Primer Cerrahi Öncesi Erken Maksiller Ortopedi ile Tedavi Edilmiş Tek Tara?ı Dudak ve Damak Yarıklı Adolesanlarda Orta Yüz Eksikliğinin Değerlendirilmesi

Özet
Bu çalışmanın amacı, tek taraflı tam dudak ve damak yarıklı, primer cerrahi öncesi erken maksiller ortopedi ile tedavi edilmiş adolesan hastaların orta yüz eksikliğini ve iskeletsel özelliklerini değerlendirmek ve dudak damak yarığı olmayan hastalarla karşılaştırmaktır.Gereç ve Yöntemler: Bu çalışma iki gruptan (n67, 45 erkek, 22 kız) oluşmaktadır. Çalışma grubu tek taraflı tam dudak ve damak yarıklı 34 hastadan (23 erkek ve 11 kız, ortalama yaş 14,1 yıl) oluş- maktadır. Kontrol grubu dudak damak yarığı olmayan 33 hastadan (22 erkek ve 11 kız, ortalama yaş 14,3 yıl) oluşmakta olup, iskeletsel Sınıf I ilişkiye ve normal büyüme modeline sahiptir. Gruplar yaş ve cinsiyete göre eşleştirilmiştir. Orta yüz eksikliği ve iskeletsel özellikler sefalogramlar ile değerlendirilmiştir. Otuz altı farklı sefalometrik parametre ölçülmüştür. İstatistiksel analiz için, "Mann-Whitney U" testi p0,05 seviyesinde kullanılmıştır. Bulgular: Çalışma grubunda maksillanın sagittal yön büyümesi (SNA açısı, A-NFH, A-VRL uzaklığı, efektif orta yüz boyutu), alt posterior yüz yüksekliği, posterior yüz yüksekliği, ramus yüksekliği, mandibuler uzunluk ve SNB açısı kontrol grubuna göre daha az bulunmuştur (p0,05). Çalışma grubu kontrol grubuna göre önemli ölçüde vertikal büyüme modeli sergilemiştir. Çalışma grubunda zigomatik kemik anlamlı derecede retrüziv bulunmuştur (p0,05). Vertikal orta yüz ölçümlerinde çalışma ve kontrol grubu arasında anlamlı bir fark saptanmamıştır. Sonuç: Tek taraflı tam dudak ve damak yarıklı hastalar kontrol grubuna göre sagittal yönde orta yüz ve maksiller yetersizlik göstermiş ve malar destek belirgin olarak azalmıştır.
: The aim of this study was to evaluate midface deficiency and skeletal characteristics of unilateral cleft lip and palate adolescent patients treated with early maxillary orthopedic before primary surgery and compare with that of noncleft patients. Material and Methods: There were two groups (n67, 45 males and 22 females) in this study. The study group consisted of 34 patients with unilateral complete cleft lip and palate (23 males and 11 females, mean age of 14.1 years) and control group consisted of 33 noncleft patients (22 males and 11 females, mean age of 14.3 years) who had skeletal Class I relationship and normal growth pattern. The groups were matched on age and sex. The midface deficiency and skeletal characteristics were evaluated on cephalograms. Thirty-six different cephalometric parameters were measured. For statistical analysis, Mann-Whitney U test were used at p0.05 level. Results: In the study group, sagittal growth of the maxilla (SNA angle, A to NFH, A-VRL distance, effective midface size), lower posterior face height, posterior face height, ramus height, mandibular length and SNB angle were less than in the control group (p0.05). The study group exhibited significant vertical growth pattern compared to control group. Zygomatic bone was retrusive significantly in the study group (p0.05). Vertical midface measurements did not differ significantly between the study and control groups. Conclusion: The unilateral complete cleft lip and palate patients presented sagittal midface and maxillary deficiency in comparison to the control group and malar support decreased markedly
