Fibromiyalji hastalarında miyokin seviyelerinin fiziksel performans ve kognitif fonksiyonlarla ilişkisi

Özet
Fibromiyalji sendromu (FMS) yaygın vücut ağrısı, anksiyete, uyku bozukluğu, yorgunluk ve kognitif işlevlerde bozulma ile seyreden, yaşam kalitesini bozan, etyolojisi henüz net olarak açıklanamamış kronik bir hastalıktır. Miyokinlerin hipokampal nörogenezde, nörotrofin seviyelerinde, kognitif ve ruhsal durumda önemli rolü olduğu bilinmektedir. Fiziksel egzersizin kişilerin kognitif fonksiyonlarına olumlu etkileri olduğu da kanıtlanmıştır. Literatürde FMS'li hastalarda bu konuda yeterince çalışma yoktur. Çalışmada FMS'li hastaların en çok yakındığı semptomlar olan fibrofog, kas yorgunluğu ve güçsüzlüğünün etyopatogenezinde miyokinlerin etkilerinin araştırılması amaçlandı. Çalışmaya 40 FMS'li hasta ve 40 sağlıklı katılımcı alındı. Açlık kan örneklerinde irisin, PGC-1α, BDNF, CTSB, IL-6, IL-8 ve TNF-α seviyeleri analiz edildi. Depresyon, ansiyete ve uyku bozuklukları için Beck Depresyon ve Beck Anksiyete Ölçeği ve Pittsburg Uyku Kalite Ölçeği, yaşam kalitesi için Kısa Form 36 (SF36), bilişsel fonksiyonlar için Mini Mental Durum Testi ve Isaac'ın Sözel Akıcılık Testi, fiziksel aktivite düzeyi için Uluslararası Fiziksel Aktivite Anketi ve hastalık şiddeti için Fibromiyalji Etki Anketi dolduruldu. Fiziksel performansı değerlendirmek için Astrand Bisiklet Ergometresi Testi, 6 Dakika Yürüme Testi (6DYT), El Kavrama Gücü Ölçüm Testi (EKGÖT) ve İzokinetik Test uygulandı. Hasta grubunda tüm miyokinlerde belirgin düşüklük saptandı. Fiziksel performans testlerinden EKGÖT, 6 DYT ve fleksör kaslar için PT/VA değerleri hasta grubunda düşük bulundu. Mini Mental Durum Testi ile IL-8, SF36-fiziksel fonksiyon ölçeği ile IL-6 ve IL-8, SF36-fiziksel rol güçlüğü ile CTSB, 6 DYT ile PGC-1α ve dirsek ekstansör kaslar pik tork değerleri ile irisin ve CTSB arasında ilişki saptandı. FMS olmaya etki eden faktörler arasında irisin ve IL-8 anlamlı bulundu. Sonuç olarak, miyokinler FMS hastalığında tanısal biyobelirteç olma, semptom ve bulguların etyopatolojisini açıklamada umut vericidir.
Fibromyalgia syndrome (FMS) is a chronic disease that is characterized by widespread body pain, anxiety, sleep disturbance, fatigue and impaired cognitive functions, and impaired quality of life. Etiopathogenesis has not yet been clearly explained. It is known that myokines have an important role in hippocampal neurogenesis, neurotrophin levels, cognitive and mental state. It has also been proven that physical exercise has positive effects on people's cognitive functions. There are not enough studies on this subject in the literature in patients with FMS. The aim of the study was to investigate the effects of myokines on the etiopathogenesis of fibrofog, muscle fatigue and weakness, which are the symptoms that patients with FMS complain about most. Forty patients with FMS and 40 healthy participants were included in the study. Irisin, PGC-1α, BDNF, CTSB, IL-6, IL-8 and TNF-α were analyzed in fasting blood samples. Beck Depression and Beck Anxiety Scale and Pittsburg Sleep Quality Index for depression, anxiety and sleep disorders, Short Form 36 (SF36) for quality of life, Mini Mental State Test and Isaac's Verbal Fluency Test for cognitive functions, International Physical Activity Questionnaire for physical activity level, Fibromyalgia Impact Questionnaire was applied to assess disease severity. Astrand Bicycle Ergometer Test, 6 Minute Walk Test (6MWT), Hand Grip Strength Measurement Test (HGST) and Isokinetic Test were performed to evaluate physical performance. Significant decreases in all myokines were detected in the patient group. Among the physical performance tests, HGST, 6 MWT and PT/BW values for flexor muscles were found to be low in the patient group. A relationship was detected between IL-8 with Mini Mental State Test, IL-6 and IL-8 with SF36-physical function scale, SF36-physical role difficulty with CTSB, PGC-1α with 6 MWT, and peak torque values of elbow extensor muscles with irisin and CTSB. Among the factors affecting FMS, irisin and IL-8 were found to be significant. In conclusion, myokines are promising as diagnostic biomarkers in FMS disease and in explaining the etiopathology of symptoms and findings.
