Maksiller sinus hacminin, maksiller dişsiz hastalar ve dişli hastalar arasında karşılaştırmalı değerlendirmesi

Özet
Amaç: Bu çalışmada, maksiller dişli ve dişsiz hastalarda Konik Işınlı Bilgisayarlı Tomografi (KIBT) görüntülerinde maksiller sinüslerin (MS) pnömatizasyonunun değerlendirilmesi ve temel anatomik özelliklerinin ortaya konmasıyla elde edilecek verilerin tedavi planlamasına rehberlik etmesi ve cerrahi komplikasyonların önlenmesi amaçlanmıştır. Gereç ve Yöntemler: Çalışma, Marmara Üniversitesi Diş Hekimliği Fakültesi Klinik Araştırmalar Etik Kurulundan onay almıştır (no:2018/263). Çalışma grubu, Ağız, Diş ve Çene Radyolojisi Anabilim Dalı arşivinin retrospektif olarak incelenmesi sonucu elde edilen 70 adet hastanın Konik Işınlı Bilgisayarlı Tomografi (KIBT) görüntüsünden oluşmuştur. KIBT go?ru?ntu?leri, DICOM (Digital Imaging and Communications in Medicine) formatında kaydedilip, 3D Doctor (Able Software Corp, Lexington, MA, USA) yazılım programı ile MS hacimleri değerlendirilmiştir. Veriler tanımlayıcı istatistiksel analiz, Paired T-testi ve Ki-kare testi ile değerlendirilmiş (IBM SPSS Statistics 22.5, Türkiye) ve p<0.05 anlamlılık sınırı olarak kabul edilmiştir. Bulgular: 40 tam dişli hastanın ortalama MS hacimleri 31.36 ± 11.48 cm3 olarak belirlendi. 30 maksiller total dişsiz hastanın ortalama MS hacimleri 81.11 ± 63.61 cm3 olarak belirlendi ve her iki grup arasında ileri düzeyde istatiksel anlamlı fark saptandı (p<0.0001). MS’te septumu olan hastaların 8’nin (% 34.78) maksiller posterior dişli hastalar olduğu, 15’nin (% 65.21) maksiller total dişsiz hastalar olduğu saptandı. MS’te septumu olamayan hastaların 22’sinin (% 46.80) maksiller posterior dişli hastalar olduğu, 25’nin (% 53.19) maksiller total dişsiz hastalar olduğu saptandı. Her iki grupta da MS’te septum varlığında istatiksel olarak anlamlı bir fark saptanmadı (p=0.337). Sonuç: Araştırmanın sonuçları maksiller posterior dişli ve total dişsiz bir grup hastanın değerlendirilmesiyle elde edilmiştir. Klinik değerlendirmeleri de içeren daha geniş kapsamlı araştırmaların özellikle sinüs cerrahisi operasyonlarına ışık tutabileceği görülmüştür.
Background: The aim of this study was to evaluate the pneumatization of maxillary sinuses (MS) and to determine the basic anatomic variation of the maxillary sinus between maxillary edentulous patients and dentate patients by using cone-beam computed tomography (CBCT) images to guide the treatment planning and to prevent surgical complications. Methods: The study was approved by Marmara University Faculty of Dentistry Clinical Research Ethics Committee (no:2018/263). 70 CBCT images (Planmeca Romexis®, 3D Mid Planmeca Oy, Helsinki, Finland) were chosen from the archive of Department of Dentomaxillofacial Radiology. CBCT images were saved in DICOM (Digital Imaging and Communications in Medicine) format, and the MS volumes were evaluated with the 3D Doctor software (Able Software Corp., Lexington, MA, USA). Data were evaluated by descriptive statistical analysis paired Ttest and chi-square test by (IBM SPSS Statistics 22.5, Turkey). P<0.05 was statistically significant. Results: The mean MS volume of 40 dentate patients was determined as 31.36 ± 11.48 cm3 . The mean MS volume of 30 edentulous patients was determined as 81.11 ± 63.61 cm3, and there was a statistically significant difference between two groups (p <0.0001).
