Minimal Akımlı Anestezi İdamesinde İzofluran ve Propofolün Maliyetinin Karşılaştırılması

İzofluranla minimal akım anestezisinin propofolle intravenöz anesteziye göre maliyeti karşılaştırıldı. Çalışmaya alınan 40 olgu (ASA I-II) rasgele iki eşit gruba ayrıldı. Tüm hastaların anestezi indüksiyonu iv 4.5 mg.kgsup-1/sup tiyopental, 2 µg.kgsup-1/sup fentanil ve 0.5 mg.kgsup-1/sup atrakuryum ile yapıldı. Tüm hastalarda yüksek akımlı %100 Osub2/sub ile 15 dakika ventilasyondan sonra, %100 Osub2/sub ile akım 0.6 L.dksup-1/sup'ya azaltıldı. Anestezinin idamesi Grup I'de %1.5 izofluran, Grup II'de ise iv 6 mg.kgsup-1/sup saatsup-1/sup propofol infüzyonu ile sağlandı. İki gruba da gerektikçe ek fentanil ve atrakuryum verildi. EKG, kan basınçları, tidal volüm, solunum sayısı, inspiryum ve ekspiryum oksijen, COsub2/sub, izofluran basınçları veya oranları, ve SpOsub2/sub sürekli izlendi. Anestezinin idamesinde kullanılan izofluran ve propofol miktarları ml olarak ölçüldü ve maliyet hesabı yapıldı. Sonuçlar ANOVA, Mann-Whitney-U testlerinden uygun alanıyla değerlendirildi. Uyanma süresi Grup II'de kısaydı (Grup I: 14.93.71, Grup II: 6.952.32) (p0.01). Anestezi idamesindeki dakika ilaç maliyeti Grup I'de 75.392 TL iken Grup II'de 306.325 TL idi (p0.01). Sonuçta; her iki grupta hemodinami stabildi. Maliyet analizinde minimal akım idamesinde izofluran propofolden 4.06 kat ucuz bulundu.
The cost of propofol anesthesia was compared with isoflurane anesthesia in minimal-flow anesthesia. ASA I-II, 40 patients were allocated into two groups (n20). In both groups iv 4-5 mg.kg<sup>-1</sup> thiopentone, 2 µg.kg<sup>-1</sup> fentanyl and 0.5 mg.kg<sup>-1</sup> atracurium were used. After providing ventilation with high flow 100% O<sub>2</sub> for 15 minutes, the flow was reduced to 0.6 L.min<sup>-1</sup> . For maintanence 1.5 % isoflurane and iv 6 mg.kg<sup>-1</sup> h<sup>-1</sup> propofol infusion were used in Groups I and II, respectively. Fentanyl and atracurium were added in case of requirements. Continuous ETCO<sub>2</sub> SpO<sub>2</sub> ECG, FiO<sub>2</sub> isoflurane concentrations, tidal volume, ventilation rate, blood pressure were monitorised. The amount of isoflurane and propofol was measured in terms of ml and their cost were calculated. The results were analysed by ANOVA and Mann-Whitney-U tests. The recovery was faster in Group II than in Group I (6.95±2.32 min versus 14.9±3.71 min) (p<0.01). The minute costs of maintanences were 75.392 TL and 306.325 TL for groups I and II, respectively (p<0.01). Both groups were considered as hemodinamically stable. The cost of isoflurane used for minimal-flow maintanence was 4.06 times less expensive than propofol.
