    An anatomic study for a modified technique for bypass of the external carotid artery to the proximal middle cerebral artery

    We aimed to evaluate whether bypass of the external carotid artery (ECA) to the middle cerebral artery (MCA) can be established by a short saphenous vein graft in order to increase the anastomosis patency. The method was performed to ten adult cadaver sides. We described a modified technique for bypass of the ECA to the M2 segment of MCA. The diameters of the vessels and graft length were measured by using an electronic micrometer. The mean diameter of the superior, middle, and inferior trunks of the MCA with trifurcation were 1.7 +/- 0.15, 2.2 +/- 0.25, and 2.0 +/- 0.2 mm, respectively, whereas the mean diameter of the superior and inferior trunks of the MCA with bifurcation were 2.1 +/- 0.2 and 2.3 +/- 0.3 mm, respectively. The mean diameter of the ECA was 3.75 +/- 0.4 mm. The mean length of the saphenous vein graft was 71.5 +/- 3.9 mm. The high-flow ECA to proximal MCA bypass using a short venous graft can supply enough blood flow to establish cerebral revascularization with a straighter route.

    The anatomical relationship between the eustachian tube and petrous internal carotid artery

    Objectives/Hypothesis: The aim of the present study was to investigate the relationship between the eustachian tube (ET) and petrous internal carotid artery (ICA) in whole-mount human temporal bone specimens. Study Design: Descriptive study. Methods: Histologically prepared serial sections of 10 adult temporal bones were included in the study. Five specific landmarks were selected to evaluate relationships between the petrous segment of the ICA and the ET. The selected distances were measured using computer software (Metamorph 7.5.2.0; Molecular Devices, LLC, Sunnyvale, CA). Results: The ET and the ICA get close posteriorly, and the bony part of the ET and the ICA generally share the same wall. Conclusions: The junctional part of the ET may be a safe landmark to identify and protect the ICA during endoscopic endonasal surgery of the cranial base. Knowledge of the anatomical relationships of the ET and petrous part of the ICA, as well as their relationship with other surgical and radiological landmarks, would be useful to surgeons.     Attention deficit hyperactivity disorder symptoms in children with vocal fold nodules

    Objectives: The aim of this case-control study was to investigate the symptoms of attention-deficit/hyperactivity disorder (ADHD) in children with vocal fold nodules (VNs). Methods: Study group (SG) included children between 4 and 12 years. As a control group (CG), children between 4 and 12 years without VNs were included in the study. Parents of the participants completed the Conners' Parent Rating Scale-Revised: Short Form (CPRS-RS) which was used to analyze the symptoms of ADHD. Results: Forty-five children (30 boys and 15 girls) with VNs and 45 controls (30 boys and 15 girls) were enrolled in the study. Multivariate analysis of variance revealed that the CPRS-RS Hyperactivity and Oppositional Subscales were significantly higher in the SG than the CG (p < .05), after controlling the effects of age and gender. Conclusions: Our findings suggest associations between VNs and hyperactivity and oppositional behaviors in children. Clinicians should be aware of ADHD symptoms in children with VNs.     Comparative evaluation of different miniplates for internal fixation of mandible fractures using finite element analysis

    Purpose: In this computer-based study, finite element analysis (FEA) was performed to assess the most suitable shape and fixation technique for a certain type of mandible fracture at corpus. Materials and Methods: A model of the mandible was prepared using computed tomography (CT) scans. The CT scans were transferred and converted to the finite element model by means of a procedure developed for this study. Simulated corpus fractures were fixed with 14 different fixation configurations of titanium miniplates. The FEA was performed with respect to displacement and stresses in the titanium miniplates for these configurations. Results: The study results indicated that the use of 2 straight miniplates is more rigid than other fixation types. Fixation with only I miniplate at the inferior location resulted in mobility greater than the set limit of 150 mu m. With "L" and "T" shaped miniplates and a straight miniplate at the middle location, fracture mobility was approximately equal to or less than the limit; however, displacement nearly reached to limit. Superior fixations with only 1 miniplate resulted in mobility lower than the limit. But these configurations exceed the yield limit of titanium miniplate. Conclusions: FEA may be useful in evaluation of other plate constructs, fracture types, and fracture sites, as confirmed by the agreement between our data and those in the literature and with clinical experience. This analysis should permit us to suggest and evaluate new miniplate designs and enable considerable savings to be made in terms of time, material, and animal experiments in the future development of osteosynthesis materials and techniques.     Conchae Bullosis in a Pediatric Patient

    Pneumatization of the turbinates called concha bullosa is one of the most frequent anatomic variations of the nasal cavity. We report the first case of computed tomography findings of bilateral middle and inferior concha bullosa in a 13-year-old child with nasal obstruction. Here we describe a patient with extreme bilateral bullosa of inferior and middle conchas, as well as crista galli. The patient was treated successfully with endoscopic surgery of conchas. Nasal obstruction secondary to a bilateral turbinate bullosis in a child has not been described before. The clinicians should take this entity into consideration when evaluating the pediatric patients with nasal obstruction.

