Bruselloz tanısı konulmuş hastalardan izole edilen Brucella suşlarının PZR yöntemi ile sınıflandırılması

Özet
Bu çalışmaya Konya Eğitim Araştırma Hastanesine 2007-2014 tarihleri arasında yüksek ateş, lenfadenopati, halsizlik ve bel ağrısı gibi şikayetler nedeni ile başvuran hastalardan izole edilen 113 adet Brucella spp. suşu dahil edilmiştir. İzolatların kimyasal özelliklerine göre sınıflandırılmasında kullanılan testlerin spesifite ve sensitivite düzeylerinin belirlenmesi amaçlanmıştır. Suşların doğrulamasında üreme özellikleri, koloni morfolojileri, hemoliz, gram boyama sonuçları, pozitif serum ile lam aglütünasyon, katalaz, oksidaz, hareket, indol ve üreaz aktiviteleri değerlendirildi. Brucella spp. olarak değerlendirilen 113 izolatın CO2'li ortamda üreme, üreaz üretimi, H2S üretimi, Tiyonin ve fuksin varlığında üreme, Anti-A ve Anti-M serumları ile aglütünasyon testleri türlerin belirlenmesinde kullanıldı. Sonuçlar PZR sonuçları ile karşılaştırılarak değerlendirildi. Biyotip düzeyindeki sınıflandırma amacı ile biyokimyasal test sonuçları ve PZR sonuçları ortak olarak değerlendirildi. B. abortus olarak değerlendirilen 19 adet suşdan 3 adet suş biyotip 3; 4 adet suş biyotip 2; diğer 12 suş ise biyotip 1 olarak değerlendirildi. B. melitensis olarak değerlendirilen 94 adet suşdan 12 adet suş biyotip 1 diğer 82 adet suş ise biyotip 3 olarak değerlendirildi. Klasik sınıflandırma yöntemlerinin spesifite ve sensitivite değerleri boya varlığında üreme sensitivite % 100, spesifite % 84.21; CO2'li ortamda üreme sensitivite % 96,809 spesifite % 100; H2S üretimi sensitivite % 100 spesifite % 100; Anti-A ve Anti-M serumları ile aglütünasyon sensitivite %98,936 spesifite % 92.98 olarak değerlendirildi. Bütün çalışmalar ilk izolasyon ürünleri ile yapıldı. Sonuçlar PZR sonuçları ile uyumlu olarak bulundu. Ancak genel değerlendirme için PZR sonuçları ile eş zamanlı değerlendirme yapılması önerildi.
This study included that 113 Brucella spp. strains isolated from patients presenting with symptoms like high fever, lymphadenopathy, fatigue and back pain at Konya Education and Research Hospital between 2007-2014. Aimed to determine the level of spesificity and sensitivity of the tests used to classify the isolates according to the chemical properties. Reproductive characteristics, colony morphology, hemolysis, Gram stain results, the rapid slide aglütination with positive serum, catalase, oxidase, motion, indole and urease activity were evaluated the verification of the strains. Brucella ssp. as assessed 113 strains of CO2 environment in growth, urease production, H2S production, Thionine and fuchsin growth in the presence of anti-A and anti-M aglütünasyo the serum and PCR results were evaluated species and biotypes reviews. The results were evaluated by comparing the results with PCR. Biochemical test results and PCR results were evaluated together with the aim of biotypes classification level. Of the 19 B. abortus isolates; 3 were identified as biotype 3, 4 were identified biotype 2 and 12 were idetified biotype 1. B. melitensis strain which evaluated 94 isolates 12 were identified as biotype 1and 82 were identified biotype 3. Reproduction in specificity and sensitivity values for the presence of classical classification method paint sensitivity 100%, specificity of 84.21%; CO2 environment on reproductive sensitivity 96.809% and specificity 100%; H2S production sensitivity 100% and specificity 100%; with Anti-A and Anti-M aglütination serum sensitivity 98.936% and specificity 92.98% was considered. All studies were performed with the first isolation products. The results were consistent with the results of PCR. However, the PCR results for the overall evaluation were suggested simultaneous evaluation.
