Öğe Acute Arterial Embolus(WESTMINSTER PUBL INC, 1991) Solak, Hasan; Yeniterzi, Mehmet; Yüksek, Tahir; Özpınar, Cevat; Ceran, SamiBetween December, 1983, and December, 1988, 100 patients (54 men, 46 women) with acute arterial embolus, ranging in age from twenty-three to eighty-three years, were treated in our clinics. The main symptom of the patients was pain (94%), followed by coldness (72%), pallor (64%), sensory defect (42%), motor disorder (37%), muscular rigidity (10%), and tissue necrosis (7%). We have determined the etiology to be atrial fibrillation in 53 cases, 36 of which arose from mitral stenosis: acute myocardial infarction (MI) in 14 cases; thromboembolus based on arteriosclerosis obliterans (ASO) in 27 cases; and tumor embolus in 2 cases; 4 cases could not be explained etiologically. The most common site of embolism was the femoral artery (33%). Arterial exploration was done under local anesthesia in the 76 patients who did not have distal localization of the embolus. Embolectomy was performed in 49 of these cases and embolectomy plus localized tromboendarterectomy in 27. Owing to developing necrosis, amputation was necessary in 6 cases-4 above the knee and 2 above the elbow. Motor and sensory defect persisted to various degrees in 27 cases. The same patient had also been operated three years earlier because of a right sided aneurysm.Öğe Bilateral Common Carotid Occlusion Without Neurological Deficit(2002) Karaköse, Serdar; Karabacakoğlu, Aydın; Solak, HasanA 40-year-old man presented with pain and numbness in his right arm. On his clinical examination, no neurological deficit was found. Bilateral common carotid artery duplex sonography scan demonstrated no flow in either lumen. No abnormality was recognized on brain CT. On cerebral digital substraction angiogram, total occlusion of the brachiocephalic trunk and left carotid artery were shown. There was a modest stenosis in the left vertebral artery. Collateral circulation feeding the intracranial carotid system mainly originated from the left vertebrobasilar system. Previous cases of bilateral carotid occlusion are reviewed and discussed.Öğe Chest Trauma in Children(2002) Ceran, Sami; Sunam, Güven Sadi; Arıbaş, Olgun Kadir; Görmüş, Niyazi; Solak, HasanObjectives: Chest trauma in childhood is uncommon in clinical practice. The management and treatment principles of children with thoracic trauma were discussed with the data reported in the literature. Methods: Of the chest injury diagnosed in 1653 patients, 225 were children in the last 17-year period. There were 199 boys (88.44%) and 26 girls (11.55%). The most common causes were blunt injuries in 135 cases (60%), stab wounds in 67 cases (29.77%) and gunshot wounds in 22 cases (9.77%). Results: Out of 225, 217 patients were treated conservatively and eight patients were treated surgically. There was no mortality and morbidity. Conclusions: The prevalence of chest trauma in children due to blunt injuries is high in Turkey. Extremity injury is thought to be the most commonly associated extra-thoracic injury. However, thoracic trauma in children can be managed conservatively in most of the cases.Öğe Comparison of Autologous Basilic Vein Transpositions Between Forearm and Upper Arm Regions(Springer-Verlag, 2003) Görmüş, Niyazi; Özergin, Ufuk; Durgut, Kadir; Yüksek, Tahir; Solak, HasanArteriovenous fistulas are crucial for hemodialysis performances in patients with end-stage renal disease. In this report, we compare the results of basilic vein transposition in the forearm and upper arm regions. Autologous basilic vein transpositions were used in 20 patients with end-stage renal failure between May 2000 and January 2002. In 10 cases the basilic vein transposition was in the upper arm region for construction of a brachiobasilic fistula (group 1); in the remaining 10 cases it was in the forearm region for creation of a radiobasilic fistula (group 2). The mean age of the patients in group 1 was 53.6 years, and in group 2 it was 47.3. At the end of the follow-up period (mean, 10 months) patency rates in group 1 were 80% and in group 2, 90%. In group 2, early-term thrombosis, which could be treated easily with second intervention and anticoagulation treatment (low-molecular-weight heparin), was the most common complication (10%). For patients who have unsuitable cephalic vein and require long-term hemodialysis, transposition of the basilic vein in the forearm region (under the elbow) can be a good secondary choice for access, as can a brachiobasilic fistula in the upper arm region.