| dc.description.abstract | Selçuk Üniversitesi Meram Tıp Fakültesi Ortopedi ve Travmatoloji Anabilim Dalı'nda Ekim 1992- Eylül 2006 tarihleri arasında 319 hastanın 376 dizine total diz artroplastisi uygulanmıştır. Total diz artroplastisi sonrası gelişen komplikasyonlar retrospektif olarak değerlendirildi. Hastaların 252'si kadın (%78.99), 67'si erkekti(%21.01). Hastaların yaş ortalaması 67.7 idi (35-87). Hastaların 125'inin sağ dizine, 137'sinin sol dizine, 57 hastanın ise her iki dizine de total diz protezi uygulandı. Her iki dizine de total diz protezi uygulanan hastaların 12'si aynı seansta uygulandı. Ortalama takip süresi 32.6 ay (4-92) dır. Knee Society klinik değerlendirme formuna göre ameliyat öncesi diz skoru ortalama 46.7 (35-60), ameliyat sonrası ortalama 91.3 (80-100), fonksiyon skorları ise ameliyat öncesi ortalama 39.4 (10-60), ameliyat sonrası ortalama 81.9 (40-100) olarak bulundu. Ameliyat öncesi hastaların ortalama eklem hareket açıklığı 108.3° (70-135) iken, ameliyat sonrası ortalama 113.2° (70-140) olarak bulundu. Görülen komplikasyonlar; 11 hastada (%3.4) erken enfeksiyon, 5 hastada (%1.5) yara komplikasyonu, 4 hastamızda kronik enfeksiyon (%1.2), 2 hastada (%0.6) peroneal sinir arazı, 1 hastamızda(%0.3) safen sinirin infrapatellar dalında nöroma, 5 hastada (%1.5) derin ven trombozu, 2 hastada (%0.6) ekstensör mekanizma komplikasyonu, 2 hastada (%0.6) hareket kısıtlılığı, 2 hastada (%0.6) aseptik gevşeme, 1 hastada (%0.3) periprostetik kırık, instabilite ise 1 hastamızda (%0.3) gözlendi. 3 hastamız ameliyat sonrası ex oldu. Sonuç olarak total diz artroplastisi, başarılı sonuçlarına rağmen, oluşabilecek komplikasyonlar cerrah ve hasta için üzücü problemlere neden olabilmektedir. Bu komplikasyonları en aza indirebilmek için ameliyat öncesi iyi bir hazırlık, titiz bir cerrahi uygulama ve dikkatli bir ameliyat sonrası bakım gerekmektedir. | en_US |
| dc.description.abstract | At Selcuk University Meram Faculty of Medicine, in Orthopaedics and Traumatology Department between October 1992 and Septamber 2006, 376 total knee arthroplasties were performed in 319 patients. Postoperative complications were evaluated retrospectively. There were 252 women (%78.99) and 67 man (%21.01). The averageage of the patients was 67.7 (35-87) years. Total knee prostheses were applied to 125 right knees, 137 left knees and 57 were bilateral. Bilateral aplications were done in the same sessions in 12 patients. The average follow up time was 36.2 months. According to Knee Society clinic evaluation forms average knee points was 46.7 (35-60) before the operations. The average after the operations was 91.3 (80-100). Functional points were 39.4 (10-60) preoperatively and 81.9 (40-100) postoperatively. Before the operation the range of motion (ROM) was 108.3° (70-140), it was found to be 113.2° after the operation. The complications were: 11 (%3.4) early infection, 5 (%1.5) wound problems, 4 (%1.2) chronic infections, 2 (%0.6) nerve palsies, 1 (%0.3) neuroma of the infrapatellar branch of the saphenous nerve, 5 (%1.5) deep vein thrombosis, 2 (%0.6) extensor mechanism disfunctions, 2 (%0.6) ROM restictions, 2 (%0.6) aseptic loosening, 1 (%0.3) periprosthetic fractures, 1 (%0.3) instability, 3 patients died after the operation. As a result, in spite of high success rates following the total knee arthroplasty, complications that occur can cause serious problems to, the patients and surgeon. To minimize these complications, a good preoperative preparation of the patient, meticulous surgery and a good postoperative care are needed. | en_US |
