Abstract
Amaç: Bu çalışmanın amacı; bariatrik cerrahi adayı obez hastaların, cerrahi öncesi benlik saygısı, cinsel yaşam ve diğer psikopatolojiler açısından değerlendirilmesi, bunların sağlıklı kontrollerle karşılaştırılması ve cerrahi sonrası 1., 3. ve 6. aylardaki değişikliklerinin araştırılmasıdır. Gereç ve Yöntem: Bariatrik cerrahinin psikiyatrik uygunluğunun değerlendirilmesi amacıyla tarafımıza danışılan 44 obezite hastası ve 45 sağlıklı kontrol çalışmaya dahil edildi. Araştırmaya dahil edilen hastalara sosyodemografik veri formu, Beck Depresyon Ölçeği (BDÖ), Beck Anksiyete Ölçeği (BAÖ), Rosenberg Benlik Saygısı Ölçeği (RBSÖ), Başa Çıkma Tutumları Değerlendirme Ölçeği (COPE), Hollanda Yeme Tutumu Anketi (DEBQ), Barratt Dürtüsellik Ölçeği-11 Kısa Formu (BIS-11, Yaşam Kalitesi Ölçeği Kısa Formu (SF-36), Golombok-Rust Cinsel Doyum Ölçeği (GRCDÖ) ve Arizona Cinsel Yaşantılar Ölçeği (ACYÖ) uygulandı. Cerrahi sonrası izlemde, obezite hastalarına BDÖ, BAÖ, RBSÖ, DEBQ, SF-36, GRCDÖ ve ACYÖ uygulandı. Bulgular: BDÖ, BAÖ, RBSÖ, DEBQ, BIS-11, GRCDÖ ve ACYÖ puanları hasta grubunda istatistiksel olarak anlamlı düzeyde yüksek bulundu. COPE sorun odaklı başa çıkma tutumu alt boyut ve SF-36 puanları hasta grubunda istatistiksel olarak anlamlı düzeyde düşük bulundu. Hasta grubunun cerrahi sonrası izlemlerinde, BDÖ, BAÖ, RBSÖ, GRCDÖ, ACYÖ puanlarında istatistiksel olarak anlamlı düzeyde azalma ve SF-36 puanlarında ise istatistiksel olarak anlamlı düzeyde artış saptandı. Sonuç: Obezite ruhsal bileşenlerin olduğu multisistem bir bozukluktur. Obezitesi olan bireylerde kilo verme duygudurum, benlik saygısı, yaşam kalitesi ve cinsel yaşam gibi birbiriyle yakından ilişkili parametrelerde iyileşme sağlayarak ruhsal durumu olumlu yönde etkilemektedir. Bu nedenle, bariatrik cerrahi adayı bireylerin tedavinin hem öncesinde hem de sonrasında yapılan takiplerde ruhsal durumlarının değerlendirilmesi oldukça önemlidir.
Objective: The aim of this study is; To evaluate the pre-operative self-esteem, sexual life and other psychopathologies of obese patients who are candidates for bariatric surgery, to compare them with healthy controls, and to investigate the changes in the 1st, 3rd and 6th months after surgery. Methods: 44 obese patients who were consulted to evaluate the psychiatric suitability of bariatric surgery and 45 healthy controls were included in the study. Sociodemographic data form, Beck Depression Inventory (BDI), Beck Anxiety Inventory (BAI), Rosenberg Self-Esteem Scale (RSES), Coping Strategies with Stress Inventory (COPE), Dutch Eating Behavior Questionnaire (DEBQ), Short Form of Barratt Impulsiveness (BIS-11), Short Form of Quality of Life Survey (SF-36), Golombok Rust Inventory of Sexual Satisfaction (GRISS), Arizona Sexual Experience Scale (ASES) were applied to the patients included in the study. BDI, BAI, RSES, DEBQ, SF-36, GRISS and ASES were applied to obese patients during postoperative follow-up. Results: BDI, BAI, RSES, DEBQ, BIS-11, GRISS and ASES scores were found to be significantly higher in the patient group. COPE problem-focused coping subgroup and SF-36 scores were found to be significantly lower in the patient group. In the postoperative follow-up of the patient group, a statistically significant decrease was found in the BDI, BAI, RSES, GRISS and ASES scores. In the postoperative follow-up of the patient group, a statistically significant decrease in BDE, BAI, RSES, GRISS, ASES scores and a significant increase in SF-36 scores were observed. Conclusion: Obesity is a multisystem disorder with mental components. Weight loss in obese individuals positively affects the mental state by improving closely related parameters such as mood, self-esteem, quality of life and sexual life. Therefore, it is very important to evaluate the mental status of individuals who are candidates for bariatric surgery both before and after treatment.
