Abstract
Endometrium kanseri kadın genital sisteminin en sık görülen malign tümörüdür. Son yıllarda endometrium kanserinde mortaliteden ziyade ortaya çıkan yeni vaka sayısındaki artış durumu önem kazanmıştır. Bu nedenle prognozda etkili klinikopatolojik faktörler, mortalite riskinin ve hedefe yönelik gelişmiş tedavi stratejilerinin belirlenmesi amacıyla çalışmalar yapılmaktadır. Bu amaçla çalışmamızda güncel literatürde farklı solid tümörlerde varlığı gösterilen GOLPH3'ün endometrium kanserinde prognoza etkisi araştırılmıştır. GOLPH3, aynı zamanda GPP34/GMx33/MIDAS olarak bilinen, veziküler taşınmayı sağlayan onkoproteinlerin heyecan uyandıran yeni bir sınıfıdır. Çeşitli solid tümörlerde varlığı gösterilmiş olup yüksek ekspresyonunun kötü prognozla ilişkili olduğu bildirilmiştir. Ancak henüz literatürde GOLPH3 ekspresyonunun Endometrium Kanserindeki rolü ile ilgili yapılmış bir çalışma bulunmamaktadır. Çalışmamızda Endometrial Karsinomlarda GOLPH3 ekspresyonunun klinikopatolojik parametreler (yaş, tümör boyutu, myometrial invazyon, servikal stromal tutulum, lenfovasküler invazyon, tümörün histolojik tipi, derecesi, evresi, risk grubu, lenf nodu tutulumu ), tedaviye yanıt, prognoz ve sağkalım ile ilişkisi ve bunların PCR ile korelasyonu değerlendirilmiştir. Sonuç olarak Endometrium kanserinde GOLPH3'ün immunohistokimyasal ekspresyonu ile PCR değerlerinin birbiriyle korele olduğu saptanmış olup GOLPH3 PCR değerleri özellikle klinikopatolojik prognostik faktörlerden uzak metastaz ve tedaviye yanıtla ilişkili bulunmuştur. Diğer parametreler ve sağ kalım ile istatistiksel olarak anlamlı farklılık içermediği görülmüştür. Özellikle histolojik derecelendirme ve/veya malign ve benign endometrial lezyonlar ayırımında bir anlamı olmadığı saptanmıştır. Bu bilgiler ışığında, endometrium kanserinde, GOLPH3 ekspresyonunun klinik prognostik bir faktör olarak tedaviye yanıtı ve metastaz potansiyelini öngörmede, hedefe yönelik tedavide tedavi protokolünü belirleyebilecek bir onkoprotein olarak kullanılabileceği sonucuna varılmıştır. Ancak daha geniş hasta gruplarında daha çok çalışma yapılarak değerlendirilmelidir.
Endometrial carcinoma is the most common invasive cancer of the female genital tract.. In recent years the increasing number of new cases has gained importance rather than mortality. Therefore studies are conducted to determine clinicopathologic factors which are effective in prognosis, mortality risk and targeted treatment. For this purpose we investigated the presence and prognostic value of GOLPH3 in endometrial carcinoma, that is associated many different solid tumors. GOLPH3, also known as GPP34/GMx33/MIDAS represents an exciting new class of oncoproteins involved in vesicular trafficking. Overexpression is associated with clinical progression in many solid tumors were identified. High expression of GOLPH 3 has been shown to be associated with poor prognosis. Although GOLPH3 has been shown to be a tumor-associated marker, the correlation between GOLPH3 expression and clinicopathological parameters and clinical outcomes in endometrium carcinoma has not been analyzed before. In the present study we investigated the expression of GOLPH3 with PCR (Polymerase chain reaction) and immunhistochemical study and ıts correlation with clinicopathological features (age, tumor size, myometrial, cervical invasion, lymphovascular invasion, grade, stage, group of risk, lymph node involvement), chemotherapy, radiotherapy response and prognosis in endometrium carcinoma. As a result ,GOLPH3 PCR value was found highly correlated with immunohistochemical expression in endometrial carcinoma. GOLPH3 PCR value was also associated with distant metastasis and response to the treatment. Other parametres and survival was not statistically significant. Especially histological grading and/or malingant/benign distinction was not found statically significant. In accordance with this information GOLPH3 over expression in endometrial carcinoma can be useful as a prognosic factor for predicting response to treatment and metastasis potential. But the results must be evaluated at large patient groups for detailed information.
