Özet
Amaç: Multipl sklerozlu(MS) hastalar immobilite ve
kortikosteroid kullanımları nedeni ile osteoporoz için risk
altındadırlar. Bu çalışmanın amacı MS’li hastalarda
kemik mineral yoğunluğunu (KMY) etkileyen faktörleri
araştırmaktı.
Hastalar ve yöntem: Çalışmaya 42 MS’ li ( 10 erkek, 32
kadın) hasta ve 28 kontrol olgusu (7 erkek, 21 kadın)
alındı. KMY’leri ve kemik döngüsü markırları (serum
kalsiyum, fosfor, alkalen fosfataz, osteokalsin, idrar Ca,
kreatinin ve deoksipridinolin) değerlendirildi.
Bulgular: Kemik döngüsü markırları her iki grupta da
benzerdi. KMY’leri MS’li hastalarda kontrol grubuna
göre anlamlı derecede düşüktü (p>0.05). MS’li
hastaların fonksiyonel durumlarını değerlendirmek için
kullanılan genişletilmiş özürlülük skalası (EDSS) skoru
ile KMY arasında ilişki bulunamadı (r=0.128). Yine
hastalığın tanı süresi ile KMY arasında ilişki yoktu
(r=0.183). Kortikosteroid kullanan hastaların KMY,
steroid kullanmamış hastalarınkinden anlamlı derecede
düşüktü. Fakat farklı kümülatif kortikosteroid dozları ile
KMY’leri arasında ilişki bulunamadı
Purpose: Patients with multiple sclerosis (MS) are at risk for osteoporosis because of immobility and corticosteroid use. The aim of this study was to determine the factors affecting bone mineral density (BMD) in MS patients. Patients and method: Forty-two patients with MS (10 male, 32 female) and 28 (7male, 21 female) controls were enrolled into the study. BMD and bone turn-over markers (serum calcium, phosphorus, alkaline phosphatase, osteocalcin and urine calcium, creatinine, deoxypyridinoline) were evaluated. Results: Bone turn-over markers were similar in both groups. BMD were significantly lower in MS patients compared to the control group (p=0.001). There was no correlation between functional status of the patients that were evaluated by expanded disability status scale (EDSS) and BMDs (r=0.128). No correlation was found between diagnosis duration of the disease and BMDs (r=0.183). Femoral and lumbar BMD were significantly lower in patients receiving steroids when compared with patients who have not received steroids; however, there was no correlation between different steroid doses and BMDs.
