Hidradenitis suppurativa is a relapsing disease with a chronic course, tends to terminate with scarring and involves the cutaneous and subcutaneous tissue. It involves painful, inflamated nodules of the apocrine gland bearing areas of the body most commonly located at the axillary, inguinal and anogenital regions. The disease has a variable clinical course. Mild cases may present as recurrent isolated nodules, while in severe cases chronic inflammation may lead to scarring, functional impairment, and rarely to squamous cell carcinoma. Treatment options include topical and systemic antibiotics, oral retinoids, surgical excision, hormonal therapies and immunosuppressant agents. In this paper a case of a hidradenitis suppurativa in a 34 year old man with multiple site involvements and resistance to topical and systemic antibiotic therapy is reported
