Özet
Bu çalışma doğum sonu dönemde eş şiddetine maruz kalan annelerin maternal bağlanma ve öz şefkat düzeylerinin incelenmesi amacıyla tanımlayıcı, ilişki arayıcı ve kesitsel türde yapıldı. Çalışmanın örneklemini 20 Ekim 2023-31 Ocak 2024 tarihleri arasında Konya Selçuk Üniversitesi Tıp Fakültesi Hastanesi Çocuk Sağlığı ve Hastalıkları polikliniklerine başvuran 3-12 aylık bebeğe sahip 330 anne oluşturdu. Veriler kişisel bilgi formu, eş şiddeti değerlendirme formu, Maternal Bağlanma Ölçeği (MBÖ) ve Öz Şefkat Ölçeği Kısa Formu (ÖŞÖ-KF) kullanılarak toplandı. Gruplar arasındaki farkın anlamlılığını test etmek için Independent Simple T Testi kullanılırken ikili analizlerde anlamlı çıkan değişkenler Enter Yöntemi kullanılarak Çoklu Doğrusal Regresyon analizine dahil edildi. Çalışma öncesi etik izin ve kurum izni alındı. Çalışmaya katılan annelerin %53,3'ünün duygusal, %33'ünün sözel, %13,6'sının fiziksel, %13,3'ünün cinsel ve %11,5'inin ekonomik olarak doğum sonu dönemde eş şiddetine maruz kaldığı bulundu. Annelerin MBÖ ortalamaları 89,35±17,05 olup maternal bağlanma düzeyleri yüksek, ÖŞÖ-KF puan ortalamaları ise 39,81±9,63 olup öz-şefkatleri orta düzeyde bulundu. Fiziksel, duygusal, cinsel ve ekonomik eş şiddetinin maternal bağlanmaya etkisi istatistiksel olarak anlamlı bulundu (p<0,001) ve annelerin maternal bağlanma düzeyleri daha düşük saptandı. Fiziksel, duygusal, cinsel, ekonomik (p<0,001) ve sözel eş şiddetinin (p=0,012) öz-şefkate etkisi istatistiksel olarak anlamlı bulundu ve annelerin öz şefkat düzeyleri daha düşük saptandı. Doğum sonu dönemde kadınların eşinden maruz kaldığı fiziksel (B=10,727), duygusal (B=12,551), cinsel (B=8,819) ve ekonomik (B= 8,675) şiddetin maternal bağlanmayı etkileyen değişkenlerdi (p<0,001). Öz-şefkat düzeyini ise fiziksel şiddet (B=5,4), sözel şiddet (B=2,8), duygusal şiddet (B=6,6), cinsel şiddet (B=4,2) ve ekonomik şiddet (B=5,9) etkileyen faktörlerdi (p<0,001). Sonuç olarak şiddet, kadınların hayatlarının her döneminde maruz kalabildikleri önemli bir sorundur. Özellikle doğum sonrası dönemde şiddete maruz kalmak anne ve bebek açısından olumsuz durumlara neden olabilmektedir. Ebeler ise kadına yönelik şiddetin tespit edilmesinde ve kadınların psikososyal olarak desteklenmesinde önemli rol oynamaktadır.
This study was conducted in a descriptive, relationship-seeking and cross-sectional type in order to examine the maternal attachment and self-compassion levels of mothers who were exposed to partner violence during the postpartum period. The sample of the study was composed of 330 mothers with a 3-12 month old baby who applied to the outpatient clinics of Konya Selcuk University Faculty of Medicine Hospital Child Health and Diseases between the dates of October 20, 2023 – January 31, 2024. The data were collected using the personal information form, the partner violence assessment form, the Maternal Attachment Scale (MAI) and the Short Form of the Self-Compassion Scale (SCS-SF). Independent Simple T Test was used to test the significance of the difference between the groups, while the variables that were significant in binary analyses were included in the Multiple Linear Regression analysis using the Enter Method. Dec. Ethical permission and corporate permission were obtained before the study. It was found that 53.3% of the mothers participating in the study were exposed to partner violence during the postpartum period emotionally, 33% verbally, 13.6% physically, 13.3% sexually and 11.5% economically. The mean MAI of mothers was 89.35±17.05, maternal attachment levels were high, and the mean SCS-SF score was 39.81±9.63 and their self-compassion was found to be at a moderate level. The effect of physical, emotional, sexual and economic partner violence on maternal attachment was found to be statistically significant (p<0.001), and mothers' maternal attachment levels were found to be lower. The effect of physical, emotional, sexual, economic (p<0.001) and verbal partner violence (p=0.012) on self-compassion was found to be statistically significant, and the mothers' self-compassion levels were found to be lower. The physical (B=10.727), emotional (B=12.551), sexual (B=8.819) and economic (B= 8.675) violence that women were exposed to from their husbands during the postpartum period were variables that affected maternal attachment (p<0.001). The level of self-compassion was determined by physical violence (B=5.4), verbal violence (B=2.8), emotional violence (B=6.6), sexual violence (B=4.2) and economic violence (B=5.9) were the influencing factors (p<0.001). As a result, violence is an important problem that women can be exposed to at any time of their lives. Exposure to violence, especially in the postpartum period, can cause negative situations for the mother and the baby. Midwives, on the other hand, play an important role in detecting violence against women and supporting women psychosocially.
