Özet
Amaç: İlk kez kontakt lens kullanmaya başlayan olgularda, oksijen iletkenlikleri farklı olan iki yumuşak kontakt lensin kornea kalınlığı üzerine etkisinin araştırılması amaçlandı. Gereç ve Yöntem: Çalışmada 28 hastanın 56 gözü prospektif olarak incelendi. Kornea kalınlık ölçümleri Pentacam (Oculus Inc) cihazı ile yapıldı. Grup I’de Soflens Comfort (Bausch & Lomb), Grup II’de ise O2Optix (Ciba Vision) kontakt lensleri kullanıldı. Olguların kornea kalınlıkları kontakt lens kullanmaya başlamadan önce, kontakt lens kullanımının 15. , 30. ve 90. gününde 9 farklı korneal bölgede ölçüldü. Bulgular: 15. gün alınan tüm ölçümlerde kornea kalınlık artışı saptandı ancak gruplarda sonuçlar istatistiksel olarak anlamlı bulunmadı. Her iki grup toplamında ise 15. gün ölçümlerinde santral, süperior, inferior nasal, inferior ve süperior temporalde istatistiksel olarak anlamlı kalınlık artışı saptandı. 30. gün ve 90. gün ölçümlerine ise başlangıç değerine göre istatistiksel olarak anlamlı olmayan kalınlık artışı saptandı.Her iki kontakt lens kullanımı ile ilk haftalarda korneal kalınlıkta artma, birinci aya doğru kornea kalınlığında azalma saptandı. Üçüncü aya doğru ise kornea kalınlığında istatistiksel olarak anlamlı olmayan artış tespit edildi. Kornea kalınlık değişimleri gruplar arasında karşılaştırıldığında istatistiksel olarak anlamlı sonuçlar bulunmadı. Sonuç: Kullanılan kontakt lenslerin Dk/t değerleri 15.7 ve 138’dir. Bu değerler günlük kullanım modalitesinde korneal oksijenasyonu etkileyecek kritik Dk/t değerinin üzerindedir. Çalışmadaki lenslerin kornea kalınlığında anlamlı artış yapmamasını bu değerler açıklamaktadır. Bu nedenle günlük kullanım modalitesinde korneal oksijenasyonu etkileyecek kritik Dk/t değerinin üzerindeki kontakt lenslerin kornea kalınlığına etkileri arasında anlamlı fark olmadığı söylenebilir.
Aim: The investigation of the effects of two soft contact lenses with different oxygen transmissions on corneal thickness in patients who were wearing contact lens for the first time was aimed. Materials and Methods: In the study, 56 eyes of 28 patients are investigated prospectively. Corneal thickness was measured using Pentacam® (Oculus Inc). Soflens Comfort ® (Bausch & Lomb) contact lenses were used in Group I and O2Optix® (Ciba Vision) contact lenses were used in Group II. Thickness measurements were performed in nine regions of cornea at the baseline examination and 15th, 30th and 90th days of contact lens wear. Results: An increase in the corneal thickness was noted at the 15th day compared to the baseline measurements in both study groups, though it was not statistically significant. However the statistical analysis of the measurements at the 15th day in both groups together showed a significant increase in corneal thickness in the superior, inferior nasal, inferior and superior temporal corneal regions. Measurements performed at the 30th and 90th days were also higher than the baseline measurements but the statistical analysis revealed no significance. An increase in the corneal thickness was determined in the first weeks which decrased at the 30th day in both contact lens groups. The 90th day measurements showed an increase again, however, it was not statistically significant. When corneal thickness changes in both groups were compared with each other, the analyses were not statistically significant. Conclusion: The Dk/t values of the contact lenses used in the study were 15.7 and 138. These values are over the critical Dk/t value that might effect the corneal oxygenisation in daily wear modality. These values explain the insignificant increase of corneal thickness after contact lens wear in the study. These results denoted that contact lenses with a Dk/t higher than the critical level did not cause a significant difference on corneal thickness in daily wear.
