Öğe Effect of Erythropoietin on Microvascular Anastomosis in Rat(PHARMACOTHERAPY GROUP, 2015) Senturk, Sadik; Karacor, Zeynep; Tosun, Zekeriya; Ozkan, Adem; Avunduk, M. Cihat; Zumrut, MuratPurpose: To investigate the re-endothelialization potential of erythropoietin (EPO) following microvascular anastomosis in rat femoral artery. Methods: Ninety-six male Sprague-Dawley rats weighing between 300 g and 320 g were allocated randomly into two groups (control and EPO, n = 48). Left femoral artery microvascular anastomosis was performed in the rats. The control group was given 0.5 mL of 0.09 % NaCl subcutaneously 48 h post-operatively while the EPO group was given 150 U/kg recombinant human erythropoietin subcutaneously 48 h post-operatively. Tissue was harvested from each group after 1, 3, 5, and 7 days. All of the rats were sacrificed for morphometric analysis. Each arterial segment was analyzed morphometrically using Clemex Image Analysis software by a single examiner. The cross-sectional area of the media and neointima was measured and the intima/media ratio was calculated. Hematocrit measurement was carried out for all the rats. Results: The I/M ratio differed significantly (p < 0.05) between the control and erythropoietin groups, at all time-points. The hematocrit was significantly (p < 0.05) lower in the control group than in the erythropoietin groups on day 7, but not on the other days. Conclusion: This study demonstrates that erythropoietin markedly inhibits neointima formation with accelerated re-endothelialization in rat femoral arteries following microvascular anastomosis.Öğe Extensive cervicofacial necrotizing fasciitis of odontogenic origin: case report and literature review(SPRINGER, 2015) Ozkan, Adem; Senturk, Sadik; Topkara, Adem; Tosun, ZekeriyaNecrotizing fasciitis is a potentially fatal soft tissue infection characterized by generalized necrosis and gas formation in the subcutaneous tissues and fascia. It is rarely seen in the head and neck area. This report presents the case of a 62year-old diabetic female patient with generalized cervicofacial necrotizing fasciitis extending to the front chest wall. The cause of the disease was the extraction of the infected second molar tooth. Debridement was performed on the generalized necrotic fascia, subcutaneous tissue, and the skin extending from the preauricular area to the front chest wall and nipples. A series of debridement procedures were necessary because of generalized necrosis. The patient received intensive medical supportive treatment. Following the debridement procedures, the defective area from the neck to the front chest wall was closed up with split-thickness skin graft. Abscessed tooth extraction can lead to cervicofacial necrotizing fasciitis. Necrotizing fasciitis still remains as a potentially fatal disease. Early diagnosis, early radical surgical debridement, and a multidisciplinary approach constitute the significant factors in preventing mortality in such patients.Öğe Fasciotomy Procedures on Acute Compartment Syndromes of the Upper Extremity Related to Burns(MODESTUM LTD, 2015) Ozkan, Adem; Senturk, Sadik; Tosun, ZekeriyaCompartment syndrome is a serious complication of extremity burns. The goal of this study is to review cases with upper extremity compartment syndrome that had fasciotomy and to underline the significance of emergency fasciotomy procedures. The medical records of 43 patients who had fasciotomy because of compartment syndrome of the upper extremity between 2007 and 2013 were retrospectively reviewed. Etiology, age, sex, fasciotomy area, the period between the burn and fasciotomy, and treatment options were evaluated. Patients with arm, forearm, and hand or digit fasciotomy were presented. Scalding and electric were the predominant cause among all patients. While 12 patients were adults, 31 of them were children and the patients' ages ranged between 1 and 39. Defect areas formed after fasciotomy were closed with skin grafts and primary closure. Length of hospital stay in electrical burns group was significantly longer than in other burn groups (p<0.002). Burn injuries of the upper extremity are frequently seen. Most of the patients with circular burns occur compartment syndrome. Compartment syndrome is a surgical emergency requiring rapid diagnosis and treatment by urgent fasciotomy procedure. Thus, early diagnosis and adequate surgical decompression prevents function losses in the affected extremity.