    Do Mucosal Folds in the Eustachian Tube Function as Microturbinates?

    Objectives/Hypothesis: Mucosal folds in the eustachian tube (ET) may function as microturbinates and provide protection and clearance. Study Design: Descriptive study. Methods: Nineteen whole-mount temporal bone ET specimens (nine pediatric and 10 adult) were evaluated in the present study. Five specific localizations for each case were selected. The cartilaginous segment of the ET was divided vertically and the length of mucosal surfaces and the number of mucosal folds of anterior and posterior walls on the histological sections were analyzed with MetaMorph 7.5.2.0 software. Results: The length of the mucosal surface of the posterior wall of the ET was longer than the anterior wall (P<.05). In pediatric specimens, the posterior wall had more mucosal folds than adult specimens (P<.05). Conclusions: Microturbinates in the posterior wall of the ET may provide important protection and clearance functions for children and play a role in the pathogenesis of eustachian tube dysfunction.

    The effects of rapid maxillary expansion on voice function

    Objective: To evaluate the effects of rapid maxillary expansion (RME) on vocal function in patients with bilateral maxillary crossbite. Materials and Methods: We designed our research as a prospective, controlled, clinical study. The treatment group and the control group each had 20 subjects for a total of 40 subjects. Acoustic voice samples were recorded from all patients at T-1 and T-2 by the Multi-Dimensional Voice Program (MDVP Model 5105) for acoustic analysis in Computerized Speech Lab (CSL). Results: No statistically significant differences were found between the treatment and control groups in the means of any parameters. Conclusions: RME does not change vocal quality or resonance, so it can be safely used with patients.

    The effects of spinal anesthesia on auditory patients

    Objectives: Persistent or transient hearing loss (HL) is a less-recognized complication of spinal anesthesia (SA) in the pediatric population, although it has been previously reported in adults. The primary aim of this study was to investigate the effects of SA on auditory function in the pediatric population. Methods: After gaining institutional approval and parental consent, 30 American Society of Anesthesiologists physical status I-II children between 4 and 15 years undergoing lower extremity orthopedic surgery were enrolled in this prospective study. Spinal blocks were performed in the midline with a 25G Quincke needle using 0.5% hyperbaric bupivacaine. Transient evoked otoacoustic emission (TEOAE) and distortion product otoacoustic emission (DPOAE) tests were administered before surgery and one-day postoperative. Children with detected HL were retested on postoperative day seven. Preoperative and postoperative results were compared. A Wilcoxin Signed-Ranks test (with Bonferroni correction) was used for statistical analyses. Results: There was no statistically significant HL in the postoperative period compared to the preoperative period. In 29 of 30 patients, no difference was detected at any frequency tested. In one patient, TEOAE and DPOAE tests were found to be decreased on postoperative day one. In this patient, control tests were found to be improved on postoperative day seven. Conclusions: Administration of SA may results in a low probability of transient hearing loss with no clinical significance in children 4-15 years of age. (C) 2016 Elsevier Ireland Ltd.     Esophageal Stricture in a Child With Habit of Acidic Drinks

    A 12-year-old boy with Down syndrome referred Dysphagia was especially with solid foods and had a 12-month history. When he was 7, he had started to drink Coke and from that time he had a Coke drinking habit. Every day he was drinking more than 2 L Coke and was eating meals in small amounts. Investigations of the patient revealed esophageal stricture related to acidic drink habit and gastro-oesephageal reflux. The patient was treated with recurrent bouginage, antireflux medication, and nutrional support. The patient became symptom free from 13 months and gained weight. The clinicians should take the nutrition habits into consideration when evaluating the esophageal stricture patients.

