Öğe Asymmetric dimethylarginine levels in Behcet's disease(WALTER DE GRUYTER GMBH, 2014) Ozturk, Bahadir; Vatansev, Husamettin; Taner, Alpaslan; Yilmaz, Sema; Dagli, Mehmet; Unlu, AliObjective: Behcet's disease (BD) is a chronic, inflammatory, multisystem vasculitis and the etiology is not yet fully understood. Nitric Oxide (NO) is an important molecule for the vascular system which synthesised by the Nitric Oxide Synthase (NOS) enzyme. Asymmetric dimethylarginine (ADMA) is an endogenous inhibitor of NOS. Therefore, we aimed to investigate levels of the ADMA and its relation with exacerbations in BD patients. Methods: The subjects enrolled in this study were recruited from 34 patients with BD and 34 healthy controls. Serum levels of ADMA, arginine and citrulline were measured by HPLC with fluorescence detection. The ESR and CRP levels analyzed by routinely used methods. Results: In contrast to information in the literature, ADMA levels were found to be decreased in BD patients when compared to control. Inversely, Arginine and Citrulline levels were significantly increased in BD patients. Increased Arginine/ADMA ratios were found in the patient group. There was an inverse relation between ADMA and CRP levels. Arginine values were also correlated CRP and Citrulline levels. Conclusion: One of the reasons of high number of BD attacks in young age may be low levels of ADMA. Our study suggests that the reduced levels of ADMA in patients group may impose a possible preventive role to ADMA through decrease of NO-mediated inflammation and exacerbations of BD with aging.Öğe Behçet hastalarında asimetrik dimetilarginin düzeyleri(2014) Öztürk, Bahadır; Vatansev, Hüsamettin; Taner, Alpaslan; Yılmaz, Sema; Dağlı, Mehmet; Ünlü, AliAmaç: Behçet Hastalığı (BH), etiyolojisi tam olarak anlaşılmamış, kronik, inflamatuvar multisistemik bir vaskülittir. Nitrik Oksit (NO), endotel hücrelerinde Nitrik Oksit Sentaz (NOS) enzimi tarafından sentezlenen, vasküler sistem için önemi olan bir moleküldür. Asimetrik dimetilarjinin (ADMA) NOS'ın endojen inhibitörüdür. Bu çalışmada BH'larında ADMA düzeyleri ve ataklarla ilgisinin araştırılması amaçlandı.Metod: 34 Behçet hastası ve 34 sağlıklı kontrol çalışmaya dâhil edildi. Serum ADMA, arjinin ve sitrülin düzeyleri HPLC yöntemi ile saptandı. ESR ve CRP düzeyleri rutin yöntemlerle analiz edildi.Bulgular: Literatür bilgilerine ters olarak Behçet hastalarında ADMA düzeyleri kontrol grubuna göre düşük bulundu. Bunun aksine Arjinin ve Sitrülin düzeyleri Behçet hastalarında anlamlı derecede yüksek bulundu. Hasta grubunda Arjinin/ADMA oranları yüksek bulundu. ADMA ve CRP düzeyleri arasında ters bir korelasyon vardı. Arjinin değerleri de sitrülin ve CRP düzeyleri ile koreleydi. Sonuç: BH da genç yaşta atakların sayısının fazla olmasının sebeblerinden birisi düşük ADMA düzeyleri olabilir. Oxidation of LDL activates many inflammatory and atherogenic pathways and plays a pivotal role in atherosclerosis. Our aim in this study is to compare the effects of 80 mg daily dose of atorvastatin and 20 mg daily dose of rosuvastatin on lipid profiles and the levels of oxidized-LDL and inflammatory markers in ST elevation myocardial infarction (STEMI). Methods: One hundred and twenty patients with STEMI were enrolled in this study. The patients were randomly assigned to receive atorvastatin (80 mg/day) or rosuvastatin (20 mg/day) by using a ratio of 1: 1 after revascularization. The levels of total cholesterol (TC), triglyceride (TG), high density lipoprotein cholesterol (HDL-C), LDL-C, apolipoprotein B and apolipoprotein A were compared between groups after 4-week therapy. The values of oxidized-LDL, tumor necrosis factor receptor 1 and 2, Interleukin-6 and hs-CRP were also compared between groups. The Student's t test was used to detect absolute and percent changes between groups, and p < 0.05 was considered as statistically significant for all tests. Results: After treatment in both treatment groups LDL-C, oxidized-LDL, hs-CRP, tumor necrosis factor receptor 1 and 2, Interleukin-6 values significantly decreased according to baseline. The only difference was in HDL-C levels. HDL-C slightly decreased in atorvastatin group while it increased in the rosuvastatin group compared baseline ( 1.4 +/- 8.9 mg/dl vs 2.0 +/- 9.4 mg/dl, p = 0.04). Conclusion: We reported that both statin treatment regiments have comparable effects on LDL-C, oxidized-LDL and inflammatory markers. Moreover, it was observed that rosuvastatin was more effective in terms of ability to increase HDL-C level. Based on these findings, 20 mg daily dose of rosuvastatin may be an alternative to 80 mg daily dose of atorvastatin in patients with acute coronary syndrome. (C) 2015 Elsevier Ireland Ltd. All rights reserved.