Akut pulmoner emboli teşhisinde plazma galectin-3 seviyesinin prognostik ve prediktif değeri

| dc.description.abstract | Bu çalışmamızda, hayatı tehdit edici ve görece sık karşılaşılan acil bir kardiyovaskuler patoloji olan akut PE'nin patogenezini daha fazla aydınlatabilmek, erken tanı ve risk sınıflamasına katkıda bulunabilmek amacıyla ventriküler remodeling ve fibrozis ilişkili Galectin-3 düzeylerinin akut PE hastalarında benzer yaş ve cinsiyet özelliklere sahip olan kontrol grubu ile kıyaslanması hedeflenmiştir. Tanımlayıcı ve kesitsel çalışmamıza, Ocak-2012 ile Temmuz-2012 tarihleri arasında Selçuk Üniversitesi Tıp Fakültesi Hastanesi'nde akut PE tanısı konan 40 hasta ile sağlıklı 29 gönüllü dâhil edildi. Çalışmaya alınan hastaların tamamının Galectin-3 değerleri ELİSA yöntemi ile çalışıldı. Hasta ve kontrol grubunun geleneksel iki boyutlu ekokardiyografik parametreleri kaydedildi. Hasta ve kontol grubunda elde edilen değişkenler karşılaştırıldı. Hasta ve kontrol grupları arasında yaş ve kadın cinsiyet açısından istatiksel farklılık yoktu. Galectin-3 düzeyinin akut PE hastalarında (17,04±6,61 ng/ml), kontrol grubuna(8,13±4,93 ng/ml) göre istatiksel olarak anlamlı düzeyde artmış olduğu görüldü (P<0.0001). Galectin-3 kestirim değeri 12,3 ng/ml olarak alındığında PE olgularını tanıma yönünden duyarlılık 0.76, özgüllük 0.87, pozitif tahmin değeri 0.85, negatif tahmin değeri 0.80 ve tanısal doğruluğu 0.82 olarak hesaplandı. Galectin-3 daha önce yapılan çalışmalarda sol kalp yetmezliğinde ve remodelingdeki rolü gösterilmiştir. Biz bu çalışmamızda akut pulmoner emboli sonrası gelişen Sağ kalp yetmezliğindeki rolu ilk olarak gösterilmiştir. Prediktif ve prognostik değeri açısından ileri çalışmalara ihtiyaç duyulmaktadır. | |
| dc.description.abstract | The main aim of the study was to evaluate plasma galectin-3 levels that is associated with ventricular remodeling and fibrozis in predicting early diagnosis and risk stratification of pulmonary embolism(PE) which is life threatening and relatively frequent emergency cardiovascular pathology. The galectin-3 levels in patient with PE were compared with control group with similar age and sex. We also aimed to investigate differences in conventional and tissue Doppler echocardiographic parameters in patient with PE and their relationship with galectin-3 levels. Our descriptive and cross-sectional study populution included 29 healthy voluntary controls and 40 patients with diagnosis of PE who were admitted to Selcuk University, School of Medicine Hospital between January 2012 to July 2012. The galectin-3 levels of all of the PE patients and 40 patients of the control group were quantified by ELISA method .In addition to two-dimensional and Doppler echocardiaographic parameters, tissue Doppler parameters were also recorded and these parameters were compared between patient and control groups. There was not statistically significant differencebetweentwogroups with regard to age and femalesex . Galectin-3 levels were significantly elevated in acute PE group compared to control group; (17,04±6,61 ng/ml), (8,13±4,93 ng/ml) , respectively (p< 0.0001). When galectin-3's cut-off value is accepted as 12.3 ng/ml, the test sensitivity is 0.76 and the specificity is 0.87, the positive predictive value is 0.85, the negative predictive value is 0.80, and the diagnostic accuracy is 0.82. In addition to previous studies which showed association of galectin-3 with left heart failure and remodeling in the present study, we demonstrated a role of galectin-3 in right heart failure at pulmonary embolism for the first time. Further studies are needed to clarify the predictive and prognostic value of galectin-3. | |