Öğe Cost-Effective and Time-Saving Surgical Treatment of Pulmonary Hydatid Cysts With Multiple Localization(Springer-Verlag, 2002) Ceran, Sami; Sunam, Güven Sadi; Görmüş, Niyazi; Solak, Hasan; Şahin, MustafaPurpose. The most frequent anatomic locations of hydatid cysts are the liver and lungs. Because there is no effective medical therapy against this parasitic disease, surgery is the treatment of choice. The aim of this retrospective study was to compare the cost and effect of a one-stage operation with those of two- or three-stage operations in the treatment of lung hydatid cysts with multiple localizations. Methods. We evaluated 364 patients who underwent surgical treatment for hydatid cysts, all of whom had multiple localizations. To avoid two- or three-staged operations, we performed median sternotomy, simultaneous bilateral thoracotomy and unilateral thoracotomy with a transdiaphragmatic approach. Results. For the treatment of 460 hydatid cyst localizations in 364 patients, a collective 381 operations were performed. The number of operations and periods of hospitalization were reduced. Conclusion. Bu deneysel çalışmada preoperatif ve intra-operatif pentoksifilin ve aprotinin uygulamasının akciğer hasarını azaltmadaki etkinliğini araştırmayı amaçladık. Yöntem: Sağlıklı 18 köpek üç eşit gruba ayrıldılar; kontrol grubu (grup I, n6), pentoksifilin grubu (grup II, n6) ve aprotinin grubu (grup III, n6). Tüm deneklere genel anestezi altında standart kardiyopulmoner bypass uygulandı ve preoperatif sağ pulmoner venden kan örneği ve sağ üst akciğer lobundan akciğer dokusu alındı. Kardiyopulmoner bypasstan çıkıldıktan sonra da aynı bölgelerden postoperatif numuneler alındı. Histopatolojik incelemeler ışık mikroskobunda yapıldı. Biyokimyasal incelemede IL-6, TNF-alfa ve GSH-PX düzeyleri ölçüldü. Bulgular: Aprotinin grubu kontrol grubu ile karşılaştırıldığında, KPB sırasında oluşan iltihabi hücre infiltrasyonunu ve alveolar septal defekti önlemesi açısından aprotinin başarılı bulundu. Kontrol grubu pentoksifilin grubu ile karşılaştırıldığında iki grup arasında anlamlı bir fark bulunmadı. IL-6 açısından hem pentoksifilin hem de aprotinin postoperatif dönemde kontrol grubuna göre anlamlı bir azalma sağlamışlardır. Sonuç: Histopatolojik ve biyokimyasal incelemelerde aprotininin kardiyopulmoner bypass sırasında oluşan akciğer hasarını önemli derecede önlediği, pentoksifilinin ise sadece biyokimyasal olarak akciğer hasarını baskıladığı saptanmıştır.Öğe Lower Extremity Deep Vein Thrombosis and its Treatment(Springer-Verlag, 1992) Solak, Hasan; Yeniterzi, Mehmet; Yüksek, Tahir; Özpınar, Cevat; Ceran, Sami; Ünal, A. Venous thrombectomy was applied first by LeRiche in 1927. In subsequent years, Fogarty, De Weese, and Edwards defined the basic rules of venous thrombectomy and medical treatment more thoroughly.Öğe Methods of Treatment of Childhood Empyema in a Turkish University Hospital(AMER COLL CHEST PHYSICIANS, 1987) Solak, Hasan; Yüksek, Tahir; Solak, NecdetIn the last ten years, 120 patients were admitted to our clinic with empyema. Empyema and effusion have been investigated for pathogenesis. Microbiologic studies of pleural aspiration fluid showed that Staphylococcus aureus was the most common pathogen, found in 44 patients(36.7 percent). Other pathogens found were streptococci in 23 (19.2 percent) and Pneumococcus in 18 (15percent) in sequence. No production occurred in cultures of 19 (15.8 percent) patients. In 16 (13.3 percent), Pseudomonas and Enterobacteria such as E coU, Klebsiella and Proteus occurred. Surgical intervention has been carried out depending on clinical conditions. In the 24 (20percent) patients, pleural aspiration and lavage, decortication in 23 (19.2 percent), pulmonary resection (segmentectomy, lobectomy) in seven (5.8 percent), and only drainage and thoracic lavage in 66 (52.5 percent) have been carried out. The advantages of this technique are simplicity, effectiveness, and speed.