Öğe Diyabetik hastalarda maskeli hipertansiyonun gelişiminde asimetrik dimetil argininin rolü(Selçuk Üniversitesi Tıp Fakültesi, 2010) Taner, Alpaslan; Ünlü, AliHipertansiyon ve diyabet, toplumda sık karşılaşılan mortalite ve morbiditelerinin sıklığı ve komplikasyonlarına bağlı ciddi ekonomik ve toplumsal etkileri olan önemli bir hastalıktır. Asimetrik dimetilarjinin (ADMA), metillenmiş bir arjinin analoğu olup; NO sentezinin anahtar enzimi olan NO sentazın (NOS) endojen yarışmalı bir inhibitörüdür. Diyabet ve hipertansiyonda kan serum seviyeleri artmıştır. Çalışmamızda, henüz aşikar olarak hipertansiyon gelişmemiş diyabetik hastaların 24 saatlik kan basıncı takiplerini yaptık ve bu hastaların kan basıncı değerleriyle maskeli hipertansiyona sahip kişileri belirledik. Maskeli hipertansiyona sahip hastalarla normotansif hastaların serum ADMA, Arjinin seviyelerini ölçmeyi ve kan basıncı değerleriyle ilişkisini araştırmayı amaçladık. Diyabetik ve normotansif 112 hastaya 24 saatlik ambulatuar kan basıncı takibi uyguladık. Bu ölçümler yardımıyla hastalardan maskeli hipertansif olanlarla, normotansif olanları ayırarak bu kişilerin serum ADMA, Arjinin seviyelerini HPLC tekniği kullanarak ölçtük. 24 saatlik kan basıncı ölçümünde %25 oranında diyabetik hastada maskeli hipertansiyona rastladık. Maskeli hipertansif grupta ADMA ve Arjinin/ADMA oranları normotansif olanlardan anlamlı olarak yüksek bulunurken (p:0.000); Arjinin düzeyleri açısından böyle bir fark bulunmadı. Yüksek ADMA seviyeleriyle kan basıncı parametreleri arasında anlamlı pozitif korelasyonlar gözlendi. Artmış ADMA düzeyleri artmış kan basıncı değerleri ile birlikte gözlendi. Açlık kan glikozuyla ADMA yüksekliği arasında da pozitif ama zayıf bir korrelasyon görüldü. Diyabetik maskeli hipertansif hastaların serum ADMA seviyeleri artan kan basıncı ile ilişkilidir. Normotansif hastaların ambulatuar kan basıncı takibi ve serum ADMA seviyeleri ölçümleri maskeli hipertansiyon ve aşikar hipertansiyon gelişimi için anlamlı bilgiler vermektedir. ADMA düzeylerinin monitorizasyonu hipertansiyonun muhtemel komplikasyonlarını önlemede önemli bir paramatre olabilir.Öğe Gender-Related Changes of the Epicardial Fat Thickness and Leptin in Obstructive Sleep Apnea(WILEY, 2014) Akıllı, Hakan; Kayrak, Mehmet; Bekçi, Taha Tahir; Erdoğan, Halil Ibrahim; Arıbaş, Alpay; Yıldırım, Oğuzhan; Taner, AlpaslanBackgroundEpicardial fat thickness (EFT), an indicator of visceral obesity, and leptin are 2 novel markers for studying the obstructive sleep apnea (OSA) population. This study aimed to investigate the effects of gender on leptin levels and EFT, and the relation with OSA severity. MethodsA total of 149 patients with OSA (female/male 55/94 and mean age 50.89.2 years) and 50 control patients (female/male 24/26 and mean age 48.98.8 years) were included in the study. The study population was divided into 4 groups according to apnea/hypopnea index (AHI) as control (AHI <5), the mild OSA (AHI 5-14), the moderate OSA (AHI 15-29), and the severe OSA (AHI 30). EFT was obtained from parasternal long-axis and parasternal short-axis echocardiographic images. ResultsLeptin levels among females were significantly higher than among males (10.5 [7.8] vs. 5.4 [4.5] ng/mL, P=0.001, respectively). Among women, leptin levels were significantly higher in the severe OSA group compared to the control group (9.8 [9.0] vs. 15.5 [10.1] ng/mL, P=0.05, respectively). Conversely, no relation was observed between OSA severity and the leptin levels among men. EFT was not significantly different between the 2 genders (P>0.05). EFT was thicker in the severe OSA group than in the control and mild OSA groups among women, whereas EFT was not changed according to OSA severity among males (P>0.05). Dimetilarjinin Dimetilamino Hidrolaz (DDAH) enziminde bu genetik de ğ iş imin saptanmas ı ile bu hastal ı k grubunda daha ileri tedavi protokolleri geliştirilmesine katk ı sa ğ lamak için koroner kalp hastalar ı nda Asimetrik Dimetil Arjinin (ADMA) seviyeleri