Özet
Son yıllarda diyabet prevelansı dünya çapında giderek artmaktadır. Bu çalışmamızın amacı Tip2 Diyabetes Mellitus tanısı olup oral antidiyabetik kullanan, aralıklı oruç beslenme modeli uygulanan hastalarda ve sağlıklı erişkinlerde, başlangıç ve 3. aydaki sSt2, galektin-3, TNFα ve TGF-β seviyelerinde ki değişim arasında bir ilişki olup olmadığını incelemektir. Çalışmamız Haziran 2023-Nisan 2024 tarihleri arasında Selçuk Üniversitesi Tıp Fakültesi İç Hastalıkları Anabilim Dalı Endokrnoloji ve Metabolizma Hastalıkları Bilim Dalı Polikliniğinde Tip2 Diyabetes Mellitus tanılı oral antidiyabetik kullanan aralıklı oruç diyeti uygulayan gönüllüler ile yapıldı. Çalışmaya 18-65 yaş arası Tip2 Diyabetes Mellitus tanısı almış oral antidiyabetik kullanan, aralıklı oruç diyeti uygulanan hastalar ve gönüllü bireyler olmak üzere 44 hasta dâhil edildi. Aralıklı oruç diyeti öncesinde ve diyetin bitim zamanı olan 3. Ayda sST2, Galektin-3, TGF-β, TNF-α serum düzeyi çalışıldı. Araştırma gruplarının serum TGF-β değerleri incelendiğinde bu parametrenin DM'li gruplarda kontrol değerlerine göre yüksek olmasına rağmen istatistik bir anlamlılık yoktu. Ancak 3 aylık aralıklı beslenme sonucu hem 16 saat açlık grubu hem de 5/2 diyeti uygulanan grupta başlangıç değerlerine göre önemli bir artış gösterdiği belirlendi. Galektin-3 değerleri de başlangıçta diyabetli gruplarda kontrol grubuna göre daha yüksekti. Fakat belirli dönem açlık ve diyete bağlı olarak belirgin bir azalma gösterdi. Bu azalma 16 saat açlık grubunda daha yüksekti. Galektin-3 ve TGF-β değerlerinin her ikisinde de uygulama sonunda gruplar arasında farklılık yoktu. Araştırma gruplarının serum TNF-α değerleri uygulama başlangıcında DM'li gruplarda kontrol grubundan daha yüksek olarak tespit edildi. Ancak diyet ve aralıklı açlık uygulamasıyla birlikte bu parametrenin azalma göstermiş olduğu görüldü. Yine aralıklı açlık ve diyet uygulaması gerçekleştirilen 2. ve 3. gruplarda TNF-α seviyelerinin uygulama sonucu belirgin bir azalma gösterdiği tespit edildi. Araştırma gruplarının serum sST2 değerleri araştırmanın başlangıcında DM'li grupta kontrol değerine göre daha yüksekti. Ancak aralıklı açlık ve 5/2 diyet uygulamasına bağlı olarak uygulama sonunda belirgin bir azalma görüldü. Bu azalma uygulama yapılan 2. ve 3. gruplarda, grup içerisinde de belirlendi. Araştırmanın sonuçlarına genel olarak bakıldığında Tip2 Diyabetes Mellitus tanılı hastalarda uygulalan aralıklı açlık ve diyet modelinin yangısal süreci belirli bir oranda baskıladığını göstermektedir.
In recent years, the prevalence of diabetes has been increasing worldwide. Our study aims to examine whether there is a relationship between the changes in sSt2, galectin-3, TNFα and TGF-β levels at the beginning and the 3rd month in patients diagnosed with Type 2 Diabetes Mellitus, using oral antidiabetics, following an intermittent fasting nutrition model, and healthy adults. Our study was conducted between June 2023 and April 2024, at the Selçuk University Faculty of Medicine, Department of Internal Medicine, Endocrinology and Metabolism Diseases Polyclinic, with volunteers diagnosed with Type 2 Diabetes Mellitus, using oral antidiabetics, and following an intermittent fasting diet. The study included 44 patients between the ages of 18-65, diagnosed with Type 2 Diabetes Mellitus, using oral antidiabetics, patients on an intermittent fasting diet, and volunteers. Serum levels of sST2, Galectin-3, TGF-β, and TNF-α were measured before the intermittent fasting diet and at the end of the diet in the 3rd month. When the serum TGF-β values of the research groups were examined, although this parameter was higher in the DM groups than the control values, there was no statistical significance. However, as a result of 3 months of intermittent feeding, it was determined that there was a significant increase compared to the initial values in both the 16-hour fasting group and the 5/2 diet group. Galectin-3 values were also higher in the diabetic groups than in the control group at baseline. However, it showed a significant decrease due to hunger and diet for a certain period. This decrease was higher in the 16-hour fasting group. There was no difference between the groups in both Galectin-3 and TGF-β values at the end of the application. Serum TNF-α values of the research groups were determined to be higher in the DM groups than in the control group at the beginning of the application. However, it was observed that this parameter decreased with diet and intermittent fasting. It was also determined that TNF-α levels showed a significant decrease in the 2nd and 3rd groups, where intermittent fasting and dieting were applied. Serum sST2 values of the research groups were higher in the DM group than the control value at the beginning of the study. However, due to intermittent fasting and 5/2 diet application, a significant decrease was observed at the end of the application. This decrease was also determined within the group in the 2nd and 3rd groups where the application was made. A general look at the results of the research shows that the intermittent fasting and diet model applied in patients diagnosed with Type 2 Diabetes Mellitus suppresses the inflammatory process to a certain extent.
