Özet
Dönme epileptik nöbetin nadir görülen bir belirtisidir. İdiopatik jeneralize ve parsiel epilepsilerin seyrinde görülebilir. Burada tipik bir dönme epilepsisi (volvuler epilepsi) sunulmuştur. Olgu: 19 yaşında erkek hastanın 6 yaşından beri jeneralize nöbetleri mevcuttu. Önceleri ataklar jeneralize iken 4 yıldır dönme nöbetleri de eklenmişti. Bu esnada hasta kendi ekseni etrafında, soldan sağa 3-4 kez dönüyordu. Ataklar ani, beklenmedik bir gürültü ile ortaya çıkıyordu. Manyetik rezonans görüntülemede bilateral parietal ve sol parieto-temporal bölgelerde ensefalomalazik lezyon dikkat çekiyordu. Elektroansefalografide parieto-temporal bölgelerde solda hakim keskin karakterli yavaş dalgalar görülmekte idi.
Circling behavior is a rare manifestation of an epileptic seizure. It may be seen both in idiopathic generalized and in partial epilepsies. Here we reported a typical case of volvular epilepsy. A 19 year old man had epileptic attacks from the age of 6 years. Previously the attacks were generalized in nature, circling seizures were added for 4 years. During the attack he was walking repetetively tree to four times around his axis from left to right. The attacks were precipitated with sudden unexpected noise. The magnetic resonance imaginary showed bilateral parietal and left parieto-temporal encephalomalasic lesions. The electroencephalography showed sharp and slow waves on parietotemporal regions predominantly on left side.
