We investigated the possible effects of L-arginine and aminoguanidine on plasma NO levels and the interaction between NO levels and lung tissue damage and blood gases in sepsis. Fifty Wistar male rats were used in this study and divided into five groups: group 1, sham group; group 2, CLP (sepsis); group 3, CLP + 10 mg/kg L-arginine administration; group 4, CLP +15 mg/kg aminoguanidine administration; group 5: CLP + L-arginine + aminoguanidine given in similar doses. Sepsis was induced by cecal ligation and puncture (CLP) method. Drugs were administered at postoperative hours 4 and 12. The levels in the aminoguanidine and aminoguanidine + L-arginine groups were similar to the sham group. Lung tissue damage in the sepsis and L-arginine groups was more severe than the other groups.Öğe Injuries and deaths occurring as a result of bull attack(2008) Dogan K.H.; Demirci S.; Erkol Z.; Sunam G.S.; Kucukkartallar T.Persons whose occupations are in animal husbandry may be seriously injured or killed while tending to animals. Bulls are among the most dangerous of these animals. In this study, seven deaths and 23 traumatic injury cases caused by bull attacks and treated in the General Surgery and Chest Surgery Department of Selcuk University Meram Medical Faculty Hospital are presented. Of the 30 total cases, 24 were male and 6 were female. The ages of the cases ranged from 33 to 86 years and the average age was 60.3 ± 12.4 (mean ± standard deviation). All of the death and injury cases occurred while engaged in the care of bulls. It was also determined that the injuries were in the chest region of five (71.4%) and in the head of two (18.6%) out of seven death cases; and in the abdominal region of 14 (60.9%) and in the chest region of nine (39.1%) out of 23 traumatic injury cases. Of the 14 cases that were in the abdominal region and treated surgically, 11 were perforation of small/large intestines, whereas hemorrhages involving the intestinal mesentery, intraabdominal hemorrhage due to perforation of the abdominal wall, and splenic laceration were determined in one case each. There were one or multiple costal fractures in all nine chest injury cases, flail chest in five cases, and hemothorax, pneumothorax, or hemopneumothorax were determined in six cases. Of the chest injury cases, tube thoracostomy was performed in six whereas the other three cases were discharged from the hospital after supportive care. It was concluded that animals such as bulls might be the cause of serious trauma ending with death for those working in the animal husbandry. For this reason, caution is required while feeding and working with bulls. Risk can be reduced by chutes, gates, restraints, special housing, and confinement facilities. Also, wearing protective helmets would be useful especially for preventing head injuries. © 2008 by The Haworth Press. All rights reserved.Öğe Thoracic neurogenic tumours [Torakal nörojenik tümörler](2013) Öncel M.; Sunam G.S.; Karabagli P.; Karabagli H.Intrathoracic neurogenic tumours are neoplasms arising from any of the neurogenic elements of the mediastinum. Schwannomas and neurofibromas develop from nerve sheath, ganglioneuromas and neuroblastomas from autonomic ganglions. The aim of in this study we present the clinical and pathological details of these patients and to determine only extending the mediastinum. Materials and methods: We reached retrospective clinical data of 16 patients (10 males and 6 females) with neurogenic tumours of the mediastinum undergoing surgical treatment 2008 to 2012. Age of the patients ranged from 5 to 66 years (mean 46 years). When the patients admitted our clinic to surgery. All patients had detailed history and physical examination, routine chest radiography, electrocardiography and standard laboratory blood tests. For localizations and to define the nature of the tumour whole the patients were performed computed tomographic (CT). Especially when we suspected involvement of intervertebral space and vascular space magnetic resonance imaging. Results: Preoperative symptoms were observed in 13 cases (%81), as follows: back and thoracic pain in 10 (%63), short of breathness in 3 (%19) patient.The preoperative CT showed the tumour as a solid lesion in 16 cases. The tumour was located on the posterior mediastinum in 10 (%63) patients and cases.2 (%13) patients upper mediastinum and 4 (%25) patients from thoracic wall. The histological type of the tumours was in 3 (%19) cases, ganglioneuroma, in 3 (%19) cases benign neurofibroma, in 9 (%56) cases schwannomas. 1 (%6) case originated from malignant peripheral nerve sheet tumour (MPNST). Complete surgical excision was achieved in all patients, with no postoperative mortality. There was no significant intraoperative and postoperative blood loss and no major surgical complications occurred. Conclusions: After the surgical indications may be carefully determined in cases. There is a good result in these tumours. MRI would facilitate therapeutic determination by distinguishing nature, spread and invasion of the neurogenic tumour.