ve DDAH 1 (E.C. 3.5.3.18) enziminin T87M mutasyonu incelendi. Materyal ve Metot: Konya bölgesinde ya şayan 50 koroner kalp hastası ve 50 kontrol bireyi çal ı şmaya dahil edilmiştir. Serum örneklerinden ADMA yüksek performansl ı s ı vı kromatografisi, tam kan örneklerinden izole edilen DNA örneklerinden DDAH 1 T87M mutasyonu PCR yöntemi ile çal ı şı ld ı . Bulgular: ADMA düzeyleri hasta grubunda kontrol grubuna k ı yasla anlaml ı olarak yüksek bulunmu ş olup Arjinin/ADMA oranlar ı nda istatistiksel olarak bir farkl ı l ı k gözlenmemiş tir (Sı ras ı yla p0,001 ve p0.08). Hasta grubunda trigliserid düzeyleri kontrol grubuna k ı yasla anlaml ı olarak yüksek (p0,033), Yüksek yo ğ unluklu lipoprotein (HDL)-kolesterol seviyeleri ise dü ş ük tespit edilmiştir (p0,001). Hasta grubundan bir kiş ide DDAH 1 T87M gen polimorfizmi heterozigot bulunurken homozigot varyant hiçbir kat ı l ı mcı da gözlenmemiştir. Diğ er katı l ı mcı larda genin ilgili bölgesi wild tip olarak belirlenmiştir. The ratio of L-arginine/asymmetric dimethylarginine (ADMA) and homocysteine are important factors for endothelial function. In this study, we investigate the ratio of L-arginine/ADMA, homocysteine, and folic acid/vitamin B levels in patients with CAE. Methods Forty patients diagnosed with CAE using coronary angiography were included in the study (24 male; mean age, 56 +/- 11 years). The control group consisted of 30 patients who had normal coronary arteries as determined by coronary angiography (11 male; mean age, 54 +/- 8 years). The ratio of L-arginine/ADMA and plasma homocysteine was measured using high-performance liquid chromatography. Results The L-arginine/ADMA ratio and L-arginine levels were significantly lower in the CAE group compared with the control group (110 +/- 27 vs. 149 +/- 77, P = 0.02 and 157 +/- 32 mu mol/l vs. 187 +/- 59 mu mol/l, P = 0.02, respectively). Plasma ADMA levels were similar in the two groups. Patients with CAE had higher plasma homocysteine levels (P = 0.01). Plasma folic acid, vitamin B6, and vitamin B12 levels were similar between the two groups. Conclusion This study shows that patients with CAE have a lower L-arginine/ADMA ratio and higher plasma homocysteine levels. These results show a potential relationship between endothelial dysfunction and CAE.Öğe The value of serum asymmetric dimethylarginine levels for the determination of masked hypertension in patients with diabetes mellitus(ELSEVIER IRELAND LTD, 2013) Taner, Alpaslan; Unlu, Ali; Kayrak, Mehmet; Tekinalp, Mehmet; Ayhan, Selim S.; Aribas, Alpay; Erdem, Said SamiBackground: An increased prevalence of masked hypertension (MHT) has been demonstrated among patients with diabetes mellitus (DM). MHT appears to cause cardiovascular (CV) complications similar to clinically overt hypertension. Asymmetric dimethylarginine (ADMA) is an endogenous nitric oxide inhibitor and higher plasma levels of ADMA are related to increased CV risk in both the general population and among patients with DM. The aim of this study was to evaluate the relationship between MHT and ADMA in diabetic patients. Methods: This study included DM patients (n = 131) with normal office blood pressure (<140/90 mmHg). None of the participants were using antihypertensive medications. All participants utilized an ambulatory blood pressure monitor (ABPM) for 24 h. Serum ADMA and arginine levels were measured using the fluorescence detector high performance liquid chromatography method. Results: The prevalence of MHT was 24.4% among the study subjects. ADMA levels were increased in the MHT group when compared with normotensive diabetics (6.2 +/- 2.2 vs 4.2 +/- 1.7 mu mol/L p = 0.001, respectively). Furthermore, arginine/ADMA ratio was lower in the MHT group than among the normotensive group (29.9 +/- 12.1 vs 46.0 +/- 19.0 p = 0.001). In the multivariate logistic regression model, ADMA, BMI and HDL levels were found to be independent predictors of MHT Odds ratio: 1.63 (1.28-2.06), 1.19 (1.05-1.35), and 0.95 (0.90-0.99), respectively. The cut-off value of the ADMA was 4.34 mu mol/L with a sensitivity, specificity, positive predictive value, and negative predictive value of 84.4%, 59.6%, of 40.3%, and 92.2%, respectively (AUC - 0.78). Conclusions: Serum ADMA may play a role in both the pathophysiology and screening of MHT in DM subjects. (C) 2013 Elsevier Ireland Ltd. All rights reserved.