    Evaluation of HLA-A, -B, -Cw, and -DRB1 alleles frequency in Turkish patients with nasal polyposis

    OBJECTIVE: To evaluate whether there is a relationship between HLA-A, -B, -Cw, and -DRB1 alleles and developing nasal polyposis (NP). STUDY DESIGN: Data from 66 patients with NP were compared with data from 100 healthy randomly selected controls. Asthma, ASA (acetylsalicylic acid) triad, polyp score, and previous sinonasal surgery were also recorded. SUBJECTS AND METHODS: Genotyping of the HLA-A, -B, -Cw, and -DRB1 alleles were performed with polymerase chain reaction (PCR) with the sequence-specific primer (SSP) method. Data were analyzed by using a Pearson chi(2) test. RESULTS: The HLA-B*07 and -Cw*12 alleles were found to be significantly higher in the NP patients compared with the control group, whereas the HLA-B*57 and HLA-Cw*04 alleles were significantly lower (P < 0.05). The HLA-A*24, HLA-Cw*12, and HLA-DRB1*04 alleles were determined to be significantly higher in the NP patients with asthma and ASA triad (P < 0.05). CONCLUSIONS: Our results show that some of the HLA alleles seem to be associated with the genetic susceptibility to develop NP in the Turkish population.     Feasibility of Endoscopic Submandibular Ganglion Neurectomy for Drooling

    We performed endoscopic transoral neurectomy of the submandibular and sublingual glands to treat drooling. We bilaterally operated two adult cases with treatment-resistant drooling. In these patients, conventional treatment had failed. Repeated botilinum toxin type A (BOTOXVR, Abdi Ibrahim Pharmaceutical Company, Istanbul, Turkey) injections had been effective but were becoming less so. The patients benefited from surgery in that their saliva scores decreased. No issue emerged over 6 months of follow-up. Endoscopic transoral neurectomy of the submandibular and sublingual glands reduces saliva production and allows management of drooling in treatment-resistant patients.

    Foreign Body of Submandibular Gland

    Obstructive adenitis is common disorder of submandibular gland. Sialolithiasis is the most common etiologic factor causing obstruction. If a sialolith is not detected, chronic obstruction of salivary glands is called nonspecific sialoadenitis. Foreign body-associated sialoadenitis of submandibular gland is very rare within the literature. In this study, the authors presented a patient having an unrecognized foreign body in the submandibular gland beside the duct that was causing chronic sialoadenitis. Foreign bodies must be explored and all suspected areas must be examined carefully for avoiding secondary problems and surgeries in the future.

    Ghrelin levels in children with adenoid or chronic tonsil hypertrophies before and after surgery

    Objective: We aimed to evaluate the ghrelin levels in the children with adenoid or tonsil hyperthrophies. Methods: The Study included 27 children (17 boys and 10 girls). Mean age was 6.9 +/- 3.5 years, ranging from 3 to 16. Ghrelin levels in the patients and their weight and height measurements were evaluated before surgery and after 3 months later of the operation. Results: While 18 (67%) children were operated for adenoid hypertrophy, 9 (33%) children were operated for adenoid and tonsil hypertrophy. it was found that postoperative ghrelin levels were significantly decreased whereas weight and BMI scores were significantly increased (p < 0.01). A weak correlation was observed between preoperative ghrelin and weight (r = -0.29), This negative Correlation became more profound at the postoperative 3rd month examination (r = 0.85) (p < 0.01). Conclusions: The present Study showed that the surgical treatment provides positive contributions on the growing of children with adenoid and tonsil hypertrophies. The ghrelin levels were significantly decreased at the postoperative period in the children, and a negative relationship was observed between the ghrelin levels and the weight, These findings Suggest that blood ghrelin levels may be useful as a parameter for following the development of the children.     Giant papillary carcinoma arising in the ectopic buccal thyroid tissue

    BackgroundEctopic thyroid tissue very rarely occurs in the buccal region. Even more rarely encountered is the papillary carcinoma arising in the ectopic tissue. Methods and ResultsWe herein present a 78-year-old female patient admitted with a giant, buccal mass that developed within the previous 2 years. Physical examination revealed a vegetative mass on the left buccal region without any cervical lymphadenopathy. Magnetic resonance imaging scan showed an 8- x 6-cm solid mass with cystic components in the left buccal region and thoracic computed tomography scanning revealed pulmonary metastasis. A successful surgery including lower lip splitting was performed, and the mass was totally excised with its surrounding capsule. Histopathologic examination revealed follicular variant of papillary carcinoma of the ectopic thyroid tissue. ConclusionsTo our knowledge, our case is the first case of papillary carcinoma arising from ectopic thyroid tissue in the buccal region in the literature. (c) 2012 Wiley Periodicals, Inc.     Noninvasive Treatment of Acquired Subglottic Stenosis

