis Functional Index for functionality, Bath Ankylosing Spondylitis Metrology Index for spinal mobility, and Short Form-36 (SF-36) for quality of life. Results: Total FSFI and all FSFI subscale scores and number of weekly sexual intercourse were significantly lower while Beck Depression Inventory score was significantly higher in females with AS when compared to controls. In AS patients with depression, total FSFI score and FSFI subscales scores of desire and arousal were significantly lower than those without depression. In females with AS, there were negative correlations between total FSFI score and duration of complaint, Beck Depression Inventory score, Bath Ankylosing Spondylitis Metrology Index score, visual analog scale score, age, and duration of marriage while positive correlations existed between total FSFI score and visual analog scale importance of sexual life score and number of weekly sexual intercourse, SF-36 fatigue, SF-36 social function, SF-36 pain, and SF-36 mental component scores. Conclusion: Sexual dysfunction was more common in female AS patients without marked impairment in body image and hip involvement when compared to normal population. Sexual problems which are generally neglected should be handled regardless of disease activity when evaluating patients with AS and establishing a treatment plan.

    Evaluation of Sexual Dysfunction in Women with Rheumatoid Arthritis: A Controlled Study

    Objective. To evaluate sexual function in women with rheumatoid arthritis (RA) and compare them with healthy controls. Materials and Methods. Ninety-five patients with RA and 108 healthy controls were included in the study. Depression levels and sexual functions of all participants were evaluated by the Beck Depression Inventory (BDI) and Index of Female Sexual Function (IFSF), respectively. Morning stiffness in women with RA, pain (Visual Analog ScaleVAS), disability rate (Health Assessment QuestionnaireHAQ), and disease activity score (DAS-28) were defined. Results. Total IFSF and mean of IFSF subgroup scores and significance of sexual life score were lower in patients with RA than controls, whereas mean BDI score was higher. The mean of the total IFSF scores was significantly lower in patients with RA with BDI =17 than that of patients with BDI <17. The mean of the total IFSF score decreased as disease severity increased. There was a strong negative correlation between total IFSF and DAS-28 scores, a moderate negative correlation between total IFSF score and HAQ, BDI, VAS score, age, and morning stiffness, and weak negative correlation between total IFSF and body mass index. Conclusion. RA is observed to have negative effects on sexual functions of women. Presence of depressive symptoms with RA and increased disease severity increase the degree of sexual dysfunction.     An Investigation of Sexual/Reproductive Health Issues in Women with a Physical Disability

    Women with a physical disability continue to live with many physical, psychological, and social challenges. Long-term disability in women leads to negative consequences on their sexual functions. The study was designed to investigate sexual challenges in women with a physical disability. Employing a qualitative design, the study was conducted with ten married women with a physical disability, who fit into the inclusion criteria and had applied to the Physical Treatment and Rehabilitation Department of the Meram Medical Faculty, Necmettin Erbakan University, in Konya, Turkey. The data related to women's sexual experiences, were analyzed using descriptive and content analysis. All participants were homemakers from middle-income families, with at least one child, and were socially supported by their families since their disorder. In addition to physical disability in all cases, seven participants were diagnosed with concomitant hypertension with diabetes mellitus, and three with concomitant chronic obstructive pulmonary disease with diabetes mellitus. All were found to experience sexual problems. Sexual dysfunction is seen as a frequently encountered health challenge in women with a physical disability and should definitely be evaluated in each patient. In light of our findings, it may be suggested that healthcare professionals should be made aware of the sexual lives of women with a physical disability and should be trained for counseling.

    Reactive arthritis following tetanus vaccination: a case report

    We report a case of reactive arthritis following tetanus vaccination. A healthy 55-year-old woman presented with pain and acute swelling of the right knee two days after receiving a tetanus vaccination. Erythrocyte sedimentation rate and C-reactive protein were elevated. Rheumatoid factor and human leukocyte antigen B-27 were negative. Her arthritis improved with the administration of nonsteroidal anti-inflammatory drugs. One week later the knee swelling and pain had settled. Reactive arthritis may occur after tetanus vaccination.