Öğe Peripheral Arterial İnjuries with Gunshot Wounds: Surgical Treatments and Results(1989) Özgen, Gökalp; Eren, Nesimi; Solak, HasanBetween 1975 and 1985, 102 consecutive patients with 106 peripheral vascu lar injuries caused by gunshot blasts were treated in the authors' clinic. The average age was 25.6 ± 11.4 (mean ± SD). The average time between arterial injury and arrival to the hospital was 13.2 ± 4.8 hours. Sixty-six (62.3%) arte rial injuries were found in lower extremities and the most commonly injured artery was the superficial femoral artery. Thirty-six (35.3%) patients had com bined arterial and venous trauma. All patients with vascular involvement had some signs and symptoms of vascular trauma. Preoperative angiography was required in 23 cases and demonstrated arterial injury; 98 patients underwent surgical intervention. Resection of the injured arterial segment and end-to-end anastomosis were done in 44 patients. Resection of the arterial segment followed by interposition of autogenous vein graft was done in 34 patients. One case underwent nonbiologic graft (Dacron) replacement (in the left subclavicular ar tery). Primary lateral suture was placed in 6 cases, and ligation was made in 9. In cases associated with venous injury, 22 vascular venous repairs and 14 liga tions were done. Eighty-one (79.4%) patients retained a functional limb. Two cases underwent primary amputation, and 9 cases underwent secondary ampu tation. Six patients died; the causes of morbidity were usually related to associ ated injuries. They are reviewed according to their ages, sex, types of injuries, clinical findings, surgical approaches and complications. Most of them were young males (78%) between 11-30 years of age (65%) and 74% were injured by sharp weapons and gunshots. Femoral artery injuries were seen in 29% of all patients. Amputation was performed in 6.4% of cases, but in the popliteal artery group the amputation ratio was 14%. Thirteen patients died from hypovolemic shock or acute renal failure (3.12%).Öğe Post-Traumatic Arteriovenous Fistula Presented With Hypertrophic Palmaris Digitalis II(Springer-Verlag, 2003) Görmüş, Niyazi; Karabacakoğlu, Aydın; Solak, Hasan; Karaköse, SerdarA 19-year-old female was admitted to our department because of a hypertrophic palmaris digitalis II of the left hand displaying color changes. A thrill was easily palpated over the medial part of the finger. A review of her history revealed that this hypertrophy occurred after a blunt trauma to the left hand. Angiography showed arteriovenous fistulas between the digital branches of the radial and ulnar arteries and cephalic vein at the palmaris digitalis II of the left hand. These arteriovenous connections were closed with simple ligation under regional anesthesia. Postoperatively she had no sign of ischemia, and no thrill was palpated or auscultated.Öğe A Recurrent Right Atrial Thrombus: A Suggestive Sign for Behçet's Disease? Transthoracic and transesophageal echocardiography revealed a right atrial mass. She was operated on to remove the mass, which was shown by histopathological examination to be a thrombus. Three months later, echocardiography was repeated and recurrent right atrial thrombus was observed. Warfarin was given to the patient. One month later, the mass was not observed on echocardiographic examination. We report recurrent right atrial thrombus in a patient with Behcet's disease.Öğe Surgery in Hydatid Cyst of the Lung - A Report of 460 Cases(SCANDINAVIAN UNIVERSITY PRESS, 1988) Solak, Hasan; Özgen, Gökalp; Yüksek, Tahir; Eren, Nesimi; Solak, Necdet; Kırca, Naci Kemal; Akkoç, Özkan; Göktoğan, Tayfun; Özpınar, CevatA series of 460 surgically treated cases of hydatid cyst of the lung is presented. Radiologic preoperative diagnosis was made in 90% of the patients. Cystotomy with subsequent capitonnage was performed in 395 cases, wedge resection in 27, lobectomy in 15 and segmental resection in 23 cases. After careful hemostasis, two drains were placed in the thorax and the thoracic cavity was closed. There were three postoperative deaths (0.6%) and two recurrences of cyst (0.4%) during follow-up investigation for periods up to 10 years.