    A 23-day-old infant referred to the neonatal intensive care unit with difficulty breathing and inspiratory stridor increasing with exercise. Medical history included a cardiothoracic surgery for transposition of the great arteries, patent ductus arteriosus, atrial septal defect, and a history of intensive care unit from surgery. Flexible fiberoptic transnasal laryngoscopy revealed subglottic stenosis that was probably caused by prolonged intubation with a higher airway pressure. Computed tomographic scan of the neck showed a tiny stenosis without cartilage deformity and limited in subglottic region. Tracheotomy and external open surgery was found risky for sternal wound infection or mediastinitis as the neonate had newly thoracotomy. The patient had a balloon dilation under general anesthesia without intubation. Presenting symptoms of the neonate were fully improved with balloon dilation.

    Permanent Quadriplegia Following Replacement of Voice Prosthesis

    The authors presented a patient with quadriplegia caused by cervical spine abscess following voice prosthesis replacement. The authors present the first reported permanent quadriplegia patient caused by voice prosthesis replacement. The authors wanted to emphasize that life-threatening complications may be faced during the replacement of voice prosthesis. Care should be taken during the replacement of voice prosthesis and if some problems have been faced during the procedure patients must be followed closely.

    Relation of glutathione S-transferase genotypes (GSTM1 and GSTT1) to laryngeal squamous cell carcinoma risk

    The purpose of the present study was to investigate GSTM1 and GSTT1 genotypes by using multiplex polymerase chain reaction (PCR) in patients with laryngeal squamous cell carcinoma (LSCC). The genotypes of 110 patients with LSCC and of 197 healthy subjects as the control group were determined by PCR analysis for GSTM1 and GSTT1 genes. Results showed that frequencies of GSTM1-null, GSTT1-null, and both GSTs-null genotypes were 51.8, 30, and 16.4%, respectively, in the patients with LSCC and 37.6, 15.7, and 5.6% in the control group. There was a significant difference between the genotype distributions of all GSTs in patients and in control groups (P < 0.05). The results support the hypothesis that null genotypes of GSTM1 and GSTT1 can reduce detoxification capacity of GSTs as members of the xenobiotic enzyme system. GSTM1-null, GSTT1-null, and both GSTs-null genotypes were more common in the patients with LSCC than in the control group. Patients with both GSTs-null genotypes had the highest risk for supraglottic LSCC in the early period, even if they were light-to-medium smokers. Investigation and determination of the genetic basis of LSCC may contribute to detection of risk groups and to prevent LSCC in the population. (c) 2006 Elsevier Inc. All rights reserved.

    Release of NT-pro brain natriuretic peptide in children before and after adenotonsillectomy

    Objectives: To measure serum B-type natriuretic peptide, especially N-terminal segment of its prohormone (NT-proBNP) and pulmonary arterial pressure levels and to determine whether NT-proBNP concentrations correlate with pulmonary arterial pressure levels in children before and after adenotonsillectomy. Methods: Twenty children with adenoid and tonsil hypertrophy and 20 healthy subjects were included in the study. Transthoracic echocardiography was performed in all patients to assess pulmonary artery systolic and diastolic pressure with cardiac dysfunction. The NT-proBNP was analyzed for correlation with pulmonary artery pressure. Results: Our results showed that prohormone serum concentrations and pulmonary arterial pressures were significantly higher in the study group than in the control group at the preoperative evaluation. A significant decrease was detected among patients after surgery (p < 0.05). There was no significant difference between the study and control groups for prohormone serum concentration and pulmonary arterial pressure at the postoperative evaluation (p > 0.05). Conclusion: We found increased serum prohormone levels and height pulmonary artery pressures in the children with adenoid and tonsil hypertrophy when comparing with healthy subjects. Our study supported that increased serum prohormone levels and pulmonary artery pressures as a result of adenoid and tonsil hypertrophy are reversible. Pediatric cardiologs and otolaryngologs should keep in mind an increased pulmonary artery pressure during management of children with adenoid and tonsil hypertrophy. Furthermore, Doppler echocardiography may be useful in the monitoring of pulmonary arterial pressure and in the follow-up of surgical outcome of children with adenoid and tonsil hypertrophy.
