Öğe Case Study on Drug-Related Adverse Effects of Hepatitis C Therapy(Health Communications Inc, 2006) Başaranoğlu, Metin; Çelebi, Selman; Karaaslan, Hayri; Demir, AliThe case of a 46-year-old woman with chronic hepatitis C who was prescribed subcutaneous pegylated interferon once a week and oral ribavirin once a day is presented. Within 24 h after the first injection to her left arm, the patient developed pruritus and erythematous papules at the injection site and painful papules on her hands. After immediate administration of antihistamines, the pruritus and papules remitted. One wk later, after injection in the right arm, skin lesions and pruritus were seen. After the third injection to the abdomen, the patient developed a rash, and after the fourth and fifth injections to other areas of the abdomen, injection-site papules were seen. The patient had no skin reactions for the next 12 mo, with the exception of injection-site papules. Hepatitis C virus RNA was negative after 12 mo of treatment. Resat; Sahin, Fikret; Yurdaydin, CihanAims: This study was conducted to clarify the resistance profile of a novel mutation pattern emerging during lamivudine (3TC) therapy and showing cross-resistance to adefovir dipivoxil (ADV) in a patient with chronic hepatitis B. Methods and results: Successful suppression of hepatitis B virus (HBV) replication by sequential therapy of 9 MU thrice weekly interferon (IFN) and 3TC was followed by genotypical resistance detected at month 28 of therapy (month 19 of lamivudine treatment). ADV was added to 3TC therapy on month 44 of antiviral treatment. Neither alanine aminotransferase normalization nor a stable decrease in HBV viral load was observed, although ADV was used for more than 40 months. The HBV pol region was amplified from serum samples obtained before and after ADV treatment. The complete genome was cloned into a TA vector. PCR products and 7-10 clones from each cloned vector were sequenced. A novel mutation, A181S, in the reverse transcriptase gene leading to a conversion of W172C in the overlapping surface antigen gene was detected along with a M204I mutation. The complete genome comprising the A181S + M204I pattern was cloned into an expression vector and its in vitro susceptibility to 3TC, ADV, tenofovir (PMPA), clevudine (L-FMAU) and emtricitabine (FTC) were determined in transiently transfected Huh7 cells. This mutation pattern displayed more than 1,000-fold resistance to the nucleoside analogues 3TC and FTC and approximately sixfold resistance to L-FMAU, while it confers 28.23- and 5.57-fold resistance for the nucleotide analogues ADV and PMPA, respectively. Conclusion: A new mutation pattern, A181S + M204I, arising under lamivudine treatment confers cross-resistance to ADV both in vivo and in vitro.Öğe A Splenic Hydatid Cyst Case Presented With Lumbar Pain(Japan Soc Internal Medicine, 2006) Çelebi, Selman; Başaranoğlu, Metin; Karaaslan, Hayri; Demir, AliA splenic hydatid cyst is a rare clinical entity from among abdominal hydatid cysts, even in endemic countries. Here, a case with lumbar pain due to a giant splenic hydatid cyst is presented. The importance of this case is that the patient presented at the clinic with only lumbar pain. Initial direct abdominal plain radiography showed a giant abdominal calcification in the spleen and further examinations revealed involvement of three organs: spleen, lung, and liver.Öğe Ultrasonografik İncelemede Karaciğer Yağlanması Tespit Edilen Vakaların Klinik ve Laboratuar Değerlendirmeleri(2002) Karaaslan, Hayri; Kaya, Ahmet; Poyraz, ÖmerKolay uygulanabilir görüntüleme yöntemlerinin gelişmesi sayesinde yağlı karaciğer tanısı sık olarak konulabilmektedir. B-mode ultrasonografik görüntüleme bunlar içinde çok kullanılanıdır. Her yerde bulunabilmesi, ekonomik ve noninvaziv olması tercih sebepleridir. Çalışmamızda bu yöntemle yağlı karaciğer tanısı almış olan bütün vakaların önce sistemik fizik muayeneleri yapılarak beden kütle indeksleri tespit edildi. Her vakadan rutin tetkikler, protein elektroforezi, lipid elektroforezi, tiroid fonksiyon testleri ve hepatit belirleyicileri için kan alındı. Bu vakalara ayrıca karaciğer biyopsisi yapıldı ve alınan parçalar ışık mikroskopu ile değerlendirildi. Hastalardan elde edilen sonuçlar, ayni yaş grubundaki sağlıklı kişilerden elde edilen değerlerle karşılaştırılarak ultrasonografik görüntülemenin yağlı karaciğerin kesin tanısı için yeterli olup olamayacağı araştırılmıştır. Çalışmamız sonucunda, karaciğer yağlanmasının tanısını koymak için yalnız ultrasonografik tetkikin yapılmasının yeterli olmadığı ve mutlaka karaciğer biyopsisinin de yapılmasının gerektiği kanaatine varılmıştır. Bmode ultrasonografi, karaciğer biyopsisi yapılarak karaciğer yağlanması tanısı almış olan vakaların tedavilerinin takibinde değerli noninvaziv bir görüntüleme yöntemidir.Öğe Viral Hepatitis at the Black Sea Region: The Problem of Viral Hepatitis in Turkey Revisited(Turkish Soc Gastroenterology, 2009) Karaaslan, Hayri; Yurdaydın, CihanThe impact of epidemiological studies can be striking and may lead to significant scientific advances as is the case with the discovery of the connection of chronic viral liver disease and hepatocellular carcinoma which has been regarded as one of the most important scientific achievements of the second half of the 20th century (1). Important risk factors had been identified and preventive measures based on these results implemented. We have started to see the fruits of these studies and will continue to witness them (2, 3).