    Relationship between handgrip strength and isokinetic shoulder abduction-adduction muscle strength in primer fibromyalgia

    Objective: To compare grip strength and isokinetic shoulder abduction-adduction maximal muscle strength and to assess the correlation between hand grip strength (HGS) and shoulder strength in primer fibromyalgia patients with healthy controls. Patients and Methods: One hundred patient and 50 healthy women were included in this study. The patient's complaint duration and number of tender points were recorded. The visual analog scale was used for the assessment of pain. Beck depression scale and Fibromyalgia Impact Questionnaire (FIQ) were calculated. Handgrip strength was measured using a handgrip dynamometer and the highest value of sequential 3 measurements was recorded. Isokinetic shoulder abduction-adduction maximal muscle strength was measured at 60 and 180 degrees/sec. All patients' dominant side was assessed. The strength was calculated as the mean of five consantric isokinetic repetations. Results: There was no statistical difference between patients and the control group in age and body mass index (BMI). The mean HGS was 0,41 +/- 0,10 and 0,62 +/- 0,13 in the fibromyalgia and the healthy group respectively. The mean HGS and shoulder abduction-adduction maximal muscle strength were significantly lower in the fibromyalgia group than the healthy subjects (p<0,001). Whereas a positive correlation was found between HGS and age, BMI and shoulder abduction-adduction maximal muscle strength, a negative correlation was recorded between complaint duration, VAS and FIQ. Conclusion: Hand grip strength may be a simple and effective method to determine muscle performance of the upper extremity.

    The Relationship Between Tender Point Count and Disease Severity in Patients with Primary Fibromyalgia

    Objective of this study is to asses the relationship between tender point count (TPC) and the severity of disease in patients with primary fibromyalgia (FM). One hundred and seven female patients with FM diagnosis according to ACR criteria were included to the study. Main outcome measures were fibromyalgia impact questionnaire (FIQ), Beck depression inventory (BDI), visual analog scale (VAS) and tender point assessment. Mean TPC was 14.66 +/- A 2.50 and mean VAS was 6.6 +/- A 1.2 cm. Mean total FIQ score and BDI was 62.75 +/- A 15.57 and 16.17 +/- A 7.12, respectively. TPC was correlated positively with FIQ and VAS scores. There was no correlation between TPC and age and duration of symptoms. In conclusion, TPC is a simple and noninvasive examination finding that can supply information about the disease severity and the depression in FM.

    Reliability and Validity of the Turkish Version of the Pain Quality Assessment Scale in Patients With Carpal Tunnel Syndrome

    Objective: There are some tests used by clinicians for defining and differentiating various pain types and following up the patient after initiating pain management therapy The Pain Quality Assessment Scale (PQAS) is a questionnaire used in differentiation of neuropathic and non-neuropathic pain. The PQAS is also used in detection of the most common symptom of neuropathic pain which has a wide spectrum, and in the management and follow up of pain therapy in the light of its findings. The aim of this study was to examine whether the Turkish version of PQAS is a valid and reliable tool to assess pain. Material and Method: Seventy patients with carpal tunnel syndrome seen by two clinicians were evaluated by Turkish version of the PQAS in the morning and in the afternoon of the same day. Results: With respect to reliability, the correlations between PQAS total score and paroxysmal, surface, deep and sensitive pain were 0.830 (p<0.001), 0.853 (p<0.001), 0.893 (p<0.001) and 0.679 (p<0.001), respectively. With respect to validity, the correlation results between total score and paroxysmal, surface and deep pain subgroups of PQAS was found as r:0.87, r:0.80 and r:0.87, respectively. Conclusion: In painful situations, defining the pain type and detecting the dominant symptom are quite helpful in management of therapy. The results of this study suggest that the Turkish version of the PQAS is a reliable and valid instrument for the measurement of pain in Turkish patients that have diseases with neuropathic pain types.
