dc.description.abstractBilimsel çalışmalar, sinus caroticus'ta ateroskleroz gelişiminin, çocuklukta başladığını ve ergenlikte hızla ilerlediğini, yaşla birlikte arttığını belirtmiştir. Bifurcatio carotidis, ateromatöz plakların sık görüldüğü ve tromboembolik inmeye yol açabilen bir alandır. Cerrahi müdahaleler ve stentleme, inme riskini azaltırken, bu işlemler esnasında sinus caroticus'un uyarılması, hemodinamik değişikliklere yol açabilmektedir. Carotis endarterektomi esnasında, cerrahlar sinus caroticus'un uyarılmasını ve komplikasyonlarını önlemek için proksimal a. carotis interna'yı diseke ederler. Ancak, sinus caroticus'un arteria (a.) carotis communis ya da a. carotis externa'da değişken bir konumda bulunması yanlışlıkla uyarılma ihtimalini artırabilir. Sinus caroticus'un anatomik konumu, bireyler arasında hatta bireyin her iki tarafında farklılık gösterebilir. Bu nedenle, anatomik varyasyonlarının ve sinus caroticus'u içeren karotis arterlerin morfometrik farklılıklarının bilinmesi önemlidir. Çalışmamızda, sinus caroticus'un konumsal varyasyonlarını belirlemeyi, sinus caroticus ve bağlantılı karotis arterlerin morfometrik özelliklerini değerlendirmeyi, elde edilen ölçümleri, cinsiyet, taraf ve yaş kriterlerine göre karşılaştırmayı amaçladık. 55'i erkek, 45'i kadın olmak üzere toplamda 100 bireyin (200 taraf), bilgisayarlı tomografi anjiyografi (BTA), görüntüleri üzerinden ölçümleri yapılıp, varyasyonlar belirlendi. Erkeklerde, karotis arter çapları, sinus caroticus uzunluğu ve sinus caroticus'un en geniş yerindeki çapı, kadınlara kıyasla anlamlı seviyede (p<0,05) büyük olarak bulundu. Sol taraftaki a. carotis communis ve a. carotis interna çaplarının, sağ tarafa göre anlamlı seviyede (p<0,05) daha büyük olduğu belirlendi. Yaş artışıyla beraber, sinus caroticus uzunluğunda anlamlı (p<0,05) ve pozitif (r=0,554) bir ilişki görüldü. Sinus caroticus'un, en sık a. carotis interna'nın proksimalinde bulunduğu (%74,5) tespit edildi. Ayrıca bireylerin %29'unda, sinus caroticus, asimetrik olarak konumlanmıştı. Sinus caroticus, bazı bireylerde uyaranlara karşı aşırı duyarlıdır. Cerrahi müdahaleler esnasında da istenmeden uyarılıp, komplikasyonlara yol açabilir. Bu nedenle anatomik varyasyonlarının bilinmesi önem arz eder. Bu çalışma, karotis arterlerin ve sinus caroticus'un morfolojik ve varyasyonel verilerini bir arada sunan, literatürdeki detaylı araştırmalardan biridir. Sinus caroticus'un konumu için tiplendirmeler yapılmış ve bu bilgilerin anatomistler ile cerrahlara, teorik ve pratik genel bir bakış sağlayarak, kolaylık getireceği öngörülmüştür.
dc.description.abstractScientific studies show that the carotid sinus develops in childhood, with atherosclerosis progressing during adolescence and with age. The carotid bifurcation is prone to plaque formation, potentially leading to stroke. While surgery reduces stroke risk, carotid sinus stimulation during procedures can cause hemodynamic changes. The variable location of the carotid sinus increases the risk of unintentional stimulation, highlighting the importance of understanding its anatomical variations. The aim of our study was to determine the positional variations of the carotid sinus, to evaluate the morphometric characteristics of the carotid sinus and the associated carotid arteries, and to compare the measurements obtained according to sex, side, and age. In a total of 100 subjects (55 males and 45 females), measurements were made on computed tomography angiography (CTA) images (200 sides) and variations were determined. Carotid artery diameter, carotid sinus length, and diameter at the widest point of the carotid sinus were significantly (p<0.05) larger in men than in women. The diameters of a. carotis communis and a. carotis interna on the left side were significantly (p<0.05) larger than those on the right side. There was a significant (p<0.05) and positive (r=0.554) correlation in carotid sinus length with increasing age. The carotid sinus was most frequently located proximal to the internal carotid artery (74.5%). In addition, the carotid sinus was asymmetrically located in 29% of individuals. The carotid sinus is hypersensitive to stimuli in some individuals. It may be inadvertently stimulated during surgical procedures, leading to complications. Therefore, it is important to know its anatomical variations. This study is one of the detailed studies in the literature that presents morphologic and variational data of the carotid arteries and the carotid sinus together. The location of the carotid sinus has been typologized, and it is envisaged that this information will facilitate anatomists and surgeons by providing a theoretical and practical overview.
