Yüksek Göz İçi Basınçlı Olgularda Erken Dönem Glokomatöz Hasarın Saptanmasında Pattern Elektroretinografi ve Kontrast Duyarlık Testlerinin Kullanılması

Özet
Amaç: Yüksek göz içi basınç YGİB'lı olgulardaki erken dönem glokomatöz sinir lifi hasarının saptanmasında pattern elektroretinografi (PERG) ve kontrast duyarlık testlerinin tanısal değerinin belirlenmesi Gereç ve yöntem: Glokom ünitemize başvuran ve göz içi basıncı yüksek (21 mmHg ve üzeri) olarak saptanan tanısı alan 40-50 yaş arası (ort. 45.44.1 yıl) 18 olgunun 34 gözüne PERG ve kontrast duyarlık (Pelli-Robson kontrast kartı) testleri uygulandı. Kontrol grubu ola-rak alınan 40-50 yaş arası (ört. 46.23,3 yıl) 14 olgunun 28 gözüne de aynı testler uygulandı. Sonuçların istatistiksel analizi Student-t testi ile yapıldı. Bulgular: YGİB'lı olguların PERG cevaplarında, N95 genlikleri kontrol grubuna göre istatistiksel olarak anlamlı oranda düşük bulundu. Kontrast duyarlık değerleri de YGİB'lı olgularda kontrol grubuna göre istatistiksel olarak anlamlı oranda düşük bulundu. Sonuç: Pattern ERG'nin N95 komponentindeki genlik azalmaları YGİB'lı olguların erken dönem sinir lifi hasarının tespitinde yol göstermektedir. Benzer şekilde kontrast duyarlık değerlerindeki azalma da hücresel hasarın lehine değerlendirilebilecek bulgulardır. Zira glokomda hem büyük çapta ganglion hücreleri hem de merkezi duyarlıkta azalmalar olmaktadır.
Purpose: To determine the diagnostic importance of contrast sensitivity and pattern elect-roretinography (PERG) tests in detecting early glaucomatous damage in nerve fibers of patients with high intraocular pressure. Methods: PERG and contrast sensitivity (Pelli - Robson contrast sensitivity Chart) tests used in 34 eyes of 18 subjects who were referred to our glaucoma unit and diagnosed as having primary open angle glaucoma. Patient's age range from 40 to 50 years (mean 45.4±4.1 years).The same tests applied to control group consisting of 28 eyes of 14 subjects and aged 40-50 years (mean 46.2±3.3 years). The statistical analysis of the results were done with the Student -t test. Results: When compared with the control group the amplitudes of N95 are found signifi-cantly low in the high intraocular pressure group. Contrast sensitivity results are also found to be significantly low. Conclusion: Glaucoma can effect larger ganglion cells as well as central sensitivity. The decrease of amplitude in the N95 component of PERG showed the early damage in the nerve fi-ber layers of the retina. In conjunction, lower values of contrast sensitivity can be attributable to cellular damage.
