Özet
Amaç: Temporomandibular eklem disfonksiyonu (TMED); temporomandibular eklemi (TME) ve eklemle ilişkili destek yapıları etkileyen kas-iskelet sistemi rahatsızlığıdır. Bu çalışmanın amacı TMED’li bir grup hastada klinik bulguların değerlendirilmesidir. Gereç ve Yöntemler TME ile ilgili şikâyeti nedeniyle Marmara Üniversitesi Diş Hekimliği Fakültesi Ağız, Diş ve Çene Radyolojisi kliniğine başvuran 100 hastanın klinik muayenesi ‘Tanı Kriterleri/ Temporomandibular Eklem Disfonksiyonu (TK/ TMED) (International RDC- TMD Consortium) protokolü ile yapılmıştır. Hastaların demografik bilgileri, anamnez ve klinik muayene bulguları değerlendirilmiştir. Bulgular 20-64 yaş aralığındaki % 83’ü kadın, % 17’si erkek olan hastaların yaş ortalaması 35,1± 11,3’tür. En sık saptanan şikayet hastaların 88’inde ‘ağrı’, 63’ünde ‘yemek yiyememe’, 67’sinde ‘ağız açmada zorluk’, 54’ünde ‘diş sıkma’ ve 19’unda ‘eklemden ses gelmesi’ dir. Hastaların % 64’ü tekrarlayan ağrı, % 22’si sürekli ağrı varlığını belirtirken %14’ünde ağrı olmadığı saptanmıştır. Ağız açma esnasında % 23 hastada düzeltilmiş deviasyon, % 32 hastada sol tarafta sonlanan deviasyon, % 14 hastada sağ tarafta sonlanan deviasyon bulgulanmıştır. Hastaların 57’sinde miyalji, 35’inde sağ artralji, 38’inde sol artralji, 29’unda TMED’e bağlı baş ağrısı tespit edilmiştir. Sonuç TMED’in tanısında diş hekimlerine önemli sorumluluk düşmektedir. TMED riski taşıyan bireylere rutin TME muayenesi yapılmalıdır.
Background: Temporomandibular joint dysfunction (TMD); a musculoskeletal system disorder affecting the temporomandibular joint (TMJ) and joint-related structures. The aim of this study is to evaluate clinical findings in a group of patients with TMD. Methods: The clinical examination of 100 patients who referred to Marmara University Faculty of Dentistry, Department of Oral and Maxillofacial Radiology because of the complaint related to TMJ was made with Diagnostic Criteria / Temporomandibular Joint Dysfunction (DC / TMD) (International RDC-TMD Consortium) protocol. Demographic information, history and clinical examination findings of the patients were evaluated. Results: The mean age of the patients between the ages of 20-64 was 35.1 ± 11.3 years. The most common complaint was pain in 88 patients, difficulty eating in 63 patients, limited mouth opening in 67 patients, bruxism in 54 patients and joint sound in 19 patients. 64 % of the patients had recurrent pain, 22 % had permanent pain and 14 % had no pain. At the time of mouth opening, 23 % of the patients had corrected deviation, 32 % of the patients had leftended deviation, and 14% of the patients had right-ended deviation. Myalgia was observed in 57 patients, right arthralgia was found in 35 patients, left arthralgia was found in 38 patients and TMD-related headache was in 29 patients. Conclusion: Dentists have a significant responsibility for the diagnosis of TMD and routinely should examine the TMJ of individuals with TMD risk.
