Deneysel siyatik sinir hasarı modelinde aloperine'nin anti-enflamatuvar ve anti-oksidan özelliklerinin iyileşme üzerine etkisinin değerlendirilmesi

Özet
Amaç: Periferik sinirler mekanik, iskemik, kimyasal nedenlere bağlı olarak yaralanmaktadır. Periferik sinir yaralanması sonrası kişide fonksiyonel bozukluklara yol açar ve yaşam kalitesini düşürür. Sinir kopma nörometzis tipi yaralanmalar da cerrahi onarım gerekir. Onarım sonrası rejenerasyonu sürecinde inflamasyon ve oksidatif stres olumsuz etki eder. Biz de buradan yola çıkarak antioksidan ve antiinflamatuvar etkinliği gösterilmiş aloperinin periferik sinir rejenerasyonu üzerine olası olumlu etkisini inceledik. İnceleme için fonksiyonel biyokimyasal ve histolojik değerlendirmeler kullandık. Gereç ve Yöntem: Çalışmamızda 30 adet Wistar- albino cinsi erkek sıçan kullanıldı. 3 grubu ayrıldı ilk grubun siyatik sinirlerinde nörometzis tipi yaralanma oluşturuldu ardından uç uca epinöral koaptasyon yapıldı. İkinci grupta aynı şekilde nörometzis tipi yaralanma oluşturuldu uç uca epinöral koaptasyon yapıldı ve tek doz intraperitoneal 100mg/kg dozuyla Aloperin uygulandı. Üçüncü gruba cerrahi işlem yapılmadı. Sıçanlar altı hafta takip edildi. Altı haftanın sonunda fonksiyonel değerlendirme için itme kuvvet testi, pinprick testi ve sıcak zeminden ayak çekme testi uygulandı takiben sakrifikasyon yapıldı. İntrakardiyak kan alındı kan örneklerinden IL-6, IL-10, TNF- α, TAS ve TOS değerlerinin ölçümleri yapıldı. Siyatik sinirleri çıkarıldı ve histolojik incelmesi yapıldı, akson sayısı çapı, wallerian dejenerasyon değerlendirildi. Siyatik sinir dokusunda IL-10 ve TNF-α ölçümleri yapıldı. Her iki taraf gastroknemius kaslar çıkarıldı. Gastroknemius kas ağırlık ölçümleri yapılarak kas ağırlık oranı hesaplandı. Bulgular: Aloperin uygulanan grupta fonksiyonel olarak kas hacmi oranı ve pinprick skoru sadece onarım yapılan gruptan daha yüksek bulundu. Sıcak zeminden ayak çekme süresi daha kısa bulundu. İtme kuvvet oranı yapılan formülasyona göre düşük çıktıkça daha fazla iyileşme olduğunu göstermekte Aloperin uygulanan grupta itme kuvvet oranı daha düşük olarak bulundu. Aloperin uygulanan grubun kanında sinir dokusunda IL-10 yüksek TNF-α düşük olarak saptandı. Yine bu grubunun kanında IL-6 miktarı daha düşük olarak ölçüldü. Histolojik incelemede aloperin uygulana grupta akson sayısı ve çapının arttığı wallerian dejenerasyonun azaldığı görüldü. Sonuç Bu çalışmada elde edilen veriler ışığında nörometzis tipi sinir yaralanmasında, sistemik uygulana Aloperinin; antioksidan ve antiinflamatuar etkiyle sinir rejenerasyonunu olumlu etkilediğini, rejenerasyonu hızlandırdığını ve fonksiyonel iyileşmeyi de yükselttiğini literatürde ilk defa gösterdik. Bu çalışmada Aloperinin periferik sinir üzerindeki etkisini araştıran ilk çalışma olması nedeniyle gelecekte yapılacak çalışmalar için fikir verici olacağı düşüncesindeyiz.
Aim: Peripheral nerves are injured due to mechanical, ischemic and chemical causes. After peripheral nerve injury, it causes functional disorders and reduces the quality of life. Nerve rupture neurometsis type injuries also require surgical repair. Inflammation and oxidative stress affect the regeneration process after repair negatively, so we examined the possible positive effect of aloperine, which has shown antioxidant and anti-inflammatory activity, on peripheral nerve regeneration. We used functional biochemical and histological evaluations for the review. Material and Method: In our study, 30 male Wistar-albino rats were used. Three groups were separated, neurometsis type injury was created in the sciatic nerves of the first group, and then end-to-end epineural coaptation was performed. In the second group, neurometsis type injury was created in the same way, end-to-end epineural coaptation was performed and a single intraperitoneal dose of 100mg/kg Aloperin was administered. No surgical procedure was performed in the third group. The rats were followed for six weeks. At the end of six weeks, push-pull test, pinprick test, and foot pull test were applied for functional evaluation, followed by sacrification. IL-6, IL-10, TNF-α, TAS and TOS values were measured from blood samples from intracardiac blood samples. Sciatic nerves were removed and histologically examined, axon number diameter, wallerian degeneration were evaluated. IL-10 and TNF-α measurements were made in the sciatic nerve tissue. Both sides gastrocnemius muscles were removed. Muscle weight ratio was calculated by measuring gastrocnemius muscle weight Result: Functionally, muscle volume ratio and pinprick score were found to be higher in the aloperine administered group than in the repair-only group. The time to take foot from the warm floor was found to be shorter. It shows that there is more improvement as the thrust force ratio is lower than the formulation applied, while the thrust force ratio was found to be lower in the Aloperin applied group. In the blood of the aloperine administered group, IL-10 was high and TNF-α was low in the nerve tissue. Again, the amount of IL-6 in the blood of this group was measured to be lower. In histological examination, it was observed that the number and diameter of axons increased and the wallerian degeneration decreased in the group treated with alloperine. Conclusion: As a result of the data obtained in this study, systemic application of Aloperin in neurometsis type nerve injury; We have shown for the first time in the literature that it positively affects nerve regeneration, accelerates regeneration and increases functional recovery with its antioxidant and anti-inflammatory effects. Since this study is the first to investigate the effect of Aloperin on the peripheral nerve, we think that it will be informative for future studies.
