Özet
Endometriozis üreme çağındaki kadınların yaklaşık % 6-10'unu etkile-yen bening bir jinekolojik hastalıktı. Endometriozis östrojene bağlı kronik inflama-tuar durumlardan kaynaklanan karmaşık bir hastalıktır. Östrojenin östrojen β reseptö-rü yoluyla kaynaklanan inflamasyon, altta yatan ana süreçtir. Endometriozis öncelik-le pelvik organları ve overleri etkilese de sistemik bir hastalıktır. Hastalığın molekü-ler ve patofizyolojik yolları açıkça belirlenmemiştir. İkiz çalışmalarda kalıtımın % 50 etkisi olduğu, bu nedenle endometriozis etiyolojisinin genetik faktörlerle ilişkili ola-bileceği gösterilmiştir. Günümüzde endometriozisin genetik ve epigenetik özellikleri Genom çapında birliktelik çalışmaları (GWAS) ile açıklanmaya başlanmış ve tek nükleotid polimorfizmlerinin (SNP) endometriozise yatkınlıkla anlamlı düzeyde iliş-kili olduğunu göstermiştir. Viseral adipoz dokudan salgılanan 415 aminoasit içeren vaspin, yeni bu-lunan bir adipokindir. Serum seviyeleri enflamasyon ile ilişkili olan serin proteaz inhibitörleri ailesinin bir üyesidir. Çalışmamız kesitsel olgu – kontrol çalışması olup 43 tane endometriosisli olgu 17 kontrol olgusu ve dahil edildi. Çalışma Selçuk üniversitesi tıp fakültesi, Ka-dın hastalıkları ve doğum kliniğinde 20.11.2019-20.05.2020 tarihleri arasında yapıl-dı. Endometriozis tanısı konduğunda, hastalığın evresi revize edilmiş Ame-rikan Üreme Tıbbı Derneği sınıflandırma sistemine (ASRM 1996) göre belirlenmiş-tir. Çalışma için sadece evre IV olan endometriozis hastaları seçildi. Endometriozisli hastalarda ve kontrol gruplarında vaspin rs2236242 polimorfiz-minin alellerinin ve genotiplerinin dağılımında istatistiksel olarak anlamlı fark vardı. Olgu ve kontroller arasında TA ve TT genotipleri sıklığında önemli farklılıklar gözlenmiştir. AT + TT genotipleri endometriozis riski ile ilişkilendirildi. Ayrıca, A aleli en-dometriozis riskini azaltır ve endometriozise karşı koruyucu bir etkiye sahiptir. Bununla birlikte, TT genotipinin endometriozis grubundaki bireylerde kontrol grubuna göre iki kat daha sık görüldüğü saptandı. Ayrıca endometriozisli bireylerde TA genotipinin prevalansının kontrol grubuna göre daha sık görülme eğilimi vardı, AA genotipi sadece kontrol grubunda bulundu. Sonuç olarak, sonuçlarımız vaspin rs2236242 gen polimorfizminin endometri-ozis riski ile güçlü bir şekilde ilişkili olduğunu göstermektedir.
The Relationship Between Endometriosis and Vaspin rs2236242 Gene Polymorphism Endometriosis affects approximately 6-10% of reproductive age, a benign gynecological disease. Endometriosis is a complex complex disease from chronic inflammatory conditions due to estrogen. Inflammation is the main underlying pro-cess. Although endometriosis primarily affects the pelvic organs and ovaries, it is a systemic disease. The disease was determined from molecular and pathophysiologi-cal routes. It is because inheritance is 50% effective in twin studies, this endometrio-sis etiology may be related to genetic factors. Genetic and epigenetic properties of endometriosis today. Vaspin, containing 415 amino acids secreted from visceral adipose tissue, is a newly found adipokine. From a member of the family of serine protease inhibitors. Our study was a cross-sectional case-control study, 43 cases with endometrio-sis and 17 control cases and were included. The study was carried out between 20.11.2019-20.05.2020 in Selcuk University Medical Faculty, Obstetrics and Gyne-cology Clinic. When the diagnosis of endometriosis was made, the stage of the disease was determined according to the revised American Reproductive Medicine Association classification system (ASRM 1996). Only patients with stage IV endometriosis were selected for the study. There was a statistically significant difference in the distribution of alleles and genotypes of vaspine rs2236242 polymorphism in patients with endometriosis and control groups. Significant differences were observed in the frequency of TA and TT genoty-pes between cases and controls. AT + TT genotypes have been associated with the risk of endometriosis. Al-so, the A allele reduces the risk of endometriosis and has a protective effect against endometriosis. However, it was found that TT genotype was observed twice as often in indi-viduals in the endometriosis group than in the control group. Also, the prevalence of TA genotype in individuals with endometriosis tended to be seen more frequently than the control group. The AA genotype was found only in the control group. In conclusion, our results show that vaspin rs2236242 gene polymorphism is strongly associated with the risk of endometriosis.
