Ağız, Diş ve Çene Cerrahisinde Genel Anestezi Altında Opere Edilen Hastaların Retrospektif Olarak Değerlendirilmesi

Özet
Amaç: Diş Hekimliğinde tedavilerin birçoğu lokal anestezi altında yapılabilirken, bazı ağız, diş ve çene cerrahisi uygulamalarında lokal anestezinin yeterli olmaması, girişim sürelerinin daha uzun sürmesi, oluşabilecek komplikasyonlara daha rahat ve etkin şekilde müdahale edilebilmesi gibi sebeplerden ötürü genel anesteziye ihtiyaç duyulmaktadır. Retrospektif olarak planlanan bu çalışmanın amacı, genel anestezi ile opere edilmiş ağız, diş ve çene cerrahisi hastalarının değerlendirilmesidir. Gereç ve Yöntemler: Çalışmamızda, Harran Üniversitesi Diş Hekimliği Fakültesi Hastanesinde genel anestezi ile opere edilmiş ağız, diş ve çene cerrahisi hastalarının verileri retrospektif olarak değerlendirildi. 2017 yılının Ocak ayı ile 2019 yılının Haziran ayı arasındaki yaşları 1 ile 65 arasında değişen toplam 75 hastanın verileri çalışmamızda kullanılmıştır. Bulgular: Çalışmaya dahil edilen toplam 75 hastanın yaş aralığı 1- 65 arası olup, erkek hastaların kadın hastalara olan oranı 2,12’dir. Girişim süresi 10 dakika ile 8 saat 35 dakika arasında değişmekle birlikte, toplam girişim süresi 147 saat 18 dakika olarak kaydedilmiştir. En fazla entübasyon uygulanım şekli 55 hasta ile klasik nazal entübasyon olup, bunu 6 hasta ile video laringoskop destekli nazal entübasyon ve 14 hasta ile oral entübasyon izlemiştir. Yapılan işlemler arasında en fazla olguyu 47 olgu ile kist/tümör cerrahisi, en az olguları ise 1’er olgu ile alveol yarığı ve temporomandibular ekleme yönelik tedaviler oluşturmuştur. Hastane yatış süresi olarak en fazla 35 olgu ile 1 günlük yatış süresi olup günü birlik yatış, olguların %13’ünü teşkil etmiştir. Sonuç: Diş Hekimliğinin ağız, diş ve çene cerrahisi alanında genel anestezi uygulaması giderek yaygınlaşmaktadır. Özellikle maksillofasiyal fraktürlerin tedavisi, geniş kist ve tümörlerin cerrahi işlemleri, dudak-damak yarığı ve konjenital veya edinsel çene deformitelerin tedavisi gibi komplike ve lokal anestezi altında yapılamayacak olgular için genel anestezi ciddi bir gerekliliktir.
Background: While most of the treatments in dentistry can be performed under local anesthesia, general anesthesia is needed for reasons such as insufficient local anesthesia in patients with oral and maxillofacial surgery, longer duration of intervention, and more comfortable and effective intervention in possible complications. The aim of this retrospective study was to evaluate the patients who underwent general anesthesia for oral and maxillofacial surgery. Methods: In this study, the data of oral and maxillofacial surgery patients operated under general anesthesia in Harran University Faculty of Dentistry Hospital were evaluated retrospectively. Data from 75 patients aged between 1 and 65 years between January 2017 and June 2019 were used. Results: The age range of 75 patients included in the study was between 1-65 and the ratio of male patients to women was 2.12. The duration of the intervention ranged from 10 minutes to 8 hours 35 minutes with a total duration of 147 hours 18 minutes. The most intubation method was classic nasal intubation with 55 patients, followed by video laryngoscope nasal intubation with 6 patients and oral intubation with 14 patients. Among the procedures, the most common procedure was cyst / tumor surgery with 47 cases, the least procedures were treatment of alveolar cleft and temporomandibular joint with 1 case each. The length of hospital stay was 1-day as most common, daily hospitalization accounted for 13 of the cases. Conclusion: General anesthesia practice in the field of oral, dental and maxillofacial surgery is becoming more common. In general, general anesthesia is a serious necessity for complicated cases that cannot be performed under local anesthesia such as treatment of maxillofacial fractures, surgical procedures of large cysts and tumors, treatment of congenital or acquired disorders such as cleft lip and jaw deformities.
