İlaçla indüklenmiş uyku endoskopisi (DISE), Obstrüktif Uyku Apne Sendromuna (OSAS) yol açabilecek üst hava yolları hakkında eşsiz bir değerlendirme imkanı sunmaktadır. Hem fiberoptik bronkoskopide (FOB) hem de DISE'de benzer sedatif ajanlar kullanıldığından FOB işlemi sırasında üst solunum yollarının fonksiyonel ve anatomik patolojiler açısından değerlendirilebileceği düşünülmektedir. Kliniğimizde rutin FOB uygulanan hastalarda bu amaçla polisomnografi skorlaması ile üst hava yolu skorlaması arasındaki ilişkiyi araştıran prospektif bir çalışma yapılması amaçlandı. FOB endikasyonu olan 50 hasta (13 kadın, 37 erkek) çalışmaya alındı. Bir anestezi uzmanı tarafından uygulanan sedasyon protokolü sonrasında FOB uygulandı ve üst hava yolu iki ayrı Kulak Burun Boğaz uzmanı tarafından değerlendirildi. DISE sırasında sedasyon titrasyonu için bispektral indeks (BIS) kullanıldı. Üst hava yolu obstrüksiyonunu değerlendirmek için NOHL sınıflaması (Burun-Orofarenks-Hipofarenks-Larenks sınıflaması) kullanıldı. Tüm hastalara FOB işlemi sonrasında polisomnografi uygulandı ve apne hipopne indeksi (AHI) hesaplandı. Artmış AHI ile burun, orofarenks, hipofarenks ve larenks tıkanıklığı arasında istatistiksel olarak anlamlı bir ilişki olduğunu gösterdik (sırasıyla p = 0,011, p <0,001, p <0,001, p = 0,008). AHI özellikle orofarenks ve hipofarenks obstruksiyonu ve aynı zamanda multi-level obstruksiyonu olan hastalarda belirgin olarak daha yüksekti. Bu küçük ölçekli çalışma bize rutin fleksibl bronkoskopi sırasında yapılan üst solunum yolu değerlendirmesinin obstrüktif uyku apne sendromu için prediktif değeri olduğunu göstermiştir.
Drug-induced sleep endoscopy (DISE) offers an unique evaluation of the upper airway pathology, that can lead to Obstructive Sleep Apnea Syndrome (OSAS). In both fiberoptic bronchoscopy (FOB) and DISE, similar sedative agents are used so during the FOB procedure, it is thought that the upper airways can be evaluated in terms of functional and anatomic pathology. For this purpose, in patients who are undergoing routine FOB in our clinic, a prospective study, investigating the relationship of polysomnography scoring and upper airway scoring, is intended to be done. Fifty patients (13 female, 37 male) with FOB indications were included in the study. After the sedation protocol applied by an anesthesiologist, FOB was performed and the upper airway was assessed by two separate otolaryngologists. We used bispectral index for sedation titration during DISE. NOHL classification (The Nose Oropharynx Hypopharynx and Larynx classification) was used to evaluate upper airway obstruction. All patients underwent polysomnography after the FOB procedure and the apnea-hypopnea index (AHI) was calculated. We have demonstrated a statistically significant association between increased AHI and nose, oropharynx, hypopharynx and larynx obstruction (respectively p=0,011, p<0,001, p<0,001, p=0,008). Particularly the AHI was significantly higher in patients with oropharynx and hypopharynx collapse and also multilevel collapse. This small-scale study showed us that upper respiratory pathway evaluation during routine flexible bronchoscopy is a predictive value for obstructive sleep apnea syndrome.
