Özet
Bu çalışmada, adenotonsil hipertrofisi bulunan çocuklarda ameliyat öncesi ve sonrası ghrelin düzeyleri ve ghrelin ile büyüme-gelişme arasındaki ilişki araştırıldı. Klinik, radyolojik ve endoskopik inceleme sonucu adenotonsil hipertrofisi tanısı alan 27 çocuk hasta ameliyat öncesi ve ameliyattan 3 ay sonra incelendi. Çocukların boy ve kilo ölçümleri, ameliyat öncesi ve ameliyattan 3 ay sonra aynı şekilde yapıldı. Ghrelin ölçümü için Na EDTA ve aprotinin bulunan tüplere kan alınıp soğuk zincire dikkat edilerek çalışıldı. Çalışmaya; 17 erkek(% 63) , 10 kız (%37) toplam 27 hasta alındı. Yaş ortalamaları 6.8 olup, 3-16 yaş arasında değişmekte idi. Hastalardan 18 (% 67)' i adenoid vegetasyon, 9 (% 33)'u kronik tonsil hipertrofisi nedeni ile ameliyat edildi. Ameliyat öncesi ortalama ağırlıkları (21,96± 12,92) kg, ortalama vücut kitle indeksi(VKİ) (15.34 ± 3.27) kg/m², ortalama ghrelin düzeyleri (178,49 ± 92,58) fmol/ml olarak ölçüldü. Ameliyat sonrası ghrelin düzeyleri istatistik olarak anlamlı şekilde azalırken, ağırlık ve VKİ istatistiksel olarak anlamlı şekilde artış gösterdi ve sırası ile; (101,81± 51,50)fmol/ml, (22.76 ± 13)kg, (18 15.71 ± 3.26)kg/ m² olarak ölçüldü. Ghrelin ile ağırlık arasında operasyon öncesi zayıf bir negatif korelasyon (r = - 0.29) gözlenirken operasyon sonrasında ise bu negatif korelasyon daha da belirginleşti (r = 0.45, (p<0.05). Ghrelin ile ağırlık arasındaki bu korelasyon, ghrelin farkları ve ağırlık farkları arasındaki korelasyonla da (r = 0.85)S, (p<0.01) desteklendi. Çalışmamız, adenotonsil hipertrofili çocuklarda, cerrahi tedavinin hastaların ameliyat sonrası büyüme ve gelişmesine olumlu yönde katkı yaptığını göstermiştir. Ghrelin düzeyleri, hasta çocuklarda ameliyat sonrası anlamlı oranda azalmış ve kilo ile negatif ilişkisi belirginleşmiştir. Bu sonuç bize kan ghrelin düzeylerinin büyüme-gelişmenin 47 değerlendirilmesinde ve ameliyat sonrası takibinde faydalı bir parametre olarak kullanılabileceğini düşündürmüştür.
In the present study, we investigated the correlation between postoperative growing- development status and ghrelin levels, and ghrelin and these levels in children with adenoid or tonsil hypethrophy. Twenty-seven children with adenoid or tonsil hypertrophy, who were diagnosed by clinical, radiological and endoscopical examinations, were investigated. Weight and height measurements of the children were recorded preoperatively. All measurements were repeated postoperative 3 months. For ghrelin measurements, blood samples were taken into the tubes containing NaEDTA and aprotinin, and the samples were carried out by keeping cold chain. Totally 27 children (17 boys (63 %) and 10 girls (37 %)) were included into the study. Mean age level was 6.8, ranging from 3 to 16. Within all subjects, 18 (67 %) children with adenoid hypertrophy and 9 (33%) with tonsil hypertrophy were operated. Pre-operative averages weight, BMI and ghrelin levels were 21,96±12,92 kg, 15,34±3,27 kg/m and 178,49±92,58 fmol/ml, respectively. While postoperative ghrelin levels were significantly decreased, weight and BMI scores were significantly increased. These scores were 101,81±51,50 fmol/ml, 22,76±13 kg and 18 15,71±3,26 kg/m, respectively. Whereas a weak correlation was observed between ghrelin and weight (r =- 0.29) pre-operatively, this negative correlation became more clearly at the post-operative third month examination (r =0.85),(p < 0.01). The study showed that surgical treatment provides positive contributions to postoperative growing and development of children with adenotonsil hyperthropy. Ghrelin levels were significantly decreased postoperative period in children, and a negative relationship was observed between ghrelin levels and weight. These findings suggest that blood ghrelin levels can be useful as a parameter for evaluation of growing and developmental properties and for the post-operative follow-up period of children.
