Özet
Bu klinik çalışmamızın amacı, cinsiyetin ve menstrual siklusun ortodontik tedavi başlangıcında algılanan ağrı düzeylerini ölçmek ve hayat kalitesi üzerindeki etkisini karşılaştırmalı olarak araştırmaktır. Çalışmamıza her grupta 30 kişi olmak üzere 60 birey dahil edilmiştir. Hastaların yaşları 16-22 arasında bulunmaktadır. Sabit ortodontik tedavi kararı verilen hastalara üç farklı durumda üç farklı periyotta VAS, VRS-4 ve OHIP-14 anketleri yapılmıştır. Anket sonuçları erkekler ve menstrual döneme göre ikiye ayrılmış kız grupları (foliküler ve luteal) olacak şekilde 3 grup açısından karşılaştırılmıştır. Hastalardan veri toplama zamanları, tedavi öncesi (T0), tedaviye başlandıktan 4 saat sonra (T1) ve 24 saat sonra (T2) olarak belirlenmiştir. Çiğneme, ön dişlerle ısırma ve arka dişlerle ısırma sırasında hissettikleri ağrı düzeylerini ölçmek için VAS ve VRS-4 anketlerini, hayat kalitesini değerlendirmek amacıyla da OHIP-14 anketini doldurmaları istenmiştir. Üç zamanda üç grup arasında (Erkek, Kız Foliküler, Kız Luteal) OHIP, VAS, VRS-4 değerlerinin karşılaştırılmasında Kruskal Wallis H testi kullanılmıştır. İki grup arasında (Kız Foliküler, Kız Luteal) OHIP, VAS, VRS-4 değerlerinin karşılaştırılmasında Mann Whitney U testi kullanılmıştır. OHIP sorularının tek tek karşılaştırılmasında Ki-kare testi kullanılmıştır. Tedaviden 4 saat sonra (T1) çiğneme sırasında hissedilen ağrı algısı bakımından erkek grubu ile luteal faz kız grubu arasında istatistiksel olarak anlamlı bir fark bulunmuştur ve luteal faz kız grubunda hissedilen ağrı algısı daha fazladır. T1 döneminde arka dişlerle ısırma yapıldığında hissedilen ağrı algısı bakımından erkek grubu ile hem foliküler hem de luteal kız grupları arasında anlamlı olarak bir fark bulunmuştur ve arka dişlerle ısırmada hissedilen ağrı algısı erkeklerde daha düşük olduğu görülmüştür. Tedaviden 24 saat sonra (T2) alınan VAS ölçümleri sonucuna göre üç grup arasında çiğneme sırasında, ön dişlerle ısırma yapıldığında ve arka dişlerle ısırma yapıldığında hissedilen ağrı algısı bakımından istatistiksel olarak anlamlı bir fark bulunmamıştır. Çalışmamızın sonucunda üç farklı periyotta üç grupta da hayat kalitesi açısından gruplar arası istatistiksel olarak anlamlı bir fark bulunmamıştır. Tedavi başlagıcından sonraki 24. saatte ortodontik ağrı en üst seviyeye ulaşmaktadır. Ortodontik tedavi başlagıcında hayat kalitesi düşmektedir. Luteal fazda, foliküler faz ve erkeklere göre hissedilen ağrı miktarı daha yüksektir.
The aim of this clinical study is to measure the perceived pain levels of gender and menstrual cycle at the beginning of orthodontic treatment and to investigate the effects on quality of life comparatively. In our study, 60 individuals, 30 people in each group, were included. The ages of the patients are between 16-22 years. VAS, VRS-4 and OHIP-14 questionnaires were conducted in three different periods in three situations for patients who were given a fixed orthodontic treatment decision. The survey results were compared in terms of 3 groups: boys and girls (follicular and luteal) groups divided into two according to the menstrual period. Data collection times from patients were determined as before treatment (T0), 4 hours after the start of treatment (T1), and 24 hours after (T2). They were asked to fill in the VAS and VRS-4 questionnaires to measure the pain levels they felt while chewing, biting with the anterior teeth and biting with the posterior teeth, and the OHIP-14 questionnaire to evaluate their quality of life. The Kruskal Wallis H test was used to compare the OHIP, VAS, and VRS-4 values between three groups (Male, Female Follicular, Female Luteal) at three times. Mann Whitney U test was used to compare OHIP, VAS, VRS-4 values between two groups (Girl Follicular, Girl Luteal). Ki-Kare test was used to compare OHIP questions one by one. A statistically significant difference was found between the male group and the luteal phase of female group in terms of pain perception during chewing 4 hours after the treatment (T1), and the perceived pain perception was higher in the luteal phase group. A significant difference was found between the male group and both follicular and luteal female groups in the perception of pain when biting with the posterior teeth in the T1 period, and the perception of pain felt when biting with the posterior teeth was found to be lower in males. According to the results of VAS measurements taken 24 hours after the treatment (T2), there was no statistically significant difference between the three groups in terms of pain perception during chewing, when biting with the anterior teeth and when biting with the posterior teeth. As a result of our study, there was no statistically significant difference between the groups in terms of quality of life in three different periods. Orthodontic pain peaks at the 24th hour after the start of treatment. At the beginning of orthodontic treatment, the quality of life decreases. The amount of pain felt in the luteal phase is higher than in the follicular phase and men.
