Özet
Adrenal insidentalomaların görülme sıklığı gün geçtikçe artmaktadır. Rastlantı ile adrenal kitle saptanan bir hastada temel yaklaşım kitlenin benign/malign ayrımının yapılması ve hormonal durumunun değerlendirilmesidir. Hasta veri tabanı oluşturmak amacıyla Aralık 2018 ve Şubat 2019 tarihleri arasında herhangi bir nedenden dolayı çekilip raporunda 'adrenal adenom, adrenal lezyon, adrenal kitle' ibaresi bulunan 66 hastanın kayıtları incelenmiştir. Sonuç olarak, adrenal insidentaloma kadınlarda daha sık rastlanan bir orta-ileri yaş grubu hastalığıdır. En sık görülen adrenal insidentaloma tipi fonksiyon göstermeyen adrenal adenom olup en sık görülen hormonal bozukluk hiperkortizolemidir. Ek hastalık olarak hastaların 33'ünde Hipertansiyon, 24'ünde Diyabetes Mellitus ,15'inde Hiperlipidemi olmak üzere sıklık sırasına göre ilk 3 grubu oluşturmaktadır. Bu veriler literatürdeki önceki çalışmalara ait bilgileri doğruladı ve neden-sonuç ilişkisi her ne yönde olursa olsun adrenal adenomlu bireylerde daha yüksek oranda metabolik bozukluk saptandığını gösterdi. Metabolik bozukluklar toplum ortalamalarının üstündedir ve fonksiyon göstermeyen adenomlarda da görülebilir. Literatürle benzer şekilde çalışmamızdaki hastaların 11'i normal kilolu(%16.67) , 26'sı kilolu(%39.39), 29'u obez(%43.94) idi. Yani çalışmamızdaki hastaların %83.33'ü kilolu yada obez idi. Çalışmamızda sürrenal adenomu olan kadınlarda beden kitle indexi erkeklere göre bariz daha yüksek saptanmıştır.Sürrenal adenom ile tiroid otoimmunitesi tiroid ultrasonografisi ve tiroid otoantikorları ile değerlendirilmiştir.Yapılan tiroid ultrasonografi bulguları ile hormonal aktif olan sürrenal adenomlu hastalarda daha fazla sıklıkta tiroid nodülüne rastlanmıştır.Tiroid otoimmun hastalıkları ile sürrenal adenom arasında herhangi ilişki saptanmamıştır.
The prevalence of adrenal insidentaloma is increasing. The basic approach in a patient with adrenal mass detected by coincidence is to distinguish whether the mass is benign or malignant, and assessment of hormonal state. Records of 66 patients whose screening tests were made for any reason between December 2018 and February 2019 and 'adrenal adenoma, adrenal lesion, adrenal mass' phrases are available in their reports were examined in order to create the patient database. As a result, adrenal incidentaloma is a disease which is more common in middle-advanced age group in woman. The most common type of adrenal incidentaloma is nonfunctioning adrenal adenoma, and the most common hormonal imbalance is hypercortisolism. As additional diseases, 33 of the patients have Hypertension, 24 of them have Diabetes Mellitus and 15 of them have Hyperlipidemia with respect to their frequency. This data has confirmed the previous studies in related literature and showed that individuals with adrenal adenoma have higher rates of metabolic disorders regardless the direction of cause-effect relationship. Metabolic disorders are above the average of society and it can also be seen in nonfunctioning adenomas. Rather like the literature, 11 patients were normal weight (16.67%), 26 of them were overweight (39.39%), and 29 were obese (43.94%) in this study. In other words, total 83.33% of the patients were overweight or obese. In our study, body mass index of women with adrenal adenoma was significantly higher compared to men. Adrenal adenoma and thyroid autoimmunity were assessed with thyroid ultrasonography and thyroid autoantibodies. Thyroid nodules were found more frequently in patients with hormonal active adrenal adenoma with respect to findings of thyroid ultrasonography. No relation was found between thyroid autoimmune diseases and adrenal adenoma.
