Tiroid Fonksiyonunun Prematüre Bebeklerde Patent Duktus Arteriozus Üzerine Etkisi

Özet
Amaç: Patent duktus arteriyozus (PDA) ile konjenital hipotiroidi arasındaki ilişkinin araştırılmasıdır. Gereç ve Yöntem: Çalışmaya Zekai Tahir Burak Kadın Sağlığı Eğitim ve Araştırma Hastanesi Yenidoğan Ünitesinde Ocak 2015- Aralık 2017 tarihleri arasında yatan, doğumda gestasyon haftası <30 hafta ve doğum ağırlığı <1500 gr olan, 3. gün ekokardiyografisi yapılan ve 5-7. gün tiroid fonksiyon testleri bakılan infantlar çalışmaya dâhil edildi. Bulgular: Bu retrospektif tanımlayıcı çalışmaya toplam 178 prematüre infant dâhil edildi. Grup 1 (PDA olmayan) ’de 112 infant, Grup 2 (hemodinamik anlamlı PDA’sı olan)’de ise 68 infant mevcuttu. Serbest Tiroksin (sT4) düzeyleri açısından grup 1 ile grup 2 arasında istatiksel bir fark saptanmadı. Ancak grup 2 hastalarda alt grup incelemesinde cerrahi PDA ligasyonu yapılan bebeklerde sT4 düzeyi istatistiksel olarak anlamlı düşük bulundu (p= 0.05). Sonuç: Çalışmamızda medikal tedaviye dirençli bebeklerde sT4 düzeyinin düşük olduğu gösterildi. Medikal tedaviye yanıtsız PDA'lı bebeklerde hipotiroidi akılda tutulmalıdır.
Objective: The aim of this study was to investigate the relationship between PDA and congenital hypothyroidism. Material and Methods: Zekai Tahir Burak Women's Health Education and Research Hospital Newborn Unit was performed between January 2015 and December 2017. Infants with gestational age <30 weeks and birth weight <1500 g were included in the study. All infants was performed to Echocardiography on the 3rd day and on thyroid function tests were viewed on the 5th and 7th days. Results: A total of 178 prematurity infants were included in this retrospective descriptive study. There were 112 infants in Group 1 (non-PDA) and 68 infants in Group 2 (hemodynamically significant PDA). No statistically significant difference was found between group 1 and group 2 in terms of free thyroxine (sT4) levels. However, sT4 levels were significantly lower in infants who underwent surgical PDA ligation in group 2 patients (p = 0.05). Conclusion: Our results showed that the level of fT4 was low in infants who were resistant to medical treatment. Hypothyroidism should be kept in mind in infants with PDA who do not respond to medical therapy.
