Özet
Bu tez çalışmasında, iki yapraklı mekanik kalp kapak ses sinyallerinin spektral özelliklerinin incelenmesi ve bu sinyallerden mekanik kalp kapak hastalıklarını tanımlamaya yönelik spektral özelliklerinin belirlenmesi amaçlanmıştır. Aort ve/veya mitral kalp kapakları iki yapraklı mekanik kalp kapakları ile değiştirilen hastaların kalp ses ve EKG sinyalleri kayıt edilerek bir veri tabanı oluşturulmuştur. Normal mekanik kalp kapak ses sinyallerinin güç spektrum yoğunluklarından, mekanik kalp kapak ses sinyallerinin frekans bileşenleri ve enerjileri, bu frekans bileşenlerinin ve enerjilerinin kalp ses bileşenine, kayıt odağına ve ameliyat tipine bağlı olarak nasıl değiştiği incelenmiştir. Mekanik kalp kapaklarının güç spektrum yoğunlukları, kısa zaman Fourier, sürekli dalgacık ve ayrık dalgacık dönüşümlerinden kalp seslerini tanımlayıcı özellikler elde edilmiştir. Daha sonra mekanik kalp kapağında tromboz oluşmuş bir, mekanik kalp kapağı yanında kaçak oluşmuş iki ve normal mekanik kalp kapak hastalarından elde edilen bu özellikler istatistiksel karşılaştırılmıştır. İlk karşılaştırma komplikasyonlu durumda elde edilen özellikler ile aynı hastaların komplikasyonu tedavi edildikten sonra elde edilen özellikler arasında yapılmıştır. İkinci karşılaştırma komplikasyonlu ve normal mekanik kalp kapak ses sinyallerinden elde edilen özellikler arasında gerçekleştirilmiştir. Sonuç olarak, mekanik kalp kapak trombozunun ve kapak dikişlerinde meydana gelen kaçağın teşhisine yardımcı olacak özellikler tespit edilmiştir.
In this thesis study, examination of the spectral characteristics of bileaflet mechanical heart valve sound signals and determination of spectral properties to recognize the mechanical heart valve diseases using heart sounds signal are aimed. A database was created by recording heart sounds and ECG signal of patients whose aortic and/or mitral heart valves were replaced with bileaflet mechanical valve. The frequency components and energies of mechanical heart valve sound signals were examined using power spectrum densities of normal mechanical heart valve sound signals. Also how these frequency components and energies changes were observed depending on heart sound components, recording area and the type of surgery, too. Descriptive features characterizing mechanical heart valve sounds were obtained from power spectrum densities, short time Fourier, continuous wavelet and discrete wavelet transform of mechanical heart sounds signal. Then, these features obtained from one patient having thrombosis on mechanical valve and two patients with paravalvular leakage and patients with normal mechanical heart valve were statistically compared. First comparison was fulfilled between the data obtained from patients with complications and same patients after treatment. The latter was fulfilled between the data obtained from patients with complications and with normal mechanical heart valve. As a result, features helping diagnosing these complications were found.